Testis torsiyonu, spermatik kordun ve içindeki anatomik yapıların
dönmesiyle karakterize ürolojik acil bir durumdur. Torsiyon bölgeye giden kan
akımının azalması ya da tamamen kesilmesiyle meydana gelmektedir.
Ergenlik dönemindeki erkeklerde en sık görülen genital hastalıkların başında
gelmektedir ve ipsilateral testis disfonksiyonu olmadan mutlaka en kısa
sürede tedavi edilmelidir. Testis torsiyonu dokuda iskemik hasara sebep
olurken, detorsiyon ise doku hasarına asıl sebep olan reperfüzyon hasarını
oluşturmaktadır. Testis dokusunda meydana gelen hasar, torsiyonun
derecesine ve süresine bağlı olarak değişmektedir. Bu nedenle çalışmamızda
iskemi/reperfüzyon (İ/R) sonucunda oluşan doku hasarı üzerine antioksidan
bir madde olan kırmızı acı biberin etken maddesi Capsaicin’in (CAP) koruyucu
etkisini incelemeyi amaçladık.
Çalışmamızda Wistar Albino cinsi, 40 adet, 250-300 gr ağırlığında erişkin
erkek sıçan kullanıldı. Deney hayvanları her grupta 10 adet olmak üzere 4
gruba ayrıldı. Torsiyon, her iki testisin spermatik kordunun 720 derece saat
yönünün tersine çevrilmesiyle oluşturuldu. Detorsiyon, iskemi süresinden
sonra testisin eski pozisyonuna getirilmesiyle sağlandı. Sham grubuna bazal
değerlerin değerlendirilmesi için skrotal deri insizyonu yapılarak 4/0 sütürle
kapatıldı. 2 saat sonra sütürler alınarak orşiektomi yapıldı. İskemi grubunda
her iki testis saat yönünde 720 derece döndürülerek fikse edildi, 2 saat iskemi
sonrası orşiektomi yapıldı. İskemi-reperfüzyon grubunda iskemi grubunda
olduğu gibi iskemi yapılarak, 2 saat iskemi sonrası testislerin detorsiyonu
sağlandı, 2 saat reperfüzyon sonrası orşiektomi yapıldı. Capsaicin
uyguladığımız deney grubunda ise 2 saat torsiyondan sonra, detorsiyon
yapıldı. Detorsiyondan 30 dakika önce Capsaicin subcutan yolla 0.5 mg/kg
verildi. 2 saat reperfüzyon işlemi sonrası orşiektomi uygulandı. Genel anestezi
altında hayvanlardan alınan intrakardiyak kan örneklerinde, lipit
peroksidasyonunun belirlenmesi için malondialdehit (MDA), süperoksit
dismutaz (SOD) ve katalaz (KAT) antioksidan enzim düzeyleri değerlendirildi.

Testis dokusu ise histopatolojik değerlendirme için hematoksilen/eozin ile
boyandı ve ışık mikroskobunda incelendi.
Yapılan analizler neticesinde, Capsaicin grubuna ait sıçan testislerinde
iskemi ve iskemi/reperfüzyon grubuna oranla azalmış tübüler hasar, normale
yakın yapıdaki seminifer tübüller ve tübül duvarındaki spermatogenik hücre
serileri görülmüş ayrıca intersitisyel alanda azalmış ödem dikkat çekmektedir.
Lipid peroksidasyonunun belirlenmesi için değerlendirilen SOD, MDA ve KAT
enzim düzeylerinin ise Capsaicin uygulanan grupta kontrol grubu değerlerine
yaklaştığı gözlendi.
Sonuç olarak, testiste oluşturulan iskemi-reperfüzyon hasarı üzerine
Capsaicin’in koruyucu etkisi olduğu belirlendi
Testicular torsion is a urological emergency characterized by the
rotation of the spermatic cord and its anatomical structures. Torsion occurs
when the blood flow to the region decreases or is stopped completely. It is one
of the most common genital diseases in men in adolescence and it should be
treated as soon as possible without ipsilateral testicular dysfunction. While
testicular torsion causes ischemic damage in the tissue, detorsion causes
reperfusion damage, which is the main cause of tissue damage. Damage to the
testicle tissue varies depending on the degree and duration of the torsion.
Therefore, in our study, we aimed to investigate the protective effect of
Capsaicin, which is an active ingredient of red hot pepper, an antioxidant
substance, on tissue damage caused by ischemia / reperpusion.
In our study, 40 male, 250-300 g adult male rats of the genus Wistar
Albino were used. Experimental animals were divided into 4 groups, 10 in each
group. The torsion was created by rotating the spermatic cord of both testicles
counter clockwise by 720°. Detorsion was achieved by bringing the testicle
back to its original position after ischemia time. The scrotal skin incision was
made to evaluate the basal values for the sham group and closed with a 4/0
suture. Sutures were taken 2 hours later and orchiectomy was performed. In
the ischemia group, both testicles were fixed by rotating 720 degrees clockwise,
orchiectomy was performed after 2 hours of ischemia. In the ischemia reperfusion group, as in the ischemia group, ischemia was performed and
detorsion of the testicles was achieved after 2 hours of ischemia, orchiectomy
was performed after 2 hours of reperfusion. In the experimental group where
we applied Capsaicin, detortion was performed after 2 hours of torsion.
Capsaicin was administered subcutaneously 0.5 mg / kg 30 minutes before
detortion. Orchiectomy was performed after 2 hours of reperfusion. Under
general anesthesia, intracardiac blood samples from the animals for the
determination of lipid peroxidation, superoxide dismutase (SOD),
malondialdehyde (MDA) and catalase (CAT) antioxidant enzyme. Testicular

tissue was stained with hematoxylin / eosin for histopathological evaluation
and examined under a light microscope.
As a result of the analyzes, the rat testicles belonging to the Capcaicin
group showed reduced tubular damage and seminiferous tubules in the
structure similar to ischemia and spermatogenic cell series in the tubular wall,
and decreased edema in the interstitial area. SOD, MDA and CAT enzyme
levels evaluated for the determination of lipid peroxidation were observed to be
close to the control group values in the Capcaicin administered group.
Consequently, Capsaicin had a protective effect on ischemia-reperfusion
injury in the testicle
