Çölyak hastalarının tanı anındaki
demografik, klinik, endoskopik, laboratuvar, patolojik ve genetik bulgularının
remisyon süresine etkisini değerlendirmek amacıyla bu çalışma yapılmıştır. Anabilim
dalımızda tanı alan çölyak hastalarının dosyaları retrospektif olarak incelendi. Hastalar
DTG Ig A’nın laboratuvar üst sınırından fazlalığına ve EMA pozitifliğine göre A (
DTG IgA ≥5 kat ve/veya EMA (++)), B (DTG Ig A <5 kat ve/veya EMA(+)) ve C
(DTG Ig A<5 kat ve EMA (-)) olarak gruplandırıldı. Tüm belirteçlerin negatifleşmesi
remisyon olarak kabul edildi. Çalışmaya 59’u kız, 49’u erkek, ortalama yaşları
7,94±4,06 yıl olan 108 hasta dahil edildi. Kızların %89.8’i, erkeklerin %91.8’i
remisyona girmişti (p=0.72). Ortalama remisyon süresi 11.04±8.95 ay idi. İntestinal
bulgusu olanların %93.2’si, ekstraintestinal bulgusu olanların %90.6’sı remisyona
girmişti. Ekstraintestinal bulgu varlığı (11.98±9.85’e karşın 9.68 ±7.36, p=0.21); HLA
DQ2/DQ8 pozitifliği (12.12±10.20’e karşın 8.40±6.90, p=0.27) ve eşlik eden
hastalıklar (11.23 ±10.68’e karşın 10.98±8.12 ay, p=0.89) remisyon süresini
etkilememişti. A,B,C gruplarında sırasıyla 68, 24 ve 15 hasta vardı. Grupların
remisyon sürelerinde fark yoktu (sırasıyla: 10.59±7.57, 9.42±7.41, 15.13±13.91 ay,
p=0.118). Endoskopik bulgusu olmayanlar 9.41 ±5.95 ayda, 1 bölgede ve 2 bölgede
endoskopik bulgusu olanlar sırasıyla, , 12.31±10.19 ve 7.63 ±3.81 ayda remisyona
girmişti (p=0.12). MARSH Tip1 olanlarda tip 2 ve 3’e göre daha geç remisyona
sağlanmıştı (sırasıyla, 23.00±25.23, 7.17±2.99, 10.9±8.21 ay, p=0.037). Klinik
remisyon serolojik remisyon ile korele idi (r=0.29, p=0.018). Sonuç olarak çölyak
hastalarında yaş ve cinsiyetin, tanı anındaki semptomların, serolojik belirteç
düzeylerinin ve endoskopik bulguların remisyon süresini etkilemediği, ancak
MARSH’ı düşük olan hastaların daha geç remisyona girdiği, klinik ve serolojik
remisyonun korele olduğu görüldü
Aim of
this study was to evaluate the effects of demographic, clinical, laboratory, endoscopic,
pathological and genetic findings on remission of celiac patients. Data of the celiac
patients diagnosed in our department have been revised retrospectively. Depending on
their TTG antibody level and EMA status patients were grouped as A (TTG IgA≥5
times of UNL and/or EMA (++)), B (TTG IgA<5 times of UNL and/or EMA(+)) and
C (TTG IgA<5 times of UNL and EMA (-)). Remission was defined as achievement
of negative serology of all markers. The study involved 108 patients composed of 59
female, 49 male with a mean age of 7.94±4.06 years. 89.8% of the girls and 91.8% of
the boys achieved remission (p=0.72). The mean remission time was 11.04±8.95
months. 93.2% of the patients with intestinal and 90.9% of the patient with
extraintestinal symptoms achieved remission. Presence of extraintestinal symptoms
(11.98±9.85 vs 9.68±7.36 months, p=0.21). HLA DQ2/DQ8 positivity (12.12±10.20
vs 8.40±6.90 months, p=0.27) and concomitant diseases (11.23 ±10.68 vs 10.98±8.12
months, p=0.89) did not have any effect on remission time. There was 68, 24 and 15
patient in group A, B and C respectively. There wasn’t any difference in terms of
remission time between groups (respectively:0.59±7.57, 9.42±7.41, 15.13±13.91
months, p=0.118). Patients without any endoscopic lesion achieve remission at
9.41±5.95 months, whereas those with one and two anatomical involvement achieved
remission at 12.31±10.19 and 7.63±3.81 months respectively (p=0.12). Remission
time was shorter in MARSH Type 1 lesion compared to type 2 and 3 (respectively,
23.00±25.23, 7.17±2.99, 10.9±8.21 months, p=0.037). Clinical and serological
remission was positively correlated (r=0.29, p=0.018). In conclusion, it was observed
that clinical and serological remissions were positively correlated and except
pathological grade none of the other parameters had any effect on remission time
