Araştırma, gebe kalma şeklinin çocuklarda hipospadias oluşumu
üzerine etkisinin belirlenmesi amacıyla gerçekleştirildi.
Gereç ve Yöntem: Tanımlayıcı tipte yürütülen araştırmanın
örneklemini, Ankara Üniversitesi Tıp Fakültesi Cebeci Hastanesi Çocuk
Cerrahisi ve Çocuk Ürolojisi Klinik ve Polikliniği’ne başvuran ya da Çocuk
Cerrahisi ve Çocuk Ürolojisi Servisi’ nde yatan, çalışmaya katılmayı kabul
eden, 18 yaş üzeri ve onamları alınan, işitme ve iletişim problemi olmayan
spontan gebelik sonucu doğan 166 ve yardımcı üreme tekniğiyle (YÜT) gebelik
sonucu doğan 27 hipospadiaslı çocuğun anneleri oluşturdu. Araştırmaya dahil
edilecek annelere araştırmacı tarafından literatür bilgisi doğrultusunda
hazırlanan Sosyodemografik Özellikleri Belirleme Formu ve YÜT’ e Đlişkin
Veri Toplama Formu kullanılarak anket uygulandı. Araştırmaya dahil edilen
bütün annelere Sosyodemografik Özellikleri Belirleme Formu dolduruldu.
Hipospadiaslı çocuğuna Yardımcı Üreme Tekniği (YÜT) ile gebelik oluşumu
sonucunda sahip olan annelere YÜT’e Đlişkin Veri Toplama Formu dolduruldu,
spontan gebelik sonucunda sahip olan annelere başka herhangi bir form
doldurulmadı. Verilerin analizinde Student-t testi, Mann Whitney U testi, Ki kare ve Fisher Exact testleri kullanıldı.
Bulgular: Araştırmanın örneklemine dahil edilen hipospadiaslı
çocukların %86.0’ ının spontan gebelik sonucu, %14.0’ ının YÜT sonucu
doğduğu bulundu. Đleri anne ve baba yaşı, evlilik süresi ve annede jinekolojik
hastalık varlığı YÜT’ lü grupta istatistiksel olarak anlamlı yüksek bulundu
(p<0.05). Hipospadiaslı çocuğun doğum haftası, doğum ağırlığı, doğumdaki
boyu ve annenin gebelik sayısı spontan gebelik grubundaki annelerde anlamlı
olarak yüksek bulundu. Doğum şekli açısından YÜT’ lü grupta zorunlu
sezaryen oranı anlamlı yüksek bulundu (p<0.05). Hipospadiaslı çocuklara eşlik

eden ürogenital anomali açısından YÜT’ lü grupta anlamlı yüksek bulundu
(p=0.036).
Sonuç ve Öneriler: Literatürle de uyumlu olarak çalışmamızda
genel olarak hipospadiasa yol açan faktörler açısından net bir sonuca
ulaşılamadı. Yine literatürle paralel olarak YÜT’ ün hipospadiasa kesin etkisi
vardır gibi bir sonuca ulaşılamadı. Bu bağlamda gebe kalma şeklinin
hipospadias oluşumunda nasıl bir rol oynadığının daha iyi anlaşılabilmesi için
benzer araştırmaların çok merkezli bir planlama yapılarak, daha geniş
örneklem grupları ile uygulanması önerilebilir
The aim of this study is to determine the effect of conception on
hypospadias formation in children.
Materials and Methods: The sample of this descriptive study
consisted of mothers who applied to Pediatric Surgery and Pediatric Clinic and
Polyclinic of Cebeci Hospital, Ankara University School of Medicine or mothers
of 166 children who were admitted to the Pediatric Surgery and Pediatric
Urology Service, who agreed to participate in the study, who were over 18
years of age and whose consent was obtained and who were born as a result of
spontaneous pregnancy without hearing and communication problems and
biological mothers of 27 children with hypospadias born as a result of
pregnancy by using Assisted Reproductive Technique ( ART). A questionnaire
was applied to the mothers to be included in the study using the
Sociodemographic Characteristics Form and Data Collection Form about ART
which were prepared by the researcher in accordance with the literature.
Sociodemographic Characteristics Form was filled out for all mothers included
in the study. The mother of children with hypospadias, who had pregnancy by
using Assisted Reproductive Technique, filled out a Data Collection Form
regarding ART, however, no form was given to the mothers who had
spontaneous pregnancy. Student-t, Mann Whitney U test, Chi-square and
Fisher Exact tests were used for data analysis.
Results: It was found that 86.0% of the children with hypospadias
included in the study sample were born as a result of spontaneous pregnancy
and 14.0% of them as a result of ART. Advanced maternal and paternal age,
duration of marriage and presence of maternal gyneocological disease were
found statistically higher in ART group (p<0.05). The birth week, birth weight,
birth lenght of the children with hypospadias and number of pregnancies of
the mothers were significantly higher in the mothers who are in the

spontaneous pregnancy group. Interms of birth type, mandatory cesarean rate
was found to be significantly higher in the ART group (p<0.05). Urogenital
anomaly in children with hypospadias was found to be significantly higher in
ART group (p=0.036).
Conclusion and Proposals: In accordance with the literature, in
our study, a clear result was not reached in terms of the factors causing
hypospadias in general. Also, in parellel with the literature, ART has no
definite effect on hypospadias. In this context, it may be suggested that similar
studies should be conducted with a larger sample group by making a multi center planning in order to beter understand the role of conception type in the
formation of hypospadias
