Bu çalışma Nisan 2007-Temmuz 2007 tarihleri arasında elektif cerrahi
uygulanacak 90 Kulak Burun Boğaz hastasında uygulanmıştır. Çalışmamızda,
anestezi indüksiyonu sırasında kullanılan propofol enjeksiyon ile oluşan ağrının
önlenmesinde ketamin ve meperidin etkinliklerini ve anestezi idamesine
hemodinamik etkilerinin karşılaştırılması amaçlandı. Operasyon öncesi 8 saat aç
bırakılan hastalara premedikasyon uygulanmadı. ASA I-II grubu 90 olgu rastgele 3
gruba ayrıldı. İndüksiyon öncesinde grup I’e 100µg/kg ketamin ,Grup II’e 40 mg
meperidin, Grup III’e 4 cc volümde % 0.9 NaCL uygulandı. Kirpik refleksi
kaybolana kadar propofol enjeksiyonuna devam edildi; kullanılan propofol dozu ve
enjeksiyon sırasında ilaç verilen elinde ağrı olup olmadığı sorgulandı ve kaydedildi.
Kirpik refleksi kaybolunca 0.5 mg/kg dozunda kas gevşetici olarak atrakuryum
verildi ve anestezi idamesi % 1-2 isofluron ile sağlandı. İndüksiyon, entübasyon,
operasyonun 3. ve 10. dk’sında sistolik arter basıncı (SAB), diyastolik arter basıncı
(DAB),ortalama arter basıncı (OAB), kalp atım hızı (KAH) ve periferik oksijen
saturasyonları (SpO2) kaydedildi. Enjeksiyon ağrısı açısından Grup I ile Grup III
arasında anlamlı fark (p<0.05), grup II ile grup III arasında anlamlı fark (p<0.05)
bulundu. Grup I ile Grup II arasında anlamlı bir fark (p>0.05) bulunamadı.Sonuç
olarak propofol enjeksiyon ağrısının önlenmesinde ketamin ve meperidinin etkileri
olmuştur fakat birbirlerine üstünlükleri saptanamamıştır. Birbirlerine alternatif olarak
kullanılabilirler.
This study was carried out with 90 patients in ear, nose and throat
unit performing elective surgery between April 2007 and July 2007. In our study, we
aimed to determine the effectiveness of ketamine and meperidine in prevention of
pain during propofol injection used in anesthesia induction and to compare their
hemodynamic effects on continuation of anesthesia. No premedication was
performed on the patients who were famished for 8 hours prior to the surgery. ASA
I-II group of 90 patients were randomly divided into three groups. Group I was
applied 100µg/kg of ketamine, Group II 40 mg of meperidine and Group II 4 cc of
NaCl with concentration of 0.9%. Propofol injection was carried on until the eyelash
reflex vanished and then dose of propofol used was recorded. The patient was asked
whether he had pain in his hand drug was injected and this was also recorded. When
the eyelash reflex vanished, it was given 0.5 mg/kg of atracurium as antipasmodic,
and continuation of anesthesia was carried on with 1-2 % of isofluran. Induction,
intubation, systolic arterial pressures (SAP), diastolic arterial pressures (DAP),
average arterial pressures (AAP), heart beat rates (HBR) and peripheric oxygen
saturations (SpO2) at the third and tenth minutes of the operation was recorded. In
terms of injection pain, we found a significant difference both between Group I and
Group III (p<0,05) and between Group II and Group III (p<0,05). However, no
significant difference was found between Group I and Group II (p<0,05). As a result,
ketamine and meperidine had effects on prevention of propofol injection pain but
neither of them was superior to another. Thus, they could be used as an alternative
for each other.
