Rahimiçi araçlar (RİA) dünyada ikinci, ülkemizde birinci sıklıkta kullanılan modern kontraseptif yöntemdir. Non-steroidal antiinflamatuar ilaçlar (NSAİİ) antibiyotiklerden sonra ikinci sıklıkta kullanılan ilaçlardır. Çalışmamızda, rahimiçi araçların etkinliğinde implantasyon faktörlerinden olan vasküler endotelyal büyüme faktörü (VEGF) ve lösemi inhibitör faktörün (LİF) etkileri ve bir NSAİİ olan piroksikam beta-siklodekstrinin (PBCD) RİA’ların kontraseptif etkinliği üzerine olan etkileri araştırılmıştır. Araştırma ratlarda iki deney şeklinde dizayn edilmiştir. İlk deneyde RİA ve PBCD’nin etkileri embriyo sayısı ile, ikinci deneyde ise VEGF ve LİF ile araştırılmıştır. Çift hornu olan ratlarda bir hornda RİA olması, o hornda kontraseptif etki gösterirken diğer hornda etki göstermemektedir (p<0.05). PBCD, RİA’nın kontraseptif etkisini azaltmaktadır (p<0.05). Ancak PBCD’nin fertilite üzerine olumsuz etkisi olmamaktadır. RİA’nın, implantasyon faktörleri olan VEGF ve LİF üzerine etkisi saptanmamıştır. Ancak RİA endometriumda damar sayısını belirgin olarak artırmıştır (p<0.05). PBCD, LİF üzerine etkisi olmazken VEGF ekspresyonunu azaltmaktadır (p<0.05). Bununla birlikte PBCD, RİA olan hornda damar sayısını da azaltmaktadır (p<0.05). Sonuç olarak; PBCD belli dozlarda gebelik oluşumu üzerine olumsuz etkisi olmayan bunun aksine RİA’ların antifertilite etkilerini azaltan bir NSAİ ilaçtır. Dolayısı ile RİA kullanıcılarında NSAİD ihtiyacı olduğunda bunun dikkatle değerlendirilmesi gerekir ve olguların gebelik konusunda uyarılmaları gereklidir. Ayrıca RİA etkinliğinde, VEGF ve LİF’in rolü olmazken PBCD’nin VEGF ekspresyonu üzerine azaltıcı etkisi vardır.
Intrauterine devices (IUDs) are the second commonly used modern contraceptive method in the world and the most common used contraceptive method in Turkey. Non-steriodal antiinflamatuary drugs (NSAIDs) are the most common used drug following antibiotics. In our study we researched the effect of vascular endothelial growth factor (VEGF) and leukemia inhibitory factor (LIF), implantation factors, on contraceptive action of IUD and the effect of piroxicam beta-cyclodextrin (PBCD), NSAID, on contraceptive action of IUD by using immunuhistochemical modalities and the number of embryos on rat model. Study design composed of two experiments with rats: effects of IUD and PBCD compared with the number of rat embryos in the first experiment and VEGF and LIF in the second experiment. IUD had no action in the empty horn although IUD had contraceptive affect in the horn which it was inserted (p<0.05). PBCD abolished the contraceptive effect of IUD (p<0.05). PBCD had no negative effect on fertility. IUD had no effect on the expression of VEGF and LIF. IUD significantly increased the number of vessels (p<0.05). Although PBCD had no action on LIF expression, it decreased VEGF expression (p<0.05). PBCD decreased the number of vessels in which IUD was inserted (p<0.05).
To conclude, PBCD is a NSAID which decreased the IUD efficacy. Hence IUD users should be carefully followed up regarding the pregnancy should they begin to use NSAID. Based on the result of this study, the mode of action of IUD was not found to be through the alteration of expression VEGF and LIF. In addition, PBCD decreased the expression of VEGF.
