Gelismis ülkelerde kromozom anomalilerinin sebep oldugu sendromların prenatal
tanısı obstetrik takibin önemli bir parçasıdır. Anöploidi tanı testlerinde altın standart, klasik
kromozom analizidir (karyotipleme). Bununla birlikte Florasan In Situ Hibridizasyon ( FISH )
ve short tandem repeats (STRs) analizleri moleküler sitogenetik ve moleküler genetik testler
olarak spesifik anomalilerin tanısında rutin uygulamada kullanılmaktadır.
MLPA; farklı DNA dizilerinin tek bir reaksiyonda relatif kantitasyonuna imkan veren
yeni bir teknik olarak ortaya çıkmıstır. Retrospektif ve prospektif olarak yapılan çalısmamızda
anöploidi riski tasıyan 500 gebeden alınan amniyon sıvısında MLPA teknigi kullanılarak 13.,
18., 21., X ve Y kromozomlarının doz tayini yapılmıstır.
Çalısmamızda, riskli gebelerden alınarak Ana Bilim Dalı’mıza gönderilen amniyotik
sıvıdan 2 cc ayrılarak fetal hücrelerin DNA ları ekstrakte edilmistir. Fetal DNA daki 13., 18.,
21., X ve Y kromozomlarının dozajlarının belirlenmesi amacı ile bu kromozomlara spesifik
probların PCR ile amplifikasyonu yapılmıstır. Elde edilen PCR ürününde 310 Prizm Genetic
Analyzer cihazında Gene Mapper programı kullanılarak amplifiye olan probların miktarını
gösteren pikler elde edilmistir. Piklerin alanları Coffalyser programı ile analiz edilerek adı
geçen kromozomların doz tayini yapılmıstır.
MLPA teknigi ile elde edilen sonuçlar rutin yöntemlerle elde edilen sonuçlar ile
karsılastırılarak testin sensitivitesi ve spesifitesi belirlenmistir. MLPA tekniginin rutinde
kullanılabilirligi sınanmıs ve yeni yöntemin rutinde kullanılan yöntemlere göre avantaj ve
dezavantajları belirlenmistir.
Yapılan çalısma sonrasında örneklerin 480 tanesinde MLPA yöntemi ile sonuçlar tayin
edilebilmistir. Triploidi saptanan bir olgu (69,XXX) dısındaki, tüm olgu örneklerinde karyotip
sonuçları ile MLPA sonuçları uyumlu bulunmustur.
Prenatal diagnosis of chromosome syndromes is a long established part of obstetric
care in developed countries. Karyotyping has provided the gold standard as a diagnostic test.
Nevertheless, molecular tests, such as fluorescent in situ hiybridisation (FISH) and short
tandem repeats (STRs) analysis, are now in common practice for the diagnosis of spesific
abnormalities.
MLPA is a new technique, that provides relative quantification of different DNA
sequences by a single tube reaction. In our study, we determined retrospectively and
prospectively the dosages of 13., 18., 21., X and Y chromosomes from amniocentesis samples
of 500 pregnants by using MLPA technique.
The DNA is extracted from 2 ml amniotic fluid samples of pregnancies at increased
risk of aneuploidy that are referred to our department. The PCR reactions by using
chromosomes 13., 18., 21., X and Y specific probes performed to quantify the amounts of
these chromosomes in fetal DNAs. The amount of peaks related with the amplified probes
analysed by using Gene Mapper Programme in 310 ABI Prism Genetic Analyser System and
then each peak area analysed by the Coffalyser programme to determine doses of
chromosomes 13., 18., 21., X and Y.
By comparing the MLPA-detected results and the findings from conventional
cytogenetic analyses, we determined the specifity and sensitivity of the MLPA technique and
the usage of the MLPA technique as a routine diagnostic method for aneuploidy detection.
There was 480 conclusive MLPA tests. Results were concordant with those of
karyotyping with the exception of the 69, XXX karyotyped sample.
