Çalışmamızın amacı, inme tanısı almış hastaların hastaneye başvurularındaki gecikmelerin nedenlerini araştırmaktı. 01.03.2018-28.02.2019 tarihleri arasında ESOGÜ acil servisine başvuran yeni tanı inme hastalarını içeren ileriye dönük gözlemsel bir çalışmadır. Çalışmaya dahil edilen hastaların %56.6‘sı (n=296) erkekti. Hastaların yaşları 19-107 arasında değişmekte olup ortalama±std 68,4±13,0 yıl olarak saptandı. Tüm olguların %83,6‘sına (n=437) iskemik inme, %7,5‘ine (n=39) hemorajik inme, %9‘una da (n=47) GĠA tanısı kondu. Hastaların %22,2‘sine (n=116) rtPA uygulandı, %10,3‘ü (n=54) endovasküler girişime alındı. Toplamda 436 hastanın hastane öncesi süreleri analiz edildi. Sürelere bakıldığında semptomları farketme süresi ortalama±std 130,7±10,8 dk, semptomları farkettikten sonra hastaneye varış süresi ortalama±std 150,6±12,1 dk, semptom kapı zamanı ise ortalama±std 362±400 dk olarak bulundu. Üniversite ve üstü eğitim alanlarda ort±std 90,8±7,0 dk (p<0,001) olarak tespit ettik. Eğitim düzeyi arttıkça hastaneye geliş sürelerinin kısaldığını gördük. NIHSS skorunun yüksek olan hastaların sürelerinin anlamlı olarak daha kısa olduğu ve iki değişken arasında pozitif korelasyon olduğu saptandı (Spearman‘s ρ= -0,315, p<0,001). Semptom-kapı zamanının uzamasında tek yaşamanın (p<0,001), 112 acil servisi aktive etmemenin (p<0.001), yakınlarının inme hakkındaki bilgi eksikliğinin (p<0,001) etkili olduğunu gördük. Bilgi kaynağı olarak halk semineri (p=0,030) ve sağlık personeli (p=0,003) cevabı verenlerin, arkadaş/akraba cevabını verenlere göre sağlık ekibini arama sürelerinin anlamlı bir şekilde daha kısa olduğunu gördük. Bulgularımız toplumda inme farkındalığının oluşması ve risk faktörlerininin bilinmesi ile inmeye daha erken müdahale şansının olduğunu gösterdi
The aim of our study was to investigate the causes of delays
in patients admitted to hospital with stroke diagnosis. It is a prospective
observational study including newly diagnosed stroke patients admitted to
ESOGÜ emergency department between 01.03.2018-28.02.2019. 56.6% (n =
296) of the patients were male. The age of the patients ranged from 19-107
years with a mean of 68.4 ± 13.0 years. 83.6% (n = 437) were diagnosed as
ischemic stroke, 7.5% (n = 39) were hemorrhagic and 9% (n = 47) were
diagnosed as TIA. rtPA was applied to 22.2% (n = 116) of the patients, and
10.3% (n = 54) of them underwent endovascular intervention. Pre hospitalization time of 436 patients was analyzed. Mean duration of symptom
recognition was 130.7 ± 10.8 min, duration of admission to hospital after
detection of symptoms was 150.6 ± 12.1 min. Mean time to symptom-door was
36.2 ± 400 min and also we found the symptom-door time 90.8 ± 7.0 min (p
<0.001) in university and higher education. As the level of education increased,
we found that the duration of hospitalization was shortened. The duration of the
patients with high NIHSS score was significantly shorter and there was a
positive correlation between the two variables (Spearmen‘s ρ = -0.315, p
<0.001). We found that single-life (p <0.001), non-using 112 emergency
services (p <0.001), and lack of knowledge about stroke (p <0.001) were
effective in prolonging symptom-door time. As a source of information, we found
that those who answered the ―public seminar‖ (p = 0.030) and ―health
professional care‖(p = 0.003) had significantly shorter time to call the 112
emergency services than those who gave the ―friend/relative‖ response. Our
findings showed that there was a chance of early intervention in stroke with the
awareness of stroke and the awareness of the risk factors
