Giriş: Serviks kanseri; vajina yüzeyini döşeyen dokuyla, serviksin iç yüzeyini
döşeyen dokunun kesiştiği transformasyon zonu adı verilen yerde başlayan yassı epitel
hücre karsinomudur. Serviks kanseri tüm dünyada kadınlarda kansere bağlı ölümler
arasında ikinci sırada yer alır. Epidemiyolojik çalışmalar, serviks kanseri için primer
risk faktörünün insan papilloma virüsü (HPV) enfeksiyonu olduğunu göstermektedir.
Amaç: Çalışmamızın amacı, preinvaziv lezyon aşamaları (CIN sınıflandırması)
tespit edilen ve HPV tiplendirmesi yapılan 50 hasta (HPV+) ve 10 kontrol (HPV -)
olgusuna ait sıvı bazlı sitolojik örneklerle (thin prep) FISH yöntemi kullanılarak,
hastalıkla ilişkilendirilmiş ve literatürde daha önce tanımlanan olan 3q26 (TERC) ve
8q24.1 (C-MYC) bölgesindeki amplifikasyonları ve 7. kromozomun anöploidileri
incelemektir. Ayrıca literatürde yayınlanan diğer çalışmalarla karşılaştırarak, saptanan
anomalilerin, hastalığın premalign evrelerinin takibinde tarama yöntemleri ile birlikte
kullanılabilirliğini incelemektir.
Gereç-Yöntem: HPV enfeksiyonu saptanmış, preinvaziv servikal lezyon
aşamaları tespit edilen 50 olgu ve 10 kontrol grubundan alınan thin prep pap smear
örneği FISH analizi ile değerlendirilmiştir. FISH analizi ile 3q26 ve 8q24 bölgelerindeki
amplifikasyonlar ve 7. kromozomun sayısal anomalileri incelenmiştir.
Bulgular: Olguların %28 inde (14/50) FISH analizi ile en az bir anomali
gözlenmiştir. Olguların % 22 sinde (11/50) C-MYC amplifikasyonu görülmüştür. C MYC amplifikasyonu gözlenen olguların 4 ünde TERC amplifikasyonu da görülmüştür.
Olguların %6 sında (3/50) trizomi 7 tespit edilmiştir. Anomali görülen olguların %85.7
sinin (12/14) yüksek riskli HPV ile, %14.3 ünün (2/14) düşük riskli HPV ile enfekte
olduğu gözlenmiştir.
Sonuç ve tartışma: En sık saptadığımız anomali C-MYC gen amplifikasyonudur
(%22). C-MYC amplifikasyonlarının % 81.8 i yüksek riskli HPV tiplerinde görülmüştür
ve literatür ile uyumludur (P<0.05). Çalışmamızda anomalili olguların %85.7 sinin
yüksek riskli HPV ile enfekte olması (P=0,020), kromozom anomalilerinin tespitinin en
az HPV tip belirlenmesi kadar hastalığın takibinde önemli olduğunu göstermiştir.
Çalışmamız Türk populasyonunda bu konuda yapılmış ilk ve tek çalışma olma
özelliğini taşımaktadır.
Background: Cervix carcinoma is formed at transformation zone which is the
interception area of vagina surface tissue and inner surface tissue of the cervix. All
over the world, cervical cancer is the second cause of death among the women.
Studies that have been done are indicated that human papillomavirus (HPV)
infection is the primer reason of the cervix cancer.
Aim: In our study to observe preinvasive lesion stages (CIN classification)
diagnosed and HPV types done 50 cases and 10 control groups. Liquid based
cytologic specimens (thin prep) are going to be observed based on the amplification
in 3q26 (TERC) and 8q24.1 (C-MYC) area and the trisomy of the 7th chromosome,
which are determined in literature before, by FISH technique. Also these obtain
data is going to be compared with the previously printed studies and use following
the premalign lesions with the scanning method will be investigated.
Methods: HPV infected (+) and preinvasive cervical lesions stages diagnosis 50
cases and 10 control thin prep pap smear specimens were determined by FISH
analysis. amplification in 3q26 and 8q24 area and numerical abnormality of the
chromosome 7 were examined by FISH analysis.
Results: Twenty-eight percent (14/50) of the patients were positive for at least one
abnormality by FISH. C-MYC amplification were detected in %22 (11/50) of cases.
Four of cases showed co-amplification C-MYC and TERC. Trisomy of 7 were
detected in %6 (3/50) of cases. %85.7 (12/14) of cases abnormality detected
infected with high risk HPV and %14.3 (2/14) of cases abnormality detected
infected with low risk HPV were observed.
Conclusion: C-MYC gene amplification was the most frequent abnormality
among our patients. %81.8 of C-MYC amplification was detected with high risk
HPV types and consistent with the previous literature (P<0,05). %85.7 of with
abnormality cases infected with high risk HPV (P=0,020),chromosome
abnormality at least HPV type indicate important role of prognosis. Our study is
important due to being the first study and unique at this topic in Turkish
population.
