Çalışmamızın amacı, ambulans ve acil bakım teknikeri (AABT) ve acil tıp teknisyeni
(ATT) ile, ambulans içinde sürüş esnasında, havayolu maketleri üzerinde direk
laringoskopi (DL) ve video laringoskopi (VL) yöntemleriyle entübasyon (ETE)
girişimlerinin özelliklerinin karşılaştırılmasıdır. Metodolojik çalışmamızda, her iki
yöntemin karşılaştırılması öncesinde, standart havayolu maketi üzerinde DL ve VL
ile 1 saat teorik, 3 saat pratik ETE eğitimi yapıldı. Her iki girişimde başarılı
entübasyonun kaçıncı denemede olduğu, ETE girişim süresi, vokal kord görülme
süresi, her iki ETE yönteminin kolaylık derecesi (1-çok zor; 10-çok kolay) gibi
bilgileri içeren veriler anket formu ile toplandı. Elde edilen veriler SPSS (versiyon
20.0) paket programında değerlendirildi. Tanımlayıcı bilgiler sayı, yüzde, standart
sapma, ortalama ile sunuldu. Verilerin normal dağılıma uygunluğu Shapiro-Wilk ile
değerlendirildi ve karşılaştırmalar için Wilcoxon, Mc Nemar ve Ki kare testleri
kullanıldı. Bir bağımlı değişken üzerinde birden fazla bağımsız faktörlerin ortak
etkisinin araştırılması için SAS University Edition programında iki yönlü varyans
analizi yapıldı. Çalışmaya, yaş ortalaması 30.3±4.5 yıl (min: 22, maks: 42) olan 22’si
(%27.8) erkek katılımcı dahil edildi. İlk denemede, başarılı olan girişim sayısı, DL
yöntemi ile 58 (%73.4), VL yöntemi ile 61 (%77.2) idi ve her iki yöntem arasında ilk
denemede başarılı olma açısından fark bulunamadı (p=0.708). VL yöntemi ile vokal
kordları görme süresi daha kısa idi (p=0.001) ve başarılı ETE yapma süresi, VL
yöntemi ile, daha kısa idi (p<0.001) ve VL yönteminin DL yöntemine göre daha
kolay bir girişim metodu olduğu tespit edildi (p<0.001). VL yöntemi vokal kordları
görmede ve başarılı ETE yapmada daha hızlı ve daha kolay bir yöntemdir
Our study aimed to compare the
characteristics of intubation procedures with DL and VL methods on airway models
while driving in an ambulance with, paramedic and emergency medical technician
(EMT). In our methodological study, before the comparison of both methods,
intubation training was performed with DL and VL on a standard airway model for 1
hour theoretical and 3 hours practical. The data was collected by a questionnaire,
including the number of attempts at successful intubation, duration of intubation
attempts, duration of vocal cord sight, degree of convenience grade of both
intubation methods (1-very difficult; 10-very easy). Data was evaluated using SPSS
(version 20.0). Descriptive information was demonstrated with a number, percentage,
standard deviation and average. Shapiro-Wilk was used to evaluated the distribution
of data and Wilcoxon, Mc Nemar and Chi square tests were used for data analysis.
Two-way analysis of variance was performed in SAS University Edition to
investigate the common effect of multiple independent factors on a dependent
variable. Twenty-two (27.8%) male, participants with a mean age of 30.3 ± 4.5 (min:
22, max: 42) years were included in the study. Forty-four (55.7%) of the participants
were EMT. In the first attempt, the number of successful individuals was 58 (73.4%)
by DL, 61 (77.2%) by VL, and there was no difference between the two methods in
terms of success (p = 0.708). In the first attempt, the duration of visualization of the
vocal cords was shorter by VL method (p = 0.001). Successful intubation time was
shorter with VL (p <0.001) and VL was an easier intervention method (p<0.001)
than DL. VL is a faster and easier method for visualization and intubation of vocal
cords compared with DL method
