İki aşamada yürütülen çalışmanın
birinci aşamasının amacı gebelerin doğum öncesi bakım (DÖB) ile ilgili sağlık
okuryazarlık (SOY) düzeylerini değerlendirme aracının geliştirilmesi olup ikinci
aşamasının amacı ise gebelerin DÖB ile ilgili SOY düzeylerinin saptanmasıdır. Bu
çalışma, 15.02.2016-01.07.2018 tarihleri arasında gerçekleştirilen metodolojik ve
kesitsel tipte bir araştırmadır. Çalışmamızın ilk aşamasında “Doğum Öncesi Bakım
İle İlgili Sağlık Okuryazarlık Ölçeği (DÖBSOY-Ö)” geliştirilmiş olup, madde
analizi, açımlayıcı faktör analizi, eş zamanlı ölçüt geçerliliği, gruplar arası ayrışma
ve doğrulayıcı faktör analizi ve Kuder-Richardson-20 (KR-20) katsayısının
belirlenmesi yöntemleri ile geçerlilik-güvernirliği test edilmiştir. Çalışmamızın ikinci
aşaması 847 gebe üzerinde gerçekleştirilmiştir. Bu çalışmanın verileri amacımıza
uygun olarak hazırlanan anket form ile toplandı. Çalışmanın ikinci aşamasında Mann
Whitney U testi, Kruskal Wallis testi, Spearman korelasyon analizi ve çoklu doğrusal
regresyon analizi kullanıldı. Bu çalışmada, DÖBSOY-Ö’de yer alan maddelerin ayırt
edicilik katsayıları 0.20-0.57 ve faktör yükleri 0.34-0.85 arasında değişmekteydi.
DÖBSOY-Ö’nün her bir alt alanında KR-20≥0.85 olduğu saptandı. Doğrulayıcı
faktör analizinde elde edilen uyum ölçütlerine ait skorlar kabul edilebilir sınırlarda
idi. Çalışmanın ikinci aşamasını oluşturanların yaşları 18 ile 44 arasında değişmekte
olup ortalama 28.5±5.2 yıl idi. Çalışmamızda, 25 yaş ve üstünde olanların, gelir
getirici işte çalışanların ve çekirdek aile yapısına sahip olanların DÖBSOY-Ö’nün
tüm alt alanları ile ilgili SOY düzeylerinin yüksek olduğu saptandı. Ayrıca gebenin
kendisinin, eşinin ve ebeveynlerinin öğrenim düzeyi arttıkça DÖBSOY-Ö’nün tüm
alt alanları ile ilgili SOY düzeyleri de artmaktaydı. Sonuç olarak, DÖBSOY-Ö
geçerli ve güvenilir bir ölçek olup, çalışma grubundakilerin doğum öncesi dönemle
ilgili SOY düzeyleri yüksekti. Gebelerin SOY düzeyleri belirlenerek hazırlanacak
eğitim materyalleri ile yapılacak sağlık eğitimiyle SOY düzeylerinin geliştirileceği
kanısına varıldı
The aim of the first
stage of the study carried out in two phases is to develop an assessment tool for
assessing health literacy (HL) levels associated with antenatal care (AC) of the
pregnants and the aim of the second stage is to determine the HL levels of the
pregnancies related to AC. The study is a methodological and cross-sectional study
conducted between 15.02.2016 and 01.07.2018. In the first stage of our study
"Health Literacy Scale Related to Antenatal Care (HLSRAC)" has been developed,
the validity and reliability were tested by means of item analysis, explanatory factor
analysis, criterion validity, inter-group decomposition and confirmatory factor
analysis and Kuder-Richardson-20 (KR-20) coefficient determination methods. The
second stage of our study was performed on 847 pregnant women. The data of this
study were collected with a questionnaire form prepared in accordance with our
purpose. Mann Whitney U test, Kruskal Wallis test, Spearman correlation analysis
and multiple linear regression analysis were used in the second stage of the study. In
this study, the discriminative coefficients of the items in the HLSRAC ranged from
0.20 to 0.57 and the factor loadings ranged from 0.34 to 0.85. KR-20 ≥ 0.85 was
detected in each sub-area of HLSRAC. The scores of the compliance measures
obtained in the confirmatory factor analysis was at acceptable limits. The age of the
second stage of the study ranged from 18 to 44 years with an average of 28.5 ± 5.2
years. In our study, it was determined that the HL levels for all sub-domains of
HLSRAC were higher in those aged 25 years and over, those working at income generating jobs, and those with core family structure. In addition, as the level of
education of her partner and her parents increased, the HL levels of all subfields of
HLSRAC also increased. In conclusion, HLSRAC is a valid-reliable scale, and the
HL levels of the antenatal period in the study group were high. It was concluded that
the HL levels would be improved by determining the HL levels of the pregnants and
with the health education to be made with the educational materials to be prepared
