Eskisehir’de Tıp Fakültesi Hastanesi’nde vankomisin dirençli
enterokokların moleküler epidemiyolojik analizi. Eskisehir Osmangazi

Tezi, Eskisehir, 2007. Bu çalısmada bir yıllık bir zaman dilimi içinde ESOGÜ
Tıp Fakültesi Hastanesi yogun bakım ünitesi hastalarında, haftada bir kez
alınan sürveyans kültürleriyle VRE gastrointestinal sistem tasıyılıgının
arastırılması ve elde edilen VRE izolatlarının direnç genotiplerinin ve genetik
yakınlıklarının belirlenmesi amaçlanmıstır. Çalısma süresince çesitli klinik
örneklerden patojen olarak soyutlanan VRE susları da çalısmaya dahil
edilmistir. Susların identifikasyonu VITEK 2 Compact Sistem GP kartları ile,
antibiyotik duyarlılıkları VITEK 2 Compact Sistem AST kartları ve E-test
yöntemi kullanılarak belirlenmistir. Glikopeptid direnç genotipi saptamada
multiplex PCR, klonal iliskinin belirlenmesinde ise PFGE yöntemleri
kullanılmıstır. Sürveyans kültürlerinin % 4,8’inde (58/1200), klinik örneklerden
izole edilen enterokok izolatlarının % 0,5’inde (3/529) VRE saptandı. Bu
susların tümü E. faecium olarak tanımlandı. E-test ile susların tümünde
vankomisin ve teikoplanin M K degerleri 256 mg/L üzerinde bulundu. Susların
hiçbirinde quinopristin-dalfopristin ve linezolid direncine rastlanmadı.
Moleküler testlerle susların hepsinin van A genotipinde oldugu ve tek bir
klondan köken aldıkları saptandı. Aynı dönemde VRE
enfeksiyonu/kolonizasyonu saptanan hastaların büyük bölümü (5/7) pediatri
YBÜ hastalarıydı. Hastanemizde ilk olarak 2002 yılında kolonizan olarak 2
VRE susunun tanımlanmasını takiben 2005 yılında alınan sürveyans
kültürlerinde % 5,9 (25/420) oranında VRE saptanması ve bizim bulgularımız
hastanemizde VRE enfeksiyonlarının karsımıza çıkmaya devam edecegini
düsündürmektedir. Bu nedenle VRE tespiti ve yayılımın önlenmesi
konusunda etkin enfeksiyon kontrol uygulamaları bu sorunun gelecekteki
boyutunu belirlemede anahtar rol oynayacaktır.
The molecular epidemiologic analysis of vancomycin resistant
enterococci in a university hospital in Eskisehir. Eskisehir Osmangazi
University Faculty of Medicine, Medical Specialty Thesis in Department of
Microbiology, Eskisehir, 2007. The aim of this study is to investigate
gastrointestinal carriage of VRE at one year period in patients that admitted to
intensive care units at Eskisehir Osmangazi University Medical Faculty Hospital
by taking surveillance cultures weekly, and to determine the genotypes of
vancomycin resistance and clonal relationship in VRE strains. During the study
period VRE strains isolated from various clinical specimens were also included
in the study. The isolated strains were identified to species level by VITEK 2
Compact System GP cards, and antimicrobial susceptibility testing was
determined by VITEK 2 Compact System AST cards and E test method.
Determination of glycopeptide resistance genotypes were performed by
multiplex PCR and, clonal relations were defined by PFGE method. The ratios
of VRE in surveillance cultures and among enterococci isolates of clinical
specimens were was 4.8% (58/1200) and 0.5% (3/529) respectively. All of the
VRE strains were identified as E.faecium. MICs of vancomycin and teicoplanin
which were tested by E test method were greater than 256 mg/L for all isolates.
No strain was found resistant to quinopristin-dalfopristin and linezolid. t was
detected all strains which tested by molecular methods had van A genotype and
showed relation with one clonal type. At the same period, most of the patients
(5/7) who had VRE colonization were in pediatric intensive care unit. After 2
VRE strains were defined as colonizan in our hospital in 2002, detection of VRE
ratio as 5,9 % (25/420) in surveillance cultures in 2005 and our results made us
considered that VRE infections will go along in our hospital. Therefore, effective
infection control applications about VRE detection and preventing, VRE
transmisson will play a key role in determining the wideness of this problem at
the future.
