Sistemik kan
basıncı klinik pratikte en sık kullanılan doku perfüzyonu belirtecidir. Çok düşük
doğum ağırlıklı (ÇDDA) bebeklerde hipotansiyon yaşamın ilk günlerinde sık görülür.
Çalışmamızda ÇDDA bebeklerde yaşamın ilk haftasında görülen hipotansiyonla
ilişkili perinatal risk faktörlerini, klinik tanıları, hipotansiyonun morbidite ve mortalite
üzerine etkisini incelemeyi amaçladık. Bu kohort çalışmasında 01.01.2014-31.12.2018
tarihlerinde 500-1499 g ağırlığında doğan preterm bebeklerin verileri retrospektif
incelendi. Gebelik haftası (GH) 29±3,1 hafta ve doğum ağırlığı 1046±296 g olan 217
bebek çalışmaya alındı. Bebeklerin %47,5’inde yaşamın ilk haftası içinde
hipotansiyon saptanırken, GH<26 hafta olması, respiratuvar distress sendromu ve
doğum sırasında anneye genel anestezi verilmesi hipotansiyon gelişimi için bağımsız
risk faktörleri olarak bulundu. Hipotansiyonu olan bebeklerin %40’ında akut dönemde
hedef organ hasarı geliştiği ve %19,4’ünün öldüğü görüldü. En sık görülen
komplikasyon akut böbrek hasarıydı (ABH) (%25,2). Hipotansiyona eşlik eden
metabolik asidoz ve vazoaktif inotrop skorunun >15 olması ABH, oligüri ve pulmoner
kanama ise mortalite için bağımsız risk faktörleriydi. Hipotansiyona yönelik
farmakolojik tedavi alan bebeklerde mekanik ventilatörde kalma ve tam enteral
beslenmeye geçiş süreleri daha uzun, intrakraniyal kanama ve mortalite daha yüksekti
(p<0,001). Düşük GH, patent duktus arteriozus varlığı ve hipotansiyon yenidoğan
yoğun bakım mortalitesi için bağımsız risk faktörleri olarak tanımlandı. Sonuç olarak,
yaşamın ilk haftasında görülen hipotansiyon ÇDDA bebeklerde hedef organ hasarına,
morbidite ve mortalitede artışa neden olmaktadır. Hipotansiyona eşlik eden klinik
bulgular ve tedavi yükü morbidite ve mortalite riskiyle ilişkilidir
Systemic blood pressure is the most frequently used
marker of tissue perfusion in clinical practice. Hypotension in very low birth weight
(VLBW) babies is common in the first days of life. In our study, we aimed to analyse
perinatal risk factors and clinical diagnoses associated with hypotension seen in
VLBW infants in the first week of life, and to determine the effects of hypotension on
morbidity and mortality. In this cohort study we retrospectively analysed the data of
babies born between 01.01.2014-31.12.2018 with a birth weight (BW) of 500-1499 g.
A total of 217 babies with 29 ± 3.1 weeks gestational age (GA) and 1046 ± 296 g BW
were enrolled. Hypotension was present in 47.5% of the babies in the first week of life.
Gestational age <26 weeks, respiratory distress syndrome and general anaesthesia
during cesarean section were found to be independent risk factors for hypotension.
Acutely after hypotension 40% of the infants had target organ damage and 19.4% died.
The most common complication was acute kidney injury (AKI) which was present in
25.2% of the hypotensive infants. Metabolic acidosis accompanying hypotension and
a vasoactive inotropic score >15 were independent risk factors for AKI, and presence
of oliguria and pulmonary hemorrhage were independent risk factors for mortality.
Infants who received pharmacological therapy for hypotension had longer duration of
mechanical ventilation and time to reach full enteral feedings, and higher rates of
intracranial hemorrhage and mortality (p<0.001). Lower GA, presence of patent ductus
arteriosus, and hypotension were defined as independent risk factors for neonatal
intensive care mortality in VLBW infants. In conclusion, hypotension seen in the first
week of life causes target organ damage, and increased morbidity and mortality in
VLBW infants. Clinical findings related to severity of hypotension and treatment
burden were associated with higher morbidity and mortality
