Kolorektal kanser dünyada kanser orjinli ölümlerin en önemli
nedenlerinden biridir. Metastazların, kanser nedenli ölümlerin büyük bölümünden
sorumlu olmasına ragmen kolorektal kanser metastazların moleküler yapıları
henüz çok az çalısılmıstır. Bu çalısma primer tümörler ile metastazlardaki
genomik anomalileri belirlemek ve böylece metastatik yayılıma yatkınlıga neden
olabilecek genetik olayları ortaya koyabilecek primer tümörler ile metastatik
tümörler arasındaki genetik farklılıklar saptanabilecektir. Çalısmada, 25 primer
tümör, 4 karaciger ve bir over metastazı ile 8 lenf nodu tümörü olmak üzere
toplam 38 tümör örnegi yüksek rezolusyonlu-karsılastırmalı genomik
hibridizasyon (YR-KGH) teknigi ile analiz edilmistir.
Bulgular, genomik kopya aberasyon frekansının dokudaki hücresel
farklılasma azaldıkça arttıgını göstermistir. Kromozom 8q, 13q, 20q artısları ile
kromozom 8p, 17p ve 18q delesyonları sıklıkla gözlenen anomalilerdi. Kromozom
2p, 7p12, ve 7p21 artısları ilk olarak bu çalısmada primer tümörlerde gözlenen
anomalilerdi. Kromozom 4q, 10q, 14q ve 20p artısları sadece primer tümörlerde
gözlenen anomalilerdi. Diger taraftan genomik kopya aberasyon sıklıgının
karaciger metastazlarında daha yüksek oldugu saptandı. Kromozom 9q ve 12p
artısları sadece karaciger metastazlarında gözlenen anomaliler iken kromozom
10p artısı sadece lenf nodu tümörlerinde gözlendi.
Tümörlerde yeni aday kromozomal bölgelerin saptanması ve daha önce
rapor edilen anomalilerin çalısmamızda daha yüksek sıklıkta gözlenmesinin
kullanılan teknikten kaynaklandıgı düsüncesindeyiz. Bilgilerimize göre bu çalısma
kolorektal tümörler ile metastazların YR-KGH teknigi ile incelendigi ilk
çalısmadır. Çalısma bulguları, tümörün yayılımındaki farklılıgın metastatik
fenotipe ait özelliklerden sorumlu spesifik degisimlerin gözlendigi rastgele
olmayan kromozom anomalilerinin birikimden kaynaklanabilecegini gösterir
niteliktedir. Ancak çalısma populasyonunun genisletilmesi ve ileri analizlerde elde
edilen verilerin hastalık prognozu ile baglantısının kurulmasının klinikte büyük
faydalar saglayacagı düsüncesindeyiz.
Colorectal cancer is one of the most common causes of cancer-related
death in the world. Although metastasis is responsible for most cancer deaths,
molecular basis of colorectal cancer metastasis remains poorly studied. This study
was undertaken to identify genomic imbalances specific to primary tumors and
metastatic tissues and so assess genetic differences between primary and metastatic
tumors that might identify genetic events predisposing to metastatic dissemination. In
total, 38 tumor specimens, including 25 primary tumors, 4 liver an done over
metastasis and 8 lymph node tumors, were analyzed by the High Resolution-
Comparative Genomic Hybridization (HR-CGH) technique.
The results showed that the frequency of genomic copy aberrations were
increasing in the tumors with poor cellular differentiation. Overrepresentations were
detected most frequently at 8q, 13q,20q and deletions were prominent at 8p, 17p and
18q. Chromosome 2p, 7p12 and 7p21gains were newly diagnosed aberrations in the
primary tumors. Gains on chromosomes 4q, 10q, 14q ve 20p were only seen in the
primary tumors. On the other hand, the frequency of genomic copy aberrations were
higher in the liver metastasis than the primary tumors. The gains on 9q and 12p were
specific aberrations in liver metastasis whereas 10p gains were seen in only lymph
node tumors.
We concluded that detection of new putative chromosomal regions in the
tumors and higher frequencies in previously reported non-random genomic copy
aberrations resulted from the used technique, HR-CGH whose resolution ability (>3-
4 Mb) is higher than the resolution capacity of KGH (>5-10 Mb). Depending on our
knowledge, this is the first study for HR-CGH analysis in colorectal tumors and
metastasis. The analysis suggested that the different pathways of tumor dissemination
are reflected by a nanrandom accumulation of chromosomal alterations with specific
changes being responsible for the different characteristics of the metastatic
phenotype. However, further analysis in larger populations are necessary and
combinations of these evidences with disease prognosis might be very helpful in
clinics.
