Amaç: Bu çalışmada Günlük Yaşam Aktiviteleri Modeli (GYAM) kullanılarak zihinsel
engelli birey (ZEB)’lere bakım veren ailelerin bakım gereksinimlerini karşılamada
yaşadıkları sorunları saptamak, sorunlara yönelik hemşirelik girişimlerini planlamak,
uygulamak ve değerlendirmek amaçlanmıştır.
Yöntem: Çalışmaya Eylül 2008-Haziran 2009 tarihleri arasında adresleri doğrulanan ve
ev ziyareti yapılmasını kabul eden, Eskişehir İli Tepebaşı Belediyesi’ne kayıtlı
mahallelerde oturan 100 ZEB ailesi dahil edilmiştir. Çalışmada verilerin toplanması üç
ev ziyareti biçiminde yapılmıştır. Birinci ev ziyaretinde; ZEB ve ailelerinin sosyo demografik özelliklerini, ailelerin engellilik ile ilgili bilgi ve uygulamalarını ve de ZEB
ve ailelerinin sorunlarını belirlemeye yönelik engelli yakın (EY)’larına anket formları
uygulanmıştır. İkinci ev ziyaretinde ZEB ve ailelerinin bakım sorunlarına yönelik uygun
hemşirelik girişimleri planlanarak ailelerin ilgili kurum ve kuruluşlara yönlendirmeleri
yapılmıştır. Üçüncü ev ziyaretinde ise bakım sorunlarına yönelik hemşirelik girişimleri
uygulanan ailelerin değerlendirilmesi yapılmıştır. Araştırmada elde edilen verilerin
değerlendirilmesinde; Wilcoxen işaret testi, Mann-Whitney U testi ve Kruskall-Wallis
H testi kullanılmıştır. Anlamlılık seviyesi olarak 0,05 kullanılmış olup, p<0,05 olması
durumunda anlamlı farklılığın olduğu belirlenmiştir.
Bulgular: GYAM’ne göre ZEB ve ailelerinin hemşirelik girişimleri öncesi ve
sonrasında saptanan bakım gereksinimleri puan ortalamaları (BGPO)’nın ZEB’in
cinsiyeti, öğrenim durumu, zeka düzeyi, eşlik eden engelinin olması ve ilaç kullanımı
açısından anlamlı farklılığın olduğu, ailelerin sosyo-demografik özellikleri açısından ise
anlamlı bir farklılığın olmadığı (p<0,05) saptanmıştır. ZEB ve ailelerinin hemşirelik
girişimleri sonrasında saptanan BGPO’nın hemşirelik girişimleri öncesine göre anlamlı
derecede düştüğü belirlenmiştir (p<0,05). Sonuç: ZEB ve ailelerinin bakım gereksinimlerinin giderilmesinde aileler ve ilgili
kurumlarla yapılan işbirliğinin sorunları çözmede olumlu etkisinin olduğu görülmüştür,
bu nedenle ZEB ve ailelerine yönelik sosyal projelerin arttırılması ve bu projelerde
hemşirelerin katılımı ve etkin rol oynamalarının sağlanması gerekmektedir.
Purpose: The aim of this study is to determine the problems that the families which
provide care for mentally disabled individuals (MDI) in fulfilling care requirements as
well as to plan, practice and evaluate nursing initiatives oriented to such problems by
using Activities of Daily Living Model (ADLM).
Method: The study is conducted with 100 MDI families registered in Tepebaşı
Municipality in Eskişehir, the addresses of which were confirmed between September
2008 and June 2009 and which accepted home visits. The data of the study were
collected through three home visits to the each family. In the first visit, the survey forms
were applied to the families of the disabled individuals in order to determine the socio demographic characteristics of MDIs and their families, the knowledge and applications
of the families about disablement as well as the problems of MDIs and their families. In
the second home visit, the appropriate nursing initiatives oriented to the care problems
of MDIs and their families were planned and the families were directed to the
concerned institutions and organizations. The third home visit was paid in order to
evaluate the families in which nursing initiatives about care problems were applied. In
the assessment of the data obtained from the research, Wilcoxon signed-rank test,
Mann-Whitney U test and Kruskall-Wallis H test were applied. The significance level is accepted as 0.05 and when p<0.05, a significant difference is found.
Findings: According to ADLM, significant differences are found between the mean
points of care requirements (APCR) of MDIs and their families before and after nursing
initiatives in terms of MDI’s sex, education level, intelligence level, lack of any
accompanying disability and medicine use while no significant difference (p<0.05) is
determined in terms of the socio-demographic characteristics of the families. It is found
that APCRs of MDIs and the families determined after nursing initiatives have
significantly decreased compared to the points before these initiatives (p<0.05).
Result: Cooperation with families and the concerned organizations are observed to have
positive effects on solutions of the problems about fulfillment of care requirements of
MDIs and their families. Therefore it is necessary to increase the social projects
oriented to MDIs and their families and to maintain involvement and active
participation of nurses into these projects.
