Erken boşalma (EB), 1
dakikadan daha kısa intravajinal boşalma süresi ile karakterize olan, erkeklerde en sık
görülen cinsel işlev bozukluğudur. Bu durum kişinin cinsel aktif olması ile ortaya
çıkan yaşam boyu EB ve daha önce iyi bir boşalma kontrolüne sahipken yaşamının
ilerleyen dönemlerinde gelişen edinsel EB olarak sınıflandırılır. Yaşam boyu EB’nin
genetik yatkınlıkla ilişkili nörobiyolojik bir işlev bozukluğu olduğu ve merkezi
serotonin nörotransmisyonu ve serotonin reseptörlerinde fonksiyon bozukluğu ile
ilişkili olduğu düşünülmektedir. Çalışmamızda, serotonin reseptörü alt tipleri olan 5-
HT1A, 5-HT1B ve 5-HT2C'yi kodlayan genlerde meydana gelen dört tek nükleotid
polimorfizminin yaşam boyu EB etiyolojisindeki rolü araştırılmıştır. Bu amaçla
22.06.2018-04.08.2019 tarihleri arasında Eskişehir Osmangazi Üniversitesi Tıp
Fakültesi Hastanesi Üroloji Anabilim Dalı ve Psikiyatri Anabilim Dalı Cinsel İşlev
Bozuklukları Polikliniği’ne EB şikayeti ile başvuran ve dahil edilme kriterlerini
karşılayan 100 hasta ile yaş bakımından eşleştirilmiş 100 sağlıklı kontrol çalışmamıza
dahil edilmiştir. Katılımcılardan yaş, eğitim durumu, medeni durum, çalışma durumu
ve boşalma süresi verileri elde edilmiş, ardından DNA eldesi için kan örneği alınmıştır.
Çalışmamızda 5-HT1B reseptör genindeki rs11568817 polimorfizmi ile yaşam boyu
EB arasında anlamlı bir ilişki olduğu saptanmıştır (p= 0,019). 5-HT2C reseptör
geninde meydana gelen rs518147 polimorfizmi ile yaşam boyu EB arasında saptanan
istatistiksel olarak anlamlı ilişki çalışmamızın diğer bir önemli bulgusudur (p= 0,016).
Çalışmamızda 5-HT1A reseptör geni rs6295 polimorfizmi ve 5-HT2C reseptör geni
rs3813929 polimorfizmi ile yaşam boyu EB arasında istatistiksel olarak anlamlı ilişki
saptanmamıştır
Premature ejaculation (PE) is the most common male sexual
disfunction which is characterized by persistent intravaginal ejaculation latency times
of less than 1 minute. The condition is classified as either lifelong premature
ejaculation, beginning when a man first becomes capable of functioning sexually, or
acquired PE, which means that a man has previously experienced an acceptable level
of ejaculatory control but develops the condition later in life. It has been postulated to
be a neurobiological dysfunction with genetic vulnerability and is related to
disturbances of central serotonin neurotransmission and serotonin receptor function.
The aim of the present study is to investigate the effects of four single nucleotide
polymorphisms (SNPs) located in genes encoding serotonin receptor subtypes 5-
HT1A, 5-HT1B, and 5-HT2C in lifelong PE. For this aim, 100 patients who were
eligible for the inclusion criterias and applied to Eskişehir Osmangazi University
Hospital Urology Department Outpatient Clinic and Psychiatry Department Sexual
Dysfunctions Outpatient Clinic between the dates 22.06.2018-04.08.2019 for PE
complaint, and age matched 100 healthy control subjects were included in the study.
Data from age, education, marital status, working status and ejaculation time were
obtained from the participants, followed by blood samples for DNA extraction.
Significant relationship between rs11568817 polymorphism in 5-HT1B receptor gene
and lifelong PE was found (p = 0,019). Another significant finding was the relationship
between rs518147 polymorphism occurring in the 5-HT2C receptor gene and lifelong
PE (p = 0,016). In our study, the relationship between 5-HT1A receptor gene rs6295
polymorphism and 5-HT2C receptor gene rs3813929 polymorphism and lifelong PE
was not found to be statistically significant
