Akciğer kanserleri; artan tedavi olanaklarına rağmen tüm dünyada mortalitesi en yüksek olan kanser türüdür. Günümüzde aynı evre ve histopatolojik özelliklere sahip olan akciğer kanserlerinde benzer tedaviler verilmektedir. Ancak kemoterapiye yanıt, prognoz ve yaşam sürelerindeki farklılıklar halen açıklanamamaktadır ve bu konuda rutin olarak kullanılan testler henüz yetersizdir. Bu çalışmada yarı invaziv metodla alınan bronşial lavaj örneğinde; akciğer kanserine özgü genetik markerlerin özgüllük ve özgünlüğünün araştırılması hedeflenmiştir. Aynı zamanda EGFR, C-MYC, HER2’nin kemoterapiye yanıt, survi ve prognoz ile ilişkisi değerlendirilmiştir.
Bu çalışmada 100 akciğer kanserli olgunun 81’i Küçük Hücreli Dışı Akciğer Kanserli (KHDAK), 16’sı Küçük Hücreli Akciğer Kanserli (KHAK), 3’ü de metastatik akciğer kanseri olarak histopatolojik tanı almışlardır. Kontrol grubu olarak; akciğer kanseri olmayan 15 olgunun bronşial lavaj materyali FISH yöntemiyle analiz edilmiş ve normal olarak değerlendirilmiştir.
Ayrıca 100 olgudan 81’inin kesin histopatolojik tanıları 1. fiberoptik bronkoskopide (1.FOB) alınan biyopsi materyali analiziyle konulmuş; 19 olgunun kesin histopatolojik tanıları ise daha sonra yapılan 2.FOB, iğne biyopsisi, cerrahi yöntemlerle alınan biyopsi materyalleri analiziyle konulabilmiştir. 1.FOB’de kesin histopatolojik tanı konulamayan 19 olgudan 8’inde FISH pozitif bulgular saptanmıştır. Sadece üç genetik marker kullanılarak yapılan (1.FOB’de alınan) bronşial lavaj materyali analiziyle 100 olgudan 65’inde FISH pozitif sonuçlar elde edilmiştir. Kombine (histopatoloji ve FISH) olarak birlikte değerlendirildiğinde ise 89 olguda (% 89) anomali saptanmıştır. Bu çalışmada histopatolojinin sensitivitesi
% 81, spesifitesi % 100; FISH analiz yönteminin sensitivitesi % 65, spesifitesi % 100 olarak bulunmuştur.
FISH yöntemiyle analiz edilen 100 akciğer kanserli olguda; EGFR probu ile
% 46, C-MYC probu ile % 38, HER2 probu ile % 35 olguda FISH pozitif (+) sonuçlar saptanmıştır ve kemoterapiye yanıt, prognoz, survive ile ilişkileri değerlendirilmiştir. Verilerimiz, kullanılan üç genetik marker ile dahi, tanı ve tedaviye ilişkin bilgilere ulaşılabildiğini göstermiştir. Buna bağlı olarak bronkoskopi ile alınan bronşial lavaj örnekleri, farklı genetik markerlar kullanılarak analiz edildiğinde; erken tanı, prognoz değerlendirme ve hatta tedaviye yol gösterici olarak değerli bilgiler vereceği düşüncesindeyiz.
Despite of improving treatment protocols, lung cancers are still the most lethal types of cancers. Similar treatments are used in patients with the lung cancer at same stages and have histopathological features. However, differences in prognosis, chemotherapy response and survival time can not be explained yet and the tests used are not sufficient to answer these questions. The goal in this study was to prospectively aimed the sensitivity and specity of FISH detected genetic markers of lung cancer from semi non-invasive bronchial lavage samples of the patients undergoing bronchoscopy for suspected lung cancer. In the same time, the relations of FISH-detected EGFR, C-MYC, and HER2 gene copy number variations with the prognosis, chemotherapy response and survival time differences were evaluated.
In this study, analyses were performed on one hundred patients referred for bronchoscopy for suspicion of lung cancer. The histopathologic results of one hundred cases were as following: 81 NSCLC, 16 SCLC and three metastatic lung cancer. The samples from 15 cases without lung cancer suspicion but with bronchoscopic indication for differential diagnosis served as control samples, for bronchial lavage material, evaluated as normal.
During the first fiberoptic bronchoscopy (1.FOB), the histopathological diagnosis could be revealed in 81 cases of 100. The diagnosis of the remaining cases could be determined through the second FOB or needle biopsy or surgical applications. Of the 19 cases in which histopathological diagnosis could not be determined in the first FOB, eight of them showed cancer-related FISH positive results. Although only three FISH-detected genetic markers used, FISH positive results from the first FOB samples could be determined in 65 samples. In the bronchial lavage samples were obtained from first FOB, FISH positive results were obtained in 65 of 100 cases. The sensitivity and specity of histopathological analysis were 81 % and 100 % whereas they were 65% and 100% for FISH analysis, respectively. The combined histopathological and FISH analyses showed positive results in 89 cases (89%).
Of one hundred cases with lung cancer, 46% had EGFR, 38% had C-MYC and 35% had HER2 gene copy number abnormalities. The relations of these FISHdetected results with the prognosis, chemotherapy response and survive were evaluated. The data showed that even with three genetic markers relating to the diagnostic and prognostic clues could be reached. Therefore, the samples obtained from semi-invasive bronchial lavage technique might be informative in early diagnosis, prognosis determination, even in therapy determination if different genetic markers are used.
