Mesanenin evre T1 değişici epitel hücreli
karsinomu olan hastalar yüksek rekürrens, progresyon ve kanser ilişkili ölüm riskine
sahiptir. T1 evresinde olan tümörlerin prognozlarını değerlendirmek ve tedavi
stratejileri geliştirebilmek üzere çeşitli klinik ve patolojik parametreler ortaya
konmuştur. Son yıllarda tümörlerin moleküler biyolojik karakteristikleri ile ilişkili
prognostik faktörlerin ortaya konulabilmesine çalışılmaktadır. Maspin, tümör
süpresör özelliklere sahip, noninhibitör bir serpin (serin proteaz inhibitör) dir.
Literatürde mesane tümörlerinde maspin ekspresyonunun incelendiği az sayıda
çalışma mevcuttur. Ki-67 hücre siklusunun, G0 fazı hariç, tüm fazlarında eksprese
olan bir nükleer proteindir. Çok sayıdaki çalışma Ki-67’nin mesane kanser rekürrensi
ve progresyonu için bağımsız prognostik belirleyici olduğunu göstermektedir. Evre
T1 mesane tümörü olan 66 hastayı içeren çalışmamızda maspin ekspresyonunun
rekürrens riski açısından bağımsız bir öneme sahip olduğu saptandı (p<0.05). Maspin
ekspresyonunun negatif olduğu hastalarda maspin pozitif olanlara göre 2.046 kat
daha fazla rekürrens riski bulunduğu gözlendi. Ayrıca maspin negatif grubun daha
kısa rekürrenssiz ve progresyonsuz sağkalım süresine sahip olduğu belirlendi
(p<0.05). Ki-67 ile tümör rekürrensi, progresyonu ve tümör nedenli ölüm arasında
anlamlı bir korelasyona rastlanmadı (p>0.05). Bulgularımız evre T1 mesane
tümörlerinde maspin ekspresyonunun değerlendirilmesinin tümörün davranışının
tahmininde yararlı bir prognostik belirleyici olabileceğini düşündürmektedir.
Patients with stage T1
bladder transitional cell carcinoma have a high risk of recurrence, progression and
cancer-related death. A number of clinical and pathological parameters are used in
evaluating prognosis and in creating therapeutic strategies for T1 tumors. Special
attention has been paid in recent years to identifying prognostic factors related to the
molecular biological characteristics of the tumor. Maspin is a non-inhibitory serpin
(serine protease inhibitor) with tumor-suppressive properties. Only a few reports are
present in the literature on maspin expression in bladder tumors. Ki-67 is a nuclear
protein that is expressed in all stages of the cell cycle except in G0 phase. A large
number of studies define Ki-67 as an independent prognostic marker of bladder
cancer progression and recurrence. In our study consisting of 66 patients with stage
T1 bladder tumor, maspin expression was identified as an independent predictor of
recurrence (p<0.05). Patients with negative maspin expression were 2.046 times
more likely to relapse than patients with positive maspin expression. Furthermore,
maspin negative group was found to have a shorter recurrence and progression-free
survival (p<0.05). There was no correlation between the Ki-67 expression and tumor
recurrence, progression and tumor-related death (p>0.05). Our findings suggested
that the evaluation of maspin expression in stage T1 bladder tumors is useful
prognostic marker for predicting the tumor behavior.
