Amaç: Tansiyon ölçme işleminin sık takibi gerektiğinden ve günde
ortalama en az (on kez*on hasta) 100 kez tekrarlanmasının, kas iskelet sistemi
rahatsızlıklarına ve tükenmişliğe etki edebileceği düşünülmektedir. Bu tezin
amacı, cerrahi ve dahili servislerde çalışan hemşirelerde ergonomik risk
analizine göre tekrarlı hareket olan tansiyon ölçme işleminin, kas iskelet
sistemi rahatsızlıklarına ve hemşirelerin tükenmişlik düzeyine etkisini
belirlemektir.
Yöntem: Çalışma Ağustos-Ekim 2017 tarihleri arasında, Eskişehir Devlet
Hastanesi Cerrahi ve Dahili Servislerde çalışan (n=91) hemşire üzerinde
yürütülen kesitsel tipte bir araştırmadır. Çalışmanın yapılabilmesi için etik
kurul, kurum izni ve çalışmaya katılan hemşirelerin yazılı onamları alınmıştır.
Çalışmada “Hemşire veri toplama formu”, “REBA (Rapid Entire Body
Assessment-Hızlı Tüm Vücut Değerlendirmesi) Çalışan Değerlendirme Formu”,
“Cornell Kas-İskelet Rahatsızlık Ölçeği” ve “Maslach Tükenmişlik Ölçeği”
kullanılmıştır. Çalışmanın amacına yönelik olarak hazırlanan anket formları
servislerdeki hemşireler tarafından doldurulurken, REBA ise araştırmacı
tarafından gözlem yoluyla doldurulmuştur. Verilerin istatistiksel analizleri için
bağımsız örneklem t-testi, Kruskall Wallis testi, Varyans ve korelesyon analizi
kullanılmıştır. İstatistiksel anlamlılık değeri p<.05 olarak alınmıştır.
Bulgular: Çalışmada n=91 hemşirenin ergonomik risk analizine göre
tekrarlı hareket olan tansiyon ölçme işlemi sırasındaki duruşlarının analizi
sonucunda REBA skoru ortalama 5.10±0.99 olup, hemşirelerin %95,6’sı orta
derecede, %2,2’si yüksek derecede risk altında saptanmıştır. Hemşirelerin
hekim tarafından tanısı konulmuş en az bir kas iskelet sistem hastalığı
prevalansı %25,3’tür. Fiziksel egzersiz yapmayan, belinde her gün en az bir
kez ağrı yaşayan, kalça bölgesinde ağrılarından orta derecede rahatsızlık
duyan, kol, sağ ve sol ayak ağrısının iş performansını çok fazla engelleyen
hemşirelerin REBA risk puanları diğerlerine göre istatistiksel olarak daha
yüksektir.
Hemşirelerin tükenmişlik ölçeğinin duygusal tükenme alt boyutundan
aldıkları puan ortalaması 22.38±6.80, duyarsızlaşma alt boyutundan aldıkları
puan ortalaması 6.59±4.14, kişisel başarı alt boyutundan aldıkları puan
ortalaması 19.47±4.85’dir. Gündüz vardiyasında ve gece vardiyasında 10 ve
altında hastanın bakımından sorumlu olan hemşirelerin duygusal tükenme
puanları daha düşüktür. Çalışmada hemşirelerin duygusal tükenme puanları
arttıkça, duyarsızlaşma ve kişisel başarı eksikliği puanları da artmaktadır. Aynı
zamanda duyarsızlaşma puanları arttıkça kişisel başarı eksikliği puanları da
artmaktadır.
Sonuç: Tansiyon ölçme işleminin REBA risk puanı orta derecededir. Dört
hemşireden birinde hekim tarafından tanısı konulmuş en az bir kas iskelet

sistemi hastalığı vardır. Bel, kalça, kol ve ayakta REBA puanı yanı sıra ağrı da
yüksektir. Hemşirelerin 10 ve altında hastaya bakması duygusal tükenmelerini
azalttığından, yönetici ve politikacıların uygun çalışma ortamı sağlamak, uygun
ekipman temin etmek, hemşirelerin var olan kas iskelet sistemi hastalıklarının
ilerlemesini önlemek amacıyla doğru duruş, vücut mekaniği, çalışma
pozisyonu, uygun kaldırma teknikleri vb. konuları içeren periyodik eğitim
programlarını düzenlemesi ve egzersiz programlarına yönlendirmeleri
önerilmektedir
Aim: It is thought that frequent follow-up of the blood pressure
measurement process and the recurrence of at least (ten times * ten patients)
at least 100 times per day may have an effect on musculoskeletal disorders
and burnout. The aim of this thesis is to determine the effect of blood pressure
measurement process on the musculoskeletal disorders and burnout level of
nurses working in surgical and internal services according to ergonomic risk
analysis.
Method: The study was a cross-sectional study conducted between
August-October 2017 in Eskişehir State Hospital (n = 91). In order to carry out
the study, the ethics committee, the permission of the institution and the
written consent of the nurses who participated in the study were taken. ”Nurse
data collection form Öl, Öl REBA (Rapid Entire Body Assessment) Employee
Evaluation Form u, Vücut Cornell Musculoskeletal Disorder Scale“ and ”Maslach
Burnout Inventory Çalış were used in the study. The questionnaires prepared
for the purpose of the study were filled in by the nurses in the services, while
REBA was filled by observation by the researcher. Independent samples t-test,
Kruskall Wallis test, variance and correlation analysis were used for statistical
analysis of the data. Statistical significance value was taken as p <.05.
Results: In the study, the REBA score was 5.10 ± 0.99 after the analysis
of the position of the nurse n = 91 according to the ergonomic risk analysis.
The prevalence of at least one musculoskeletal disorder diagnosed by the
physician is 25.3%. Nurses who do not exercise physical exercise, have at
least one pain in the waist at least once a day and have moderate discomfort
in the hip area, and who have a great deal of pain in the arm, right and left
foot pain, have a significantly higher REBA risk score than the others.
The mean score of the burnout scale of emotional exhaustion sub dimension of the nurses was 22.38 ± 6.80, the mean score from the
desensitization sub-dimension was 6.59 ± 4.14, and the mean score of
personal accomplishment sub-dimension was 19.47 ± 4.85. Emotional
exhaustion scores of nurses who are responsible for patient care in day shifts
and night shifts are lower than 10. As the emotional exhaustion scores of
nurses increased, desensitization and lack of personal success scores
increased. At the same time, as the desensitization scores increase, the scores
of personal accomplishment scores also increase.

Conclusion: The REBA risk score of the blood pressure measurement process
is moderate. One in four nurses has at least one musculoskeletal disorder
diagnosed by a physician. The waist, hip, arm and standing REBA score as well
as pain are high. As the nurses look at the patient at 10 and below, it
decreases their emotional exhaustion, to ensure proper working environment
of managers and politicians, to provide appropriate equipment, correct
posture, body mechanics, working position, proper lifting techniques etc. It is
recommended that they organize periodic training programs with topics and
direct them to exercise programs
