Despite the fact that peripheral nervous system has the
capacity for axonal regeneration, full functional recovery is seldom seen after
peripheral nerve repair. Currently, we can assume that, the applied microsurgical
neurorrhaphy technique has reached its plateau. Several agents have been studied as
an adjuvant to nerve repair but none has reached a clinical application stage. We, in
this study, evaluated the response to sildenafil citrate and erythropoietin given
intraperitoneally for 30 days after nerve repair in rats with lacerating type nerve
injury, from functional and histological perspective. As a result, it has been seen that,
the effect of sildenafil citrate on regeneration is statistically better than the control
and erythropoietin groups. It is concluded that, sildenafil citrate therapy can be
applied with success to patients who have undergone surgical nerve repair in the
postoperative period.
Periferik sinir sisteminde aksonal rejenerasyon kapasitesi bulunmasına rağmen, periferik sinir onarımını takiben tam fonksiyonel iyileşme nadir görülmektedir. Günümüzde, uygulanan mikrocerrahi tekniğin bir platoya ulaştığını kabul edebiliriz. Sinir onarımını takiben pekçok farmakolojik ajan denenmiştir. Ancak hiçbiri klinik uygulama aşamasına erişememiştir. Biz, bu çalışmamızda, ratlarda kesilme tipi siyatik sinir yaralanmalarını epinöral koaptasyonla onardıktan sonra, 30 gün boyunca, intraperitoneal yolla sildenafil sitrat ve eritropoetin uygulayarak, elde edilen yanıtı fonksiyonel ve histolojik açıdan değerlendirdik. Sonuçta, sildenafil sitrat’ın sinir rejenerasyonuna etkisinin, kontrol ve eritropoetin grubuna göre, istatistiksel olarak üstün olduğu görülmüştür. Sildenafil sitrat tedavisinin, sinir onarımı yapılan hastaların postop döneminde başarıyla uygulanabileceği düşünülmüştür.
