Ankilozan spondilit (AS), aksiyel iskeleti ve periferik eklemleri tutan kronik, ilerleyici, inflamatuvar bir hastalıktır. Hastalık aktivitesini, progresyonunu ve prognozunu tam olarak hastalık durumunu tanımlamak zordur. Bu çalıĢma ile Türk AS hastalarında klinik, radyolojik ve eklem dıĢı tutulumu yansıtabilecek hastalıkla iliĢkili veya iliĢkisiz belirteçleri belirlemeyi amaçladık. 1984 Modifiye Newyork sınıflandırma ölçütlerine uyan 102 AS hastası çalıĢmaya alındı. Klinik özellikler, sosyal statü ve tedavi rejimlerini içeren bilgiler kaydedildi. Veriler hastalar tarafından anket doldurularak toplandı. Temel fizik muayeneleri yapıldı ve metrolojik indeksler ölçüldü. Eritrosit sedimentasyon hızı (ESH), C-reaktif protein (CRP) düzeylerine bakıldı. HLA-B27 pozitifliği değerlendirildi. Tüm radyolojik değiĢiklikler skorlandı. Radyolojik hasar (Bath Ankilozan Spondilit Radyoloji Ġndeksi, BASRI) ve fonksiyonel yetersizlik (Bath Ankilozan Spondilit Fonksiyonel Ġndeks, BASFI) AS için sonlanım ölçütü olarak alındı. Hastalık süresi, erkek cinsiyet, renal taĢ varlığı yüksek radyolojik skorla iliĢkili bulundu. Yüksek ESH düzeyi ve yüksek hastalık aktivite indeksi yüksek BASFI skoru ile iliĢkili bulundu. CRP düzeylerinin yüksekliği ve kalça tutulumu hem yüksek BASFI hem de yüksek BASRI skoru ile iliĢkili idi. AS sonlanımı için en önemli bağımsız belirleyici kalça tutulumu olarak saptandı. Böylece bu risk faktörlerine sahip bireyler saptandığında erken ve agresif tedavi ile fonsiyonel durum iyileĢitirilebilir ve hastalığın yaratabileceği hasar durdurulabilir veya geciktirilebilir.
Ankylosing spondylitis (AS) is a chronic progressive inflammatory disorder affecting the axial skeleton and peripheral joints. It is difficult to define the disease activity, progression and prognosis of AS. Our aim in this study was to define the markers which can associated with the clinical, radiological and extraarticular manifestations of the disease. One hundred and two patients with AS who fulfilled the 1984 Modified Newyork Criteria for diagnosis of AS were included for this study. Information regarding predictor variables, including clinical features, social statu and treatment regimens were recorded. Data were corroborated using questionnaries completed by the study patients. Physical examination was done and metrological indexes were measured. Erythrocyte sedimentation rate (ESR) and Creactive protein (CRP) levels were recorded. HLAB-27 positivity were evaluated. Full sets of radiographs were scored. Radiological damage (Bath Ankylosing Spondylitis Radiology Index, BASRI) and functional disability (Bath Ankylosing Spondylitis Functional Index, BASFI) were used to outcome measure of AS. The results showed that disase duration, male sex, renal stone occurrence were associated with higher radiological score. Hihger ESR levels and higher disease activity score were associated with high BASFI scores. Hihger CRP levels and hip involvement were both associated with high BASRI and BASFI scores. Radiological hip involvement was the most important independently factor for outcome in AS. Thus, when individuals with these risk factors can be detected earlier and more aggresive therapies may improve functional outcome and the damage of the disease may be prevented or delayed.
