cerrahisinden sonra levobupivakain i le parasternal blok ve lokal
anestezik infiltrasyonu uygulamasının; postoperatif agrı,
pulmoner fonksiyon ve ekstübasyon zamanına etkilerinin

Koroner arter cerrahisinden sonra analjeziyi saglamak için kullanılan çesitli klasik
yöntemler mevcuttur. Heparin kullanılan bu hasta grubunda rejyonel yöntemler
riskleri nedeni ile tartısmalıdır. Parasternal blok ve lokal infiltrasyon uygulaması
yeni bir yöntemdir. Bu teknigin; postoperatif agrı, analjezik kullanım miktarı,
solunum fonksiyonları ve ekstübasyon zamanına etkilerini arastırmayı amaçladık.
Çalısmaya ASA III-IV risk grubu 20 hasta dahil edildi. Operasyondan 12 saat önce
portabl Spirometri cihazı kullanılarak ZEV-1 ve ZVK ölçümleri yapılarak
“ilk ölçüm” olarak kaydedildi, aynı islem postoperatif 6. 12. ve 24. saatlerde de
tekrarlandı. Cerrahi islem bi t iminde sternum kapanmadan önce cerrahi
ekip tarafından görerek; Grup P (plasebo)’ye 50 ml SF, grup LB’ye % 0.25 lik
levobupivakain’den; bilateral interkostal 5 seviyeye, sternumun her iki tarafına
periost üzerine ve gögüs tüplerinin giris yerlerine derin infiltrasyon seklinde toplam
50 ml volüm ve 1.35 mg/kg dozu asmayacak sekilde hesaplanarak enjekte edildi.
Postoperatif agrıları VAS skoru ile degerlendirilerek kaydedildi. Hastalara
postoperatif analjezi amaçlı morfin, hasta kontrollü analjezi yöntemi (HKA) ile i.v
uygulanarak, tüketilen ilaç miktarı mg cinsinden kaydedildi. Hastaların yogun
bakıma alınmasından itibaren entübasyon ve ekstübasyon zamanlarıda kaydedildi.
Elde edilen veriler istatistiksel olarak degerlendirildi. Ortalama VAS degeri grup
LB’de grup P’ye göre anlamlı olarak düsük bulundu (p<0,05). Her iki grubun
karsılastırılmasında ZEV-1 ve ZVK’de meydana gelen azalmalar arasında fark
bulunmadı ( P >0,05). 24 saatlik morfin tüketim miktarı; grup LB‘de
(32.8 ± 0.29 mg) grup P’ ye göre ( 45.4 ± 0.33 mg) belirgin olarak daha düsük
bulundu (p<0,001). Grup LB’ nin ortalama entübasyon süresi ve ortalama
ekstübasyon süresi grup P ‘ye göre daha kısa oldugu tespit edildi ( p<0.01).
Sonuç olarak, pompasız koroner arter by-pass greftleme cerrahisinden sonra,
levobupivakain ile parasternal blok ve lokal infiltrasyon uygulaması etkin
postoperatif analjezi saglayarak, hastaların ortalama VAS degerini azalttıgı, toplam
morfin tüketim miktarında azalmaya neden oldugu ve ekstübasyon zamanını
kısalttıgı, ancak solunum fonksiyonlarında meydana gelen azalmayı
engelleyemedigi saptandı.

of analgesia after coronary artery surgery. n the pat ient group where
heparin is administered, regional methods are discussed be cause of
their risks. Parasternal block and local infi l t rat ion technique is a new
method. We aimed to research the ef fect of this method on
postoperative pain, amount of analges ic consumpt ion, respi ratory
funct ions and the t iming of extubat ion. 20 pat ients of ASA III-IV are
included in this study. 12 hours before the operation, using portable Spirometer
device we measured the Forced Expiratory Volume in 1th seconds (FEV-1) and
Forced Vital Capacity (FVC) as the ‘first measurement’; the same procedure was
repeated at the postoperative 6th,12th and 24th hours. At the end of surgery, before
sternum is closed, under the vision of the surgeon; 50 ml of SF is administered to
group P (placebo), 50 ml of % 0,25 levobupivacaine to group LB; at the level of the
fifth intercostal level bilaterally to the both sides of sternum over the periosteum
and at the entrance of the chest tubes in a way of deep infiltration, below the dose of
1,35 mg/kg. Postoperative pain was evaluated with VAS score and recorded. For
the postoperative analgesia, .V morphin with PCA was administered and the
consumed drug amount was recorded as mg. ntubation and extubation times,
staying time in intensive care unit are also recorded. The collected data are
evaluated statistically. Average VAS level in group LB was found significantly
lower than group (P<0,05). The comparison of the decrease of FEV-1 and FVC was
not found to be significant (P>0,05). The 24-hour consumption of morphin amount
was found to be significantly lower in group LB (32,8 ± 0,22mg) than group P (45,4
± 0,33 mg) ( P<0,001). Average intubation time and avarage extubation time of
group LB was found to be shorter in group LB than group P (P<0,01). As a result
after OPCABG surgery, parasternal block and local infiltration with
levobupivacaine, was found to be effective in postoperative analgesia, decreasing
the avarage VAS levels, decreasing the total morphin consumption amount and
reducing the extubation time ; but couldnot prevent the decreasing of the respiratory
functions.
