Klinik örneklerden soyutlanmış Acinetobacter baumannii suşlarının antibiyotik
direnç profillerinin belirlenmesi hastanelerde dirençli suşların yayılımınının
kontrolünde büyük öneme sahiptir. Ayrıca klinisyenlere antimikrobiyal seçiminde
yol gösterici olmaktadır. Otomatize sistemler klinik mikrobiyoloji laboratuvarları
tarafından antimikrobiyal duyarlılık testleri için yaygın olarak kullanılmaktadır.
Ne yazık ki, otomatize sistemlerin A. baumannii suşlarında kolistin duyarlılığını
belirlemede güvenilir olmadığı bildirilmektedir. Bu nedenle otomatize sistemlerle
saptanan sonuçlarının doğrulanmadan rapor edilmemesi gerekmektedir.
Günümüzde kolistin çoğul dirençli suşların neden olduğu enfeksiyonların
tedavisinde en etkili antibiyotik olmakla birlikte, dünyanın her yerinden kolistine
dirençli suşlar bildirilmektedir. Yeni dirençli izolatların ortaya çıkmasını önlemek
için kolistin monoterapisinden kaçınılması önerilmektedir.
Bu çalışma; Eylül 2019 – Ağustos 2020 tarihleri arasında Eskişehir Osmangazi
Üniversitesi Tıp Fakültesi Tıbbi Mikrobiyoloji Anabilim Dalı Bakteriyoloji
Laboratuvarı’nda yürütülmüştür. Kan örneklerindeki üremeler BACTEC (BD
BACTEC™ FX) sürekli moniterize kan kültür sistemi ile saptandı. Suşlar Phoenix
(BD Phoenix™ M50) otomatize bakteri identifikasyon sistemi ile tanımlandı ve
antimikrobiyal duyarlılık sonuçları elde edildi. En az üç antibiyotik sınıfına direnç
gösteren toplam 92 adet A. baumannii kompleks izolatı çalışmaya dâhil edildi.
Ayrıca BD Phoenix™ M50 cihazı tarafından saptanan kolistin duyarlılık
sonuçları(hem duyarlı hem de dirençli suşlarda) referans yöntem olan sıvı
mikrodilüsyon yöntemi ile doğrulandı. Çalışmamızın son aşamasında da 50 çoğul
dirençli A.baumannii kompleks suşuna karşı kolistin/sulbaktam
kombinasyonunun etkinliği dama tahtası yöntemiyle araştırıldı.
Bu çalışmada toplam 92 izolatın 10’unun (%10,9) referans yöntem ile kolistine
ve 80 (%86,9) izolatın da sulbaktama dirençli olduğu saptandı. Referans yöntem
ile kolistine dirençli olarak saptanan 10 izolatın sadece 2’si otomatize sistemle
dirençli olarak, 8 izolatın ise otomatize sistemi tarafından duyarlı olarak rapor

edildiği gözlendi. Dolayısıyla Phoenix M50 otomatize sisteminin dirençli izolatlar
arasında Çok büyük hata oranı 8/10 olarak belirlendi. Kolistin/sulbaktam
kombinasyonunun etkinliğinin saptanmasında elde edilen FİK (fraksiyonel
inhibisyon konsantrasyonu) sonuçları değerlendirildiğinde izolatların 6’sında (%12)
kolistin-sulbaktam kombinasyonu sinerjistik etki gösterirken, 44 (%88) izolatta
aditif etkileşim olduğu belirlendi. Bu çalışmada kolistin-sulbaktam kombinasyonuyla
antagonistik etkileşim saptanmadı.
Klinik mikrobiyoloji laboratuvarlarında antibiyotik duyarlılık testleri için
günümüzde yaygın olarak kullanılan otomatize sistemlerle bazı antibiyotikler için
hatalı duyarlılık sonuçları alınabileceği unutulmamalıdır. A.baumannii suşları
otomatize sistemler tarafından saptanan MİK sonuçlaına karşı mutlaka referans
yöntem olan sıvı mikrodilüsyon yöntemiyle doğrulanmalıdır.
Ayrıca her ne kadar kolistin-sulbaktam kombinasyon etkinliği sonuçlarında
aditif etki saptamış olsak da gelecekte A.baumannii izolatlarında kolistin
direncinin yaygınlaşmasını geciktirmesinde A.baumannii suşlarının etkin olduğu
bakteremi gibi ciddi enfeksiyonlarının tedavisinde kolistinin tek başına
kulanımından kaçınılması kanısına varılmıştır
The determination of antibiotic resistance profiles of Acinetobacter baumannii
strains isolated from clinical samples is of great importance in controlling the
spread of resistant strains in hospitals. It is also guiding clinicians in the selection
of antimicrobials. Automated systems are widely used by clinical microbiology
laboratories for antimicrobial susceptibility tests. Unfortunately, automated
systems are known to have no reliability for colistin susceptibility in A. baumannii
strains. Therefore, the results determined by automated systems should not be
reported without verification. Although colistin is currently the most effective
antibiotic in the treatment of infections caused by multiple resistant strains,
colistin resistant strains have been reported from all over the world. It is
recommended to avoid colistin monotherapy to prevent the emergence of new
resistant isolates.
This work was conducted in Eskişehir Osmangazi University, Faculty of
Medicine, Medical Microbiology Laboratory between September 2019 - August
2020. Growth of blood samples was detected with the BACTEC (BD BACTEC ™
FX) continuous moniterized blood culture system. Strains were identified with the
Phoenix (BD Phoenix ™ M50) automated bacterial identification system and
antimicrobial susceptibility results were obtained. A total of 92 A. baumannii
complex isolates resistant to at least three antibiotic classes were included in the
study. In addition, colistin susceptibility results (in both susceptible and resistant
strains) detected by the BD Phoenix ™ M50 device were confirmed by the reference
method; broth microdilution In the last phase of our study, the effectiveness of
colistin/sulbactam combination against 50 multi-resistant A.baumannii complex
strains was investigated by checkerboard method.
In this study, 10 of 92 isolates (10.9%) were found to be resistant to colistin with
reference method and to sulbactam in 80 (86.9%) isolates. It was observed that 2 of
10 isolates detected as resistant by the reference method appeared resistant in the
automated system and 8 of them were reported as susceptible in the automated

system. Therefore, the very large error rate among resistant isolates of the Phoenix
M50 automated system was determined as 8/10. When the FIC (fractional
inhibition concentration) results obtained in the determination of the effectiveness
of the colistin/sulbactam combination were evaluated, it was determined that while
the colistin-sulbactam combination showed a synergistic effect in 6 (12%) of the
isolates, an additive interaction was found in 44 (88%) isolates. In this study, no
antagonistic interaction was found with the colistin-sulbactam combination.
It should be kept in mind that erroneous sensitivity results may be obtained for
some antibiotics with automated systems, which are widely used today for
antibiotic susceptibility tests in clinical microbiology laboratories. A.baumannii
strains must be verified by the reference method, broth microdilution, against MIC
results detected by automated systems.
Although we found an additive effect in the results of colistin-sulbactam
combination activity, it was concluded that the use of colistin alone should be
avoided in the treatment of serious infections such as bacteremia, in which
A.baumannii strains are effective in delaying the prevalence of colistin resistance
in A.baumannii isolates in the future
