Kliniğimizde Renal Hücreli Karsinom (RHK) nedeniyle tedavi ve
takip ettiğimiz hastaların retrospektif olarak yaş, cins dağılımı, epidemiyolojik
özelliklerini ortaya koymak ve hastaların klinik ve patolojik parametrelerinin
hastalığın prognozuna etkisinin değerlendirilmesi amaçlandı. 2010 ile 2017 yılları
arasındaki patoloji sonucu RHK olarak rapor edilen 69 hasta çalışmaya dahil edildi.
Olguların 52’i erkek (% 75,4), 17’i kadındı (% 24,6), yaş ortalaması 58,6±9,7 yıl idi.
Ortalama takip süresi 39 ay olup, izlem sonunda 48 (%69) hastanın tümör nedeniyle
kaybedildiği görüldü. Evre (p<0,001), lenf nodu tutulumu (p=0,04), tanı anında
metastaz bulunması (p=0,006), laboratuar parametrelerinden ise tanı anındaki
trombosit sayısı (p=0,001), kandaki nötrofil-lenfosit oranı (p=0,015), nüks-metastaz
sırasındaki lökosit sayısı (p=0,004), kalsiyum (p<0,001) ve LDH (p=0,02) değeri
prognoza etki eden faktörler olarak belirlendi. Hedefe yönelik tedavi (HYT) almış
hastaların ortanca genel sağkalımı ve 1 yıllık genel sağkalımı HYT almamış hastalara
göre daha uzun saptanmıştır (sırasıyla 22 aya karşın 6 ay, %52,9 karşın %11)
(p=0,011). Metastazektomi yapılmış hastaların metastazektomi yapılmayanlara göre
prognozu daha iyi saptanmıştır (sırasıyla ortanca genel sağkalım 71 aya karşın 28 ay)
(p=0,013). Sonuç olarak, klinik ve patolojik evrelemenin yapılması, prognostik
parametrelerinin belirlenmesi ve uygun tedavi seçimi hastalığın yönetimi açısından
kritik öneme sahiptir
It was aimed to reveal the age and gender distribution
and epidemiological features as well as to evaluate the effects of the clinical and
pathological features on the prognosis of the disease retrospectively in patients with
renal cell carcinoma (RCC), who were treated and followed in our department. A
total of 69 patients in whom the pathological diagnosis was made as RCC between
the years 2010 and 2017 were included to the study. Of them, 52 (75,4%) were male
and 17 (24,6%) were female, with a mean age of 58,6±9,7. The mean follow-up
duration was 39 months, and 48 patients (69%) died due to tumor. The factors that
affect the prognosis of RCC were stage (p<0,001), lymph node involvement
(p=0,04), presence of metastasis at the time of diagnosis (p=0,006), and several
laboratory parameters including platelet count (p=0,001), the ratio of neutrophile to
lymphocyte (p=0,015) at the time of diagnosis, leukocyte count (p=0,004), the levels
of calcium (p<0,001) and LDH (p=0,02) at the time of recurrence or metastasis.
Median overall survival and 1-year overall survival of patients who received targeted
therapy was significantly better compared to who not received (22 months versus 6
months and 52,9% versus 11%, respectively) (p=0,011). The prognosis of patients
with metastasectomy was better compared to patients without metastasectomy
(median overall survival was 71 months versus 28 months, respectively) (p=0,013).
In conclusion, it is critically important to perform clinical and pathological staging,
identify the prognostic parameters and choose appropriate treatment for management
of disease
