Diyabet Poliklinik Materyalinin Retrospektif Degerlendirilmesi.

amacıyla, ESOGÜTF (Eskisehir Osmangazi Üniversitesi Tıp Fakültesi)
Endokrinoloji ve Metabolizma Hastalıkları Bilim Dalı poliklinigine 1989-2005
yılları arasında müracat 3198 diyabetik hastanın degerlendirilmesi yapılmıstır.
Hastaların yası, cinsiyeti, vücut kitle indeksi, diyabet baslama yası ve baslangıç
biçimi, diyabet süresi, diyabet tipi ve tedavisi, takip sıklıgı ve süresi, aile öyküsü, iri
dogum ve sigara öyküsü, diyabete iliskin akut komplikasyonlar ve retinopati,
nöropati ve nefropati gibi spesifik komplikasyonlar ve diyabet seyrinde sık
karsılasılan durumlar (dislipidemi, kardiyovasküler hastalık, hipertansiyon,
kolelitiazis, hepatik steatoz, karaciger enzim anormallikleri, osteoporoz, tiroid
anormallikleri) degerlendirilmis ve bunların birbiriyle iliskileri arastırılmıstır. Hasta
grubumuzun %9.4’ ü tip 1, %85.7’si tip 2, %4.9’u gestasyonel diyabetik hastalardan
olusmaktadır. Tip 2 diyabetik hastaların %41.2’sinin tesadüfen tanı aldıgı tespit
edilmistir. Genel olarak glisemisi kontrolsüz olanlarda ve takip dısı hastalarda akut
metabolik komplikasyonlarla karsılasılmıstır. Tüm spesifik komplikasyonların
gelisimi birbiri ile ve diyabet süresi ile paralellik göstermistir. Mikrovasküler
komplikasyonlar gliseminin kötü olması ve diyabet süresinin uzun olmasından
etkilenirken, makrovasküler komplikasyonların bu durumdan bagımsız oldugu
görülmüstür. Tip 2 diyabetik hastalarda en sık OAD(%45) kullanıldıgı, tüm diyabetik
hastaların %53.2’sinde steatoz, %22.4’ünün tiroid sorunu ve erkeklerin %54.3’ünde,
kadınların %54.4’ünde osteoporoz oldugu görülmüstür.
Sonuç olarak, diyabetes mellitus özellikle arastırılmalıdır ve daha iyi seyri için
düzenli takip önemlidir.
Retrospective evaluation of the material of diabetes polyclinic. Eskisehir
Osmangazi University (ESOGU) Faculty of Medicine, Medical Speciality Thesis in
Department of Internal Medicine. Eskisehir, 2007. In order to evaluate progress of diabetes
mellitus (DM), in patients attended to outpatient clinic in ESOGU, Medical School, Division
of Endocrinology between 1989-2005, 3198 patients with diabetes were reviewed. The
characteristics (age, sex, body mass index, age and manner of beginning of diabetes, duration
of diabetes, type of diabetes and its treatment, duration and frequency of following, family
history for diabetes, macrosomic baby and smoking history, acute and all specific
complications of diabetes such as retinopathy, neuropathy and nephropathy and a number of
problems including dyslipidemia, cardiovascular diseases, hypertension, cholelithiasis,
hepatic steatosis and abnormalities of liver enzymes, osteoporosis, thyroid abnormalities) of
these patients and relations of these characterizations were investigated. Of our patients, 9.4
% were type 1 DM, 85.7% were type 2 DM and 4.9 % were gestational DM. 41.2 % of Type
2 diabetics were diagnosed coincidentally. In general, acute metabolic complications were
usually seen in patients who had uncontrolled glycemia or haven’t come to controls regularly.
Progression of all specific complications were corraleted with each other and with duration of
diabetes as well. Microvascular complications had been affected by uncontrolled glycemia
and long duration of disease although macrovascular complications had been found
independent from these states. The most common treatment type of Type 2 diabetic patients
with a frequency of 45 % were oral antidiabetic agents (OAD). In all diabetic patients, 53.2
% had steatosis, 22.4 % had thyroid pathology. 54.3 of male patients and 54.4 % of the
females had osteoporosis. In conclusion, DM must be investigated specially and regular
follow up is important for better prognosis.
