Bu çalışmada Ocak 2012 ve
Aralık 2016 yılları arasında Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Çocuk
Acil servisine nöbet şikayeti ile başvuran 1 ay - 18 yaş arası çocuklar geriye dönük
olarak değerlendirildi. Çalışmaya dahil edilen hastaların 69 (% 43,4)’u kız, 90 (%
56,6)’ı erkekti. Hastaların yaş ortalaması 69,8±62,8 ay olarak saptandı. Olguların
nöbet tipi değerlendirildiğinde 73 (% 45,9) hastada febril nöbet, 75 (% 47,2) hastada
afebril nöbet, 11 (% 6,9) hastada status epileptikus saptandı. Nöbet tipi ile başvuru anı
bakılan serum glikoz arasında istatistiksel olarak anlamlı fark bulunmuştur(p=0,015).
Nöbet tipi ile kan gazı laktat, nöbet öyküsü, ailede nöbet öyküsü, acil serviste kalış
süresi, hastaneye yatış ihtiyacı ve nöbet tekrarı arasında anlamlı fark bulunmamıştır.
Hastaların ortalama nöbet süresi 3,8±3,1 dakika olarak saptandı. Hastaların nöbet
süresi ile kan gazı pH arasında negatif korelasyon, serum glikozu ile pozitif korelasyon
saptandı. 84 (% 52,8) hastanın başvuru anındaki kan gazı laktat değeri ≥2 mmol/L
saptandı. 41 (% 25,7) hastada başvuru anında hiperglisemi saptanmıştır. Hipoglisemi
saptanan hasta yoktu. Hastaların acil serviste gözlem süreleri ortalama 8±2,9 saat
olarak bulundu. 12 (% 7,5) hastanın hastaneye yatırılarak takip ve tedavisinin devam
ettiği gözlendi. Hastanede yatış süreleri ortalama 3±1,8 gündü. Ölen hasta yoktu. 27
(% 17) hastada nöbet tekrarı görüldü. Nöbet tekrarı ile kan gazındaki CO2 arasında
istatistiksel olarak anlamlı fark bulundu(p=0,04). Nöbet tekrarı açısından yapılan ROC
analizinde laktat cut off değeri 1,9 mmol/L, kan şekeri cut off değeri 110 mg/dL olarak
bulundu. Bu çalışma çocuklarda nöbetlerin klinik seyir üzerine etkili faktörlerin
aydınlatılması açısından literatüre katkıda bulunmaktadır
In this study, children
aged between 1 month and 18 years who were admitted to Eskişehir Osmangazi
University Medical Faculty Pediatric Emergency Service between January 2012 and
December 2016 were evaluated retrospectively. In this study 69 (43,4 %) of the
patients were female and 90 (56,7.6 %) were male. The mean age of the patients was
69,8±62,8 months. When the type of seizure was assessed, 73 (45,9 %) patients had
febrile seizures, 75 (47,2 %) had afebrile seizures and 11 (6,9 %) patients had status
epilepticus. There was a significant statistically relationship between the type of
seizure and the serum glucose which was examined at the time of admission (p=0,015).
There was no significant difference between seizure type, blood gas lactate, seizure
history, family history of seizures, duration of stay in emergency department, need for
hospitalization and seizure recurrence. The average seizure time of the patients was
3,8 ± 3,1 minutes. There was a negative correlation between seizure duration and blood
gas pH and positive correlation with serum glucose. 84 (52,8 %) patients had a blood
gas lactate value ≥2 mmol / L at the time of admission. 41 patients (25,7 %) had
hyperglycemia at admission. There were no patients with hypoglycemia. The patients
in the emergency department observation period was found to be an average of 8 ± 2,9
hours. It was observed that 12 (7,5 %) patients were hospitalized and followed-up. The
average length of stay in hospitalwas 3 ± 1,8 days. There was no patient with mortality.
27 (17 %) patients had seizure recurrence. There was a significant statistically
relationship between the seizure recurrence and the CO2 in the blood gas(p=0,04).
ROC analysis of seizure recurrens revealed lactate cut off 1,9 mmol / L and blood
glucose cut off 110 mg / dL. This study contributes to the literature in terms of
elucidating the factor affecting the clinical course of seizures in children
