Fetal ventrikülomegali (VM) ile ilgili çok sayıda çalışma olmasına rağmen yenidoğan dönemi sonuçları ile ilgili veriler kısıtlıdır. Bu çalışmanın amacı fetal VM tanılı olguların neonatal dönemdeki iyilik hallerini değerlendirmek, yoğun bakım, cerrahi ihtiyaçlarını ortaya koymak ve fetal VM şiddeti ile ilişkisini incelemektir. Retrospektif olarak yapılan bu çalışmada 20132018 yılları arasında fetal VM tanımlanan 87 olgudan canlı doğan, verilerine ulaşılabilen 39 yenidoğanın neonatal prognozları fetal VM’nin hafif yada ağır olmasına göre 2 gruba ayrılarak incelenmiştir. 39 yenidoğanda (22 kız-17 erkek) fetal VM ortalama 24+2 gestasyonel haftada saptanmış ve doğum 37+5 (29-41+4) haftada gerçekleşmiştir. Olguların %76’sı (30/39) hafif VM, %24’ü (9/39) ağır VM idi. Hafif ve ağır olgular arasından kord kan gazı ve Apgar skorları arasında fark yoktu. Hafif olguların 6’sında (%20), ağır olguların 3’ünde (%33) yoğun bakıma gerek duyulmuştu (p=0.023). Hafif olguların %63’ü (19/30), ağır olguların %22’si izole VM idi (p=0.030). V-P şant cerrahisi hafif olgularda gerekmezken, ağır olguların %33’ünde uygulandığı saptandı (p<0.001). Hastanede kalış süresi hafif olgularda 4.97±4.7 gün, ağır olgularda 7.33±6.65 gün idi. Uzun dönemde, hafif grupta 24 (%80), ağır grupta 7 (%77) olguda nörolojik izlemin sürdürüldüğü görüldü. Sonuç olarak fetal VM’nin hafif yada ağır olmasının mortalitede fark oluşturmadığını, ağır VM’de YYBÜ ve cerrahi ihtiyacının daha fazla, yatış süresinin daha uzun olduğunu, hafif olguların yaklaşık yarısında, ağır olguların %22’sinde VM’nin regrese olduğunu saptadık. Prenatal danışmanlık, gebeliğin yönetimi ve postnatal izlemin planlanması konusunda ventrikülomegalinin düzeyi, etyolojisi ve eşlik eden genetik, infeksiyöz, serebral yada ekstraserebral anomalilerin varlığı yol gösterici olabileceği gibi neonatal prognozun bilinmesi de yardımcı olacaktır.
Despite many studies on fetal ventriculomegaly (VM) data of neonatal period is restricted. The main goal of the current study is evaluating wellbeing of neonates during the neonatal period of diagnosed fetal VM case as well as propounding intensive care and surgery requirements and assessment of the link between neonatal period well-being during the neonatal period of diagnosed fetal VM case and fetal VM intensity. In this retro-perspective study 39 newborn prognosis with accessible data within the diagnosed fetal VM 87 cases from year 2013 to year 2018 have been examined in terms of classification into 2 groups of mild or severe fetal VM. 69.4% of 87 case is mild VM and 31.6% of case is severe VM. Fetal VM is determined in the average 22+2 (13+2-35+4) gestation week. The average gestation week of the examined 39 neonates is 24+2 week and the gestation week of the birth is 37+5 (29-41+4) week. 76% of cases is mild VM and 24% of cases is severe VM. Any difference between mild and severe case is not observed by the means of cord blood gas and Apgar scores. ICU requirement is found to be 20% for 6 mild cases and 33% for 3 severe cases (p=0.023). 63% of mild cases and 22% of severe cases is observed to be isolated VM (p=0.030). V-P shunt surgery is not required at mild cases meanwhile it is observed to be applied for 33% of severe cases (p<0.001). Duration of hospital stay is 4.97±4.7 days for mild cases while this duration is 7.33±6.65 days for severe cases. As a result of the study it is found that mild or severe fetal VM has no effect on the mortality, hospital stay duration. NICU and surgery requirements are higher for severe fetal VM. Additionally approximately half of the mild cases and 22% of the severe cases VM is determined to be regressed according to the results of the research. Knowing neonatal prognosis can be beneficial as well as ventriculomegaly level, etiology and accompanying genetics, infectious, cerebral or extra-cerebral anomaly existence can be guiding for prenatal supervision, gestation management and scheduling of the prenatal monitoring.
