Hipertonik solüsyonlar ve pentoxifillinin sepsisteki karşılaştırmalı etkileri.

Uzmanlık Tezi, Eskişehir,2007.Hipertonik solüsyonlar ve pentoxifillinin sepsisteki
etkilerini araştırmak amacıyla 32 adet Spraque-Dawley cinsi rat kullanıldı. Deney
hayvanları 8’er adet ratdan oluşan sepsis-laparotomi(kontrol), hipertonik solüsyonlar (HS),
pentoxifillinin (PTX), hipertonik solüsyonların ve pentoxifillinin (HS+PTX) birlikte
kullanıldığı dört gruba ayrıldılar. Postoperatif 24 saat süre ile ratlar takibe alındı. Sepsis
modeli oluşturmak üzere bütün gruplara çekal-ligasyon ve perforasyon yapıldı ve tüm
ratlarda 24 saat sonra rektal yoldan ateş ölçümü ve relaparotomi uygulandı. Hematolojik
parametreler için intrakardiak kan alınıp, doku örneklerinin(karaciğer, akciğer, ileum)
histopatolojik incelemeleri yapıldı. Sepsis oluşturulan, relaparotomi yapılan, her grupta
8’er adet bulunan deneklerde mortalite oranına bakıldı. Sonuç olarak hipertonik
solüsyonlar ve pentoxifillinin sepsisin erken dönemlerinde kullanılmasının başarılı olduğu
görüldü. Sepsis modeli oluşturulduktan sonra yapılan hematolojik, histopatolojik inceleme
sonucunda ve mortalite oranları değerlendirildiğinde hipertonik solüsyonlar ve
pentoxifillinin birlikte sepsisin erken dönemlerinde inflamatuar yanıtı azaltarak, etkili ve
yararlı olduğunu düşünmekteyiz.

study to compare the effects of hypertonic solutions and pentoxifylline in sepsis, 32
Sprague-Dawley rats were used. Experimental sepsis was induced by cecal ligation and
puncture in all rats. At the end of the septic shock period animals were divided into four
groups according to the treatment received as follows; sham (cannulation only, sepsis, no
resuscitation), HS (3% NaCl 10 ml/kg/hr), PTX (6 mg/kg/hr) and HS+PTX. Twenty four
hours resuscitation after surgery, rats were relaparotomized. Blood obtained from heart for
hematologic parameters and tissue samples(liver, lung, ileum) were collected for
histopathologic analization. The body temparature of animals was obtained throughout the
end of the experiment. Mortality rates of all groups were evaluated and compared. In
conclusion, hypertonic solutions and pentoxifyllin were useful in treating sepsis.
According to the hematologic and histopathologic assesments and the evaluation of the
mortality rates, early coadministration of hypertonic solutions and pentoxifyllin reduces
the incidence of mortality and morbidity due to septic shock and sepsis. Although the
experimental data are positive and the rationale for use of hypertonic solutions in the care
of patients in sepsis or septic shock is reasonable, definitive clinical studies as to the safety
and efficacy in this population are still required.
