Diffüz büyük B hücreli lenfoma yetişkinlerde görülen Non-Hodgkin lenfomaların
yaklaşık %35’ini oluşturmaktadır. Diffüz büyük B hücreli lenfoma kategorisi
heterojen olup, Dünya Sağlık Örgütü (DSÖ) 2016 lenfoma sınıflamasında, farklı
klinikopatolojik ve moleküler özellikler temelinde yeni antiteler oluşturulmuş,
yüksek dereceli ve özel tip lenfoma olarak sınıflandırılamayan büyük B hücreli
lenfomalar (DLBCL), daha ileriye spesifiye edilemeyen (NOS) DLBCL olarak
tanımlanmıştır. Bu kategorideki olgular moleküler özelliklerine göre GEP analizi ve
immünohistokimyasal (Hans) algoritmalarla GCB ve ABC alt tiplere
gruplandırılmıştır. Çalışmamızda BCL2, BCL6, MYC gen anormalliklerinin FISH ile
değerlendirilmesi sonucu ‘’DLBCL, NOS’’ kategorisine alınan 45 olgu Hans
algoritmasına göre GCB (15 olgu) ve Non-GCB (45 olgu) olmak üzere moleküler alt
tiplere ayrılmıştır. Bu olgularda CD30, BCL2, BCL6, MYC protein ekspresyonları,
double ekspresyon durumu değerlendirilmiş ve FISH ile saptanan gen anormallikleri
ile aralarındaki ilişki araştırılmıştır. FISH ile saptanan BCL2, BCL6, MYC gen
anormallikleri ile protein ekspresyonları arasında korelasyon saptanmamıştır
(p>0,05). Nodal orjinli lenfomalarda BCL2 ekspresyon sıklığı ve Non-GCB tipte
BCL2 ve double ekspresyon sıklığı yüksek bulunmuştur (p<0,05). CD30
ekspresyonu olan olgularda Ki-67 proliferasyon indeksi yüksektir (p<0,05). CD30
ekspresyonu olan olgularda MYC gen anormallikleri saptanmamıştır (p>0,05). MYC
translokasyon ve kopya sayısı artışı olan olgularda Ann-Arbor evre ve LDH düzeyi
yüksek saptanmıştır (p<0,05). BCL2, BCL6 ve MYC kopya sayısı artışlarında
sıklıkla birliktelik görülmüştür (p<0,05). Günlük pratikte olgularda bu faktörlerin
bilinmesinin prognozun öngörülebilirliği ve tedavi planlaması açısından faydalı
olacağı düşünülmektedir
Diffuse large B cell lymphoma accounts for approximately 35% of Non Hodgkin lymphomas seen in adults. Diffuse large B-cell lymphoma category is
heterogeneous and in the World Health Organization (WHO) 2016 lymphoma
classification, new entities have been created on the basis of different
clinicopathological and molecular features. Large B-cell lymphomas, which cannot
be classified as high-grade and special-type lymphoma is defined as DLBCL, NOS.
The cases in this category were grouped into GCB and ABC subtypes with GEP
analysis and immunohistochemical (Hans) algorithms according to their molecular
features. In our study, 45 cases that were included in the "DLBCL, NOS" category as
a result of the evaluation of BCL2, BCL6, MYC gene abnormalities with FISH were
divided into molecular subtypes as GCB (15 cases) and Non-GCB (45 cases)
according to Hans algorithm. In these cases, CD30, BCL2, BCL6, MYC protein
expressions, double expression status were evaluated and their relationship with gene
abnormalities detected by FISH was investigated. There was no correlation between
BCL2, BCL6, MYC gene abnormalities detected by FISH and protein expressions
(p> 0.05). BCL2 expression frequency in lymphomas of nodal origin and BCL2 and
double expression frequency in Non-GCB type were found to be high (p <0.05). In
cases with CD30 expression, the proliferation index of KI-67 is high (p <0.05). There
were no MYC gene abnormalities in cases with CD30 expression (p> 0.05). Ann Arbor stage and LDH level were found to be high in cases with MYC translocation
and copy number increase (p <0.05). There was frequent association in BCL2, BCL6
and MYC copy number increases (p <0.05). It is thought that knowing these factors
in cases in daily practice will be useful in terms of predictability of prognosis and
treatment planning
