Miyelodisplastik sendrom (MDS) kemik iliğindeki hematopoietik öncül hücrelerin
morfolojik ve fonksiyonel anormallikleri ile karakterize malign bir hematolojik
hastalıktır. AML ise hematopoietik sistemin proliferatif, klonal, anormal farklılaşmış
hücreleri tarafından infiltrasyonu ile karakterize hematolojik bir malignitedir.
Hastaların yaş, risk durumları, performans durumları, ek hastalıkları göz önünde
bulundurularak tedavi planlanmakta ve bazı hastalar yüksek doz tedaviyi tolere
edememektedir. Bu çalışmada yüksek doz tedavi almayan MDS ve AML tanılı
hastaların klinik ve laboratuar özelliklerinin yanı sıra aldıkları tedavilere olan
yanıtların değerlendirilmesi amaçlandı. Çalışmaya ESOGÜTF İç Hastalıkları
Anabilim Dalı, Hematoloji Bilim Dalı’nda Ağustos 2010-Şubat 2020 tarihleri arasında
WHO 2008 AML ve MDS tanı kriterlerine göre tanısı konmuş, 18 yaş üstü, yüksek
doz tedavi almayan 120 hasta alındı. Tüm olguların 74 (%61.7)’ü erkek, 46 (%38.3)’sı
kadındı. Hastaların 63 (%52.5)’ü MDS, 57 (%47.5)’si AML tanılı hastalardı.
Hastaların 64 (%53.3)’ü azasitidin, 8 (%6.7)’i desitabin ve 48 (%40)’i ardışık
azasitidin ve desitabin tedavisi almıştı. Hastaların tanı dönemi ve HMA sonrasındaki
laboratuvar bulguları karşılaştırıldığında HMA sonrasında laboratuvar bulgularında
anlamlı derecede iyileşme mevcuttu. Hastaların 63 (%52.5)’ünde tedaviye yanıt
saptandı. MDS hastalarında sağkalım, AML hastalarına göre daha uzundu (p=0.003).
Tedaviye yanıt oranı desitabin tedavisi alan grupta diğer gruplara oranla daha yüksekti
(p=0.03). Desitabin tedavisinde azasitidine göre daha sık FEN atağı gözlendi.
Azasitidin alan hastalarda diğer gruplara oranla sağkalım daha kısaydı. Azasitidin desitabin alan grupta diğer gruplara oranla daha sık relaps/progresyon gözlenirken, son
yanıt diğer gruplara oranla daha düşüktü. AML tanısı, FEN atağı varlığı, tedavi öncesi
trombosit replasman sayısının yüksek olması mortalite riskini arttırırken, azasitidin
kür sayısında artış riski azaltmaktaydı
Myelodysplastic syndrome (MDS) is a malignant hematological disease characterized
by morphological and functional abnormalities of hematopoietic precursor cells in the
bone marrow. AML is a hematological malignancy characterized by infiltration of the
hematopoietic system by proliferative, clonal, abnormally differentiated cells.
Treatment is planned considering the age, risk status, performance status, and
additional diseases of the patients, and some patients cannot tolerate high-dose
therapy. In this study, it was aimed to evaluate the clinical and laboratory
characteristics of patients with MDS and AML who did not receive high dose therapy,
as well as their responses to the treatments they received. 120 patients over the age of
18 who were diagnosed according to WHO 2008 AML and MDS diagnostic criteria
between August 2010-February 2020 in the ESOGUMF Department of Internal
Medicine, Department of Hematology, who did not receive high-dose therapy were
included in the study. 74 (61.7%) of all cases were male and 46 (38.3%) were female.
63 (52.5%) of the patients were MDS and 57 (47.5%) were AML patients. 64 (53.3%)
of the patients received azacitidine, 8 (6.7%) decitabine and 48 (40%) consecutive
azacitidine decitabine treatment. When the laboratory findings of the patients at
diagnosis and after HMA were compared, there was a significant improvement in the
laboratory findings after HMA. Response to treatment was detected in 63 (52.5%) of
the patients. Survival in MDS patients was longer than in AML patients. Response rate
to treatment was higher in the group receiving decitabine compared to the other groups.
More frequent FEN attacks were observed in decitabine treatment than azacitidine.
Survival was shorter in patients receiving azacitidine. While relapse/progression was
observed more frequently in the azacitidine-decitabine group compared to the other
groups and the final response was lower. While the diagnosis of AML, the presence of
a FEN attack, and a high pre-treatment platelet replacement count increased the risk
of mortality, an increase in azacitidine cycle count decreased the risk
