Over kanseri jinekolojik maligniteler içerisinde en sık ölüme
sebep olan hastalıktır. Amerika Birleşik Devletleri verilerine göre 2018 yılında 22,240 yeni vaka tespit
edilmiş olup, 14,070 hasta over kanseri nedenli hayatını kaybetmiştir. Bu hastalığa yönelik etkin bir
tarama yöntemi mevcut değildir. Tarama yöntemi olmaması nedeniyle hastaların çoğu ileri evrede tanı
alabilmektedir. Hastalığın ileri evrede tanı alıyor olması ve genellikle ileri yaşta görülüyor olması primer
tedavi şekli olan cerrahi sitoredüksiyonu güçleştirmektedir. Bu nedenle neoadjuvan kemoterapi
uygulaması bazı hastalarda ilk tercih edilecek tedavi yöntemi olabilmektedir. Çalışmamızda temel
olarak ileri evre over kanseri olan hastalarda primer sitoredüktif cerrahi ile neoadjuvan kemoterapi
uygulanan hastaların sonuçları ve sağ kalım oranlarını karşılaştırmak amaçlanmıştır. Gereç ve Yöntem:
Çalışma 1 Ocak 2010 ve 31 Ağustos 2018 tarihleri arasında over kanseri nedeniyle Eskişehir Osmangazi
Üniversitesi Kadın Hastalıkları ve Doğum Ana Bilim Dalı Jinekolojik Onkoloji Bölümü’nde tedavi gören
hastaların kayıtları retrospektif olarak incelenmiştir. Çalışmaya over kanseri nedenli primer
sitoredüksiyon yapılan ve over kanseri nedenli neoadjuvan kemoterapi uygulandıktan sonra cerrahi
tedavi uygulanan ve kesin patolojik tanıları over karsinomu olan hastalar dahil edilmiştir. Belirtilen
kriterlere uyan 223 over kanserli hastanın kayıtları, operasyon bilgileri, patoloji sonuçları, klinik
takipleri ve laboratuvar sonuçları değerlendirilmiştir. Bulgular: Primer sitoredüksiyon grubunda 159
hasta, neoadjuvan kemoterapi grubunda ise 64 hasta yer almaktadır. Hasta grupları arasında yaş,
tümör belirteçleri ve hastalığın evresi açısından anlamlı farklılık vardır. Primer cerrahi uygulanan grupta
intraoperatif komplikasyon oranı %12,5 iken neoadjuvan kemoterapi grubunda %17,2 olarak
bulunmuştur (p=0,759). Hastaların postoperatif transfüzyon ihtiyacı ise ilk grupta %28,9, ikinci grupta
ise %21,8 olarak saptanmıştır (p=0,641). Birinci grupta hastanede yatış süresi ortalama 5,66 gün iken
ikinci grupta 5,92 gün olarak saptanmıştır (p=0,608). Hastaların optimal cerrahi oranları birinci grupta
%71,7, ikinci grupta ise %78,1 olarak bulunmuştur (p=0,498). Birinci grupta ortalama yaşam süresi 42,9
ay iken ikinci grupta bu süre 39,1 ay olarak saptanmıştır (p=0,044). Sonuç: Primer sitoredüktif cerrahi
yapılan hasta grubunda sağ kalım süresi ve hastalıksız yaşam süresi neoadjuvan kemoterapi grubuna
göre belirgin derecede arttığı tespit edilmiştir. İki grup arasında optimal cerrahi oranları ve
komplikasyon oranları açısından anlamlı fark bulunmamıştır. Fakat grupların homojen yapıda olmaması
yorum yapmayı güçleştirmektedir, bu konuda geniş tabanlı randomize kontrollü çalışmalara ihtiyaç
vardır
Aim: Ovarian cancer is the most common cause of death among gynecological malignancies. According
to the United States data, 22,240 new cases were identified in 2018, and 14,070 patients died due to
ovarian cancer. There is no effective screening method for this disease. Due to the lack of screening
methods, most patients can be diagnosed in advanced stages. The fact that the disease is diagnosed
at an advanced stage and usually seen at an advanced age complicates surgical cytoreduction, which
is the primary form of treatment. Therefore, neoadjuvant chemotherapy may be the first choice of
treatment in some patients. The aim of this study was to compare the results and survival rates of
primary cytoreductive surgery and neoadjuvant chemotherapy in patients with advanced ovarian
cancer. Material and Method: In this study, the records of patients treated for ovarian cancer between
January 1, 2010 and August 31, 2018 at Gynecological Oncology Department of Gynecology and
Obstetrics Department of Eskişehir Osmangazi University were examined retrospectively. Patients who
underwent primary cytoreduction due to ovarian cancer and underwent surgical treatment after
neoadjuvant chemotherapy were included in the study. The records, operation information, pathology
results, clinical follow-up and laboratory results of 223 ovarian cancer patients were evaluated.
Results: There were 159 patients in the primary cytoreduction group and 64 patients in the
neoadjuvant chemotherapy group. There was a significant difference between patient groups in terms
of age, tumor markers and disease stage. The intraoperative complication rate was 12.5% in the
primary surgery group and 17.2% in the neoadjuvant chemotherapy group (p = 0.759). Postoperative
transfusion requirement was 28.9% in the first group and 21.8% in the second group (p = 0.641). The
mean hospital stay was 5.66 days in the first group and 5.92 days in the second group (p = 0.608).
Optimal surgical rates were 71.7% in the first group and 78.1% in the second group (p = 0.498). The
mean survival was 42.9 months in the first group and 39.1 months in the second group (p = 0.044).
Conclusion: Survival and progression free survival in patients with primary cytoreductive surgery were
significantly increased compared to neoadjuvant chemotherapy group. There was no significant
difference between the two groups in terms of optimal surgical rates and complication rates. However,
the lack of homogeneity of the groups makes it difficult to comment, and larger randomized controlled
trials are needed
