Bu
çalısma Aralık 2006-Mart 2007 tarihleri arasında elektif cerrahi uygulanacak 90
Kulak Burun Bogaz hastasında uygulanmıstır. Çalısmamızda; anestezi indüksiyon ve
idamesinde kullanılan Propofolün iki farklı formülasyonunun enjeksiyon agrısı ve
entübasyona verilen yanıtlar açısından karsılastırılması amaçlandı. Operasyon öncesi
8 saat aç bırakılan hastalara premedikasyon uygulanmadı. ASA I-II grubu 90 olgu
rastgele 3 gruba ayrıldı. ndüksiyonda grup I’e %1 propofol 600 ml/saat, grup II’ye
%2 propofol 600 ml/saat ve grup III’e ise %2 propofol 300 ml/saat hızında verildi.
Kirpik refleksi kaybolana kadar infüzyona devam edildi ve giden ilaç miktarları ve
infüzyon süreleri kaydedildi. Kirpik refleksi kaybolunca hastalara 0,5 mg/kg dozunda
kas gevsetici olarak atraküryum verildi ve propofol infüzyonu 6-8 mg/kg/saat olarak
verilmeye devam edildi. Operasyon boyunca hastaların sistolik arter basıncı (SAB),
diastolik arter basıncı (DAB), ortalama arter basıncı (OAB), kalp atım hızı (KAH) ve
periferik oksijen satürasyonları (SpO2) kaydedi ldi . Hastalara indüksiyon
sırasında i laç veri len el inde agrısı olup olmadıgı soruldu ve
kaydedi ldi . Enjeksiyon agrısı açısından gruplar arasında bi r fark
bulunmazken olusan hafif derecede agrı açısından grup 1 ile grup 2 arasında fark
(p<0,05), grup 1 ile grup 3 arasında önemli derecede fark (p<0,01) bulundu. Hafif
derecede enjeksiyon agrısı açısından grup 2 ile grup 3 arasında istatistiksel açıdan
anlamlı bir fark yoktu (p>0,05). Sonuç olarak %1’lik propofol enjeksiyon agrısı
açısından avantajlıdır. Ancak enjeksiyon agrı insidansı ve entübasyona olan
hemodinamik yanıtlar açısından benzer özellik tasıyan ve daha az lipit içermesi
sebebiyle sınırda lipidemi veya hipertrigliseridemisi olan hastalarda %2’lik propofol
iyi bir alternatif olabilir.
This study was carried out with 90 patients
having ear, nose and and throat issues who would have elective surgery betweeen
December 2006 and March 2007. In our study, it was aimed to compare two different
formulations of propofol used during and after anaesthesia induction by means of
their response to injection pain and intubation. Patients who were not nourished for 8
hours prior to the surgery were not premedicated. 90 cases from Group ASA I-II
were randomly seperated into three groups. In the induction, Group I was given
propofol of 1% at a rate of 600 ml/hr, Group II 2% at 600 ml/hr and Group III 2% at
300 ml/hr. Infusion was performed until eyelash reflex terminated and drugs
consumed and infusion times were recorded. After termination of eyelash reflex,
patients were given atrakuryum of 0.5 mg/kg as muscle relaxant and propofol
infusion was continued to be given as 6-8 mg/kg/hr. During the operation, sistolic
arterial pressure, diastolic arterial pressure, mean arterial pressure, heart rate and
peripheric oxygen saturations of patients were recorded. While induction
period,patients were asked whether they had pain in their hands during propofol
infusion and these data were recorded. A difference between Group I and Group II
(p‹ 0.05 ) and a significant difference between Group I and Group III (p‹ 0.01 ) was
found by means of mild pain whereas no difference was found by means of injection
pain. Statistically, there was no significant difference between Group II and Group
III by means of mild injection pain (p‹ 0.05 ). In conclusion, 1% propofol is
advantageous in terms of injection pain. However, 2% propofol may be good
alternative because of its lower lipid content and being i good choice in patients with
hyperlipidemia and hypertrigliseridemia as well as the comparable hemodynamic
responses to intubation and injection pain.
