Bu randomize kontrollü, çift kör çalışmada, malignite nedenli tek
taraflı mastektomi planlanan hastalarda preoperatif uygulanan erektör spina plan
(ESP) blok ve torakal epidural anlajezinin (TEA), sempatik blok düzeyi ve
hemodinamik etkileri, duysal blok ve postoperatif analjezik etkilerini karşılaştırmayı
amaçladık. Çalışmaya, ASA fiziksel sınıflaması I-III olan, 18-70 yaş arası, 38 kadın
hasta çalışmaya dahil edildi. Hastalar randomize olarak Grup E (Erektör spina plan
bloğu sonrası genel anestezi uygulananlar) ve Grup T (Torakal epidural blok sonrası
genel anestezi uygulananlar) olmak üzere iki gruba ayrıldı. Blok sonrası hastaların
deri iletkenliği (galvanik deri cevabı- GSR), vücut ısısı değişikliği, sempatik ve
duysal blok seviyeleri, peroperatif hemodinami takibi, postoperatif ağrı skorları ve
analjezik tüketimi kaydedildi. Blok sonrası değerlendirmede, gruplar arasında GSR
ölçümleri, vücut ısısı artışları ve sempatik blok açısından istatistiksel anlamlı fark
görülmese de, ESP blok daha geniş duysal bloğa neden oldu. Her iki bloğun da
peroperatif hemodinami üzerine etkileri benzer şekilde kalp hızında (KH) önemli bir
düşüşle beraber ortalama arteriyel basınçta (OAB) minimal bir azalmaydı.
Postoperatif ağrı skorları, hasta kontrollü analjezi (HKA) ve kurtarıcı analjezik
tüketimlerinde iki grup arasında istatistiksel anlamlı fark bulunmadı. Bu çalışmanın
sonuçları, ESP bloğun etki mekanizmasında paravertebral alana yayılımın rolü
olduğu fikrini desteklemektedir ve ayrıca torakal ESP blok TEA ile benzer etkinlik
ve hemodinamik cevaplar oluşturabilir
In this randomized controlled, double-blind study,
we aimed to compare the sympathetic block level and effects on
hemodynamics, sensory block and postoperative analgesic effects of erector
spinae plane (ESP) block and thoracic epidural anlagesia (TEA) performed
in unilateral mastectomies due to malignancy. Thirty-eight female patients
aged 18-70, ASA physical classification I-III were included in the study.
The patients were randomised into two groups as Group E (those who
received general anesthesia after the erector spina plan block) and Group T
(those who received general anesthesia after thoracic epidural block). After
the block, skin conductivity (galvanic skin response - GSR), body
temperature changes, sympathetic and sensory block levels, peroperative
hemodynamic datas, postoperative pain scores and analgesic consumption
were recorded. In the preoperative evaluation, there was no statistically
significant difference between the groups in terms of GSR measurements,
body temperature increases and sympathetic block. ESP block caused a
wider sensory block. The effects of both blocks on peroperative
hemodynamics were a minimal decrease in mean arterial pressure (MAP)
with a significant decrease in heart rate (HR). There was no statistically
significant difference in postoperative pain scores, patient controlled
analgesia (PCA) and rescue analgesic consumption between the two groups.
The results of this study support the idea of spreading to the paravertebral
area plays a role in the mechanism of action of the ESP block, and the
thoracic ESP block may generate similar efficacy and hemodynamic
response to TEA
