Ratlarda mesane İskemi/Reperfüzyon
(İ/R) hasarına leptinin koruyucu etkisini immünohistokimyasal yöntemlerle apoptozis
üzerinden değerlendirmek. Yöntemler: Bu deneysel çalışmada 24 adet Spraque-Dawley cinsi
albino ratlar kullanıldı. Deney grupları 8’er adet ratdan oluşmaktaydı.
Grup I (n═8): Kontrol Grubu, Grup II (n═8): İ/R Grubu, Grup III: (n═8):Leptin + İ/R
Grubu
Kontrol grubundaki ratlara herhangi bir işlem uygulanmaksızın alt abdominal kesi ile
mesaneleri çıkarıldı. İ/R grubunda iskemi oluşturmak için aort bifurkasyonun hemen
üzerinden abdominal aorta 45 dk. oklüze edildi, takiben 60 dk. reperfüzyon yapıldı. L+İ/R
grubunda ise iskemi başlatılmadan 30 dk. önce intra-peritoneal leptin uygulandı. Her iki
grupta reperfüzyon süresinin sonunda ratlar sakrifiye edilerek mesane dokuları alındı. Doku
malondialdehit (MDA) ve TNF-α düzeyleri ile biyokimyasal, ışık mikroskobunda
Hematoksilen&Eozin boyama ile histopatolojik, TUNEL ve bcl–2 boyama yötemleriyle de
apoptozis değerlendiridi. Sonuçlar: İ/R grubunda artan doku MDA ve TNF-α düzeyleri leptin
tedavisi verilen (L+İ/R) grupta önemli derecede azalmıştı (p<0.01). Histopatolojik olarak
gruplar karşılaştırıldığında leptin verilen tedavi grubunda (L+İ/R), polimorf nüveli lökosit
(PNL) infiltrasyonu ve konjesyonda azalma tespit edildi. Ürotelyum, stroma ve kas
dokusundaki TUNEL pozitif hücre sayıları İ/R grubunda belirgin olarak arttığı saptanırken,
leptin verilen tedavi grubunda ise azalma gözlendi (p<0.01). Bcl–2 ile yapılan
immünohistokimyasal boyama yönteminde İ/R grubunda sık rastlanan apoptotik hücrelerin,
L+İ/R grubunda daha az izlendiği saptandı. Verilerimiz leptinin rat mesanesinde İ/R hasarı
üzerine koruyucu etkisinin olduğunu göstermiştir.
To investigate the protective effect of leptin
in rats with induced ischemia/reperfusion (I/R) in bladder using immunohistochemical
methods for apoptosis and biochemical parameters. Methods: In this experimental study, 24
albino rats from Spraque-Dawley genus were used. Each experimental group contained 8 rats.
Group I (n═8): Control Group, Group II (n═8): I/R Group, Group III: (n═8):Leptin + I/R
Group
Bladders of rats in control group were removed using lower abdominal incision
without performing any procedure. In I/R group, abdominal aorta was occluded just upper
side of aortic bifurcation for 45 minutes for inducing ischemia, and then reperfusion was
performed for 60 minutes. In L+I/R group, intraperitoneal leptin was administered 30 minutes
before inducing ischemia. In both group, rats were sacrificed at the end of the reperfusion and
their bladders were removed. Biochemical assessments were performed using tissue
malondialdehyde (MDA) and TNF-α levels, histopathological assessments using
Hematoxilin&Eosin staining in light microscope, apoptosis assesment using TUNEL and bcl–
2 staining. Results: Tissue MDA and TNF-α levels were increased in I/R group, but were
significantly decreased in group treated with leptin (L+I/R) (p<0.01). Histopathological
comparison of the groups showed a decrease both in polymorphonuclear leukocytes and
congestion in L+I/R group. TUNEL-positive cell counts in urothelium, stroma and muscle
tissue were significantly increased in I/R group, but decreased in leptin group (p<0.01). With
immunohistochemical staining using Bcl–2, apoptotic cells were seen frequently in I/R
group, but less frequent in L+I/R group. Our findings showed that leptin had a protective
effect on I/R injury in rat bladder.
