Sitotoksik bir alkilleme maddesi olan siklofosfamid (CP), yaygın olarak kullanılan bir
antimitotik ajandır. Ancak CP kulanımı, hemotoksisite ve ürotoksisitenin de dahil olduğu
çoklu organ toksisitelerine neden olmaktadır. Hemorajik sistit, CP nedenli oksidatif strese
atfedilen doz sınırlandırıcı yan etkilerden biridir. Borik asit (BA) serbest radikallere ve
oksidatif ataklara karşı olduğu bilinen güçlü antioksidanlardan biridir. Bu çalışmanın amacı,
sıçanlarda CP'nin neden olduğu mesane hasarı üzerine BA'nın olası koruyucu etkisini
belirlemekti. Toplam 24 adet Sprague Dawley cinsi erkek sıçana standart kemirgen diyeti
uygulandı. Sıçanlar rastgele 4 eşit gruba ayrıldı: Kontrol grubundaki sıçanlara 6 gün boyunca
her gün 0.5 mL serum fizyolojik (SF) intraperitonel (i.p.) olarak verildi. 2. gruptaki sıçanlara
6 gün boyunca her gün 0.5 mL SF, 4. gün tek doz 200 mg/kg CP verildi. 3. gruptaki sıçanlara
6 gün boyunca her gün 200 mg/kg BA verildi. 4. grupta 6 gün boyunca her gün 200 mg/kg
BA alan sıçanlara 4. günde tek doz 200 mg/kg CP verildi. Son BA veya SF uygulamasından
24 saat sonra, sıçanlar anestezi altında sakrifiye edildi. Deneyin sonunda etik kurallara uygun
olarak, sıçanların mesaneleri ışık mikroskobik (histolojik) ve immünohistokimyasal (bax, bcl 2 ve kaspaz-3 gibi apoptotik belirteçler) analizler için alındı. CP uygulanan sıçanların
mesanelerinde, geçici epitelyal incelme, belirgin bir inflamatuar reaksiyon ve kanama
gözlendi. CP uygulanan grupta ayrıca, bcl-2'nin aktivitesi azalırken, bax ve kaspaz-3 pozitif
hücrelerin sayısında artış görüldü. Buna karşılık, tedavi edilen CP+BA grubunda, mesane
dokusunda hasarın oldukça azaldığı, bax ve kaspaz-3 pozitif hücre sayısında bir azalma, bcl-2
pozitif hücre sayısında bir artış saptandı. Ayrıca BA'nın kendi başına verildiğinde mesanede
herhangi bir dejenerasyon ve apoptoza neden olmadığı tespit edildi. Sonuçlarımız, BA ön
tedavisinin, anti-apoptotik ve antioksidan özellikleri ile CP nedenli mesane hasarına karşı
sıçan mesanesini önemli derecede koruduğunu göstermektedir
Cyclophosphamide (CP), a cytotoxic alkylating agent, is an prevalently used
antimitotic agent. However, the use of CP leads to multiple organ toxicities including
hemotoxicity and urotoxicity. Hemorrhagic cystitisis noted as one of the dose limiting side
effects of its use, which is attributed to oxidative stress. Boron (B) is strong antioxidants
against free radicals and oxidative attacks. The aim of the current study was to assess the
probable caring effect of B on CP induced bladder damage in rats. A total of 24 Sprague
Dawley male rats received a standard rodent diet. Rats were randomly divided into 4 equal
groups: Rats in the control group were given 0.5 mL saline (SF) intraperitoneally (i.p.) every
day for 6 days. Rats in group 2 were given 0.5 mL of SF daily for 6 days and 200 mg/kg of
CP on day 4. The rats in group 3 were given 200 mg/kg BA every day for 6 days. The rats in
group 4 receiving 200 mg / kg BA every day for 6 days were given 200 mg/kg CP on the 4th
day. Twenty-four hours after the last BA or SF administration, the rats were sacrificed under
anesthesia. In accordance with the ethical rules, at the end of the experiment, bladders of rats
were taken for light microscopic (histological) and immunohistochemistry (apoptotic markers
such as bax, bcl-2 and caspase-3). In the bladders of rats treated with CP, transient epithelial
thinning, marked inflammatory reaction and bleeding were observed. In the CP-treated group,
the activity of bcl-2 decreased while the number of bax and caspase-3 positive cells increased.
On the other hand, in the treated CP+BA group, there was a decrease in bax and caspase-3
positive cell number, an increase in the number of bcl-2 positive cells, and a decrease in bax
and caspase-3 positive cells. As well it was determined that boric acid did not cause any
degeneration and apoptosis in the bladder when given on its own. Our results indicate that BA
pretreatment significantly protects rat bladder against CP-induced bladder damage with its
anti-apoptotic and antioxidant properties
