Son, O. Tip1 Diabetes Mellituslu Hastalarda Karbohidrat Sayım Yönteminin
İrdelenmesi. Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Endokrinoloji ve
Metabolizma Bilim Dalı Tıpta Yandal Uzmanlık Tezi, Eskişehir, 2006. Diabetes
Control and Complications Trial (DCCT)’den çıkan sonuçlar Diabetes Mellitus
(DM)’un tedavisinde tıbbi beslenme tedavisi yaklaşımının önemini ortaya
koymuştur. DCCT’nin önerdiği tıbbi beslenme tedavilerinden biri de karbohidrat
sayım yöntemi olup, son yıllarda ilgi çekmeye başlamıştır. Karbohidrat (Kh) sayım
yönteminde; yiyeceklerdeki karbohidratların hastalara öğretilmesi ve yiyecek
alımında esneklik sağlanması ile günlük beslenme planının yapılması, sıkı kan
şekeri kontrolünü sağlayabilmek yanısıra yaşam kalitesine de olumlu katkılar
getirmektedir. Biz bu çalışmada Kh sayım yönteminin, Tip1 DM’lu hastalarda
tedavinin başarısına etkileri yanısıra yaşam kalitesi üzerine olan etkilerini irdelemeyi
amaçladık. Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Endokrinoloji ve
Metabolizma Bilim Dalı Diyabet Polikliniğine müracaat eden 37 Tip 1 DM’lu
hastanın 22’si Kh sayım yöntemi ile 15 olgu ise kontrol grubu olarak multipl doz
intensif insulin tedavisi ve standart diyabetik diyet uygulanarak 6 ay boyunca yakın
takibe alınmıştır. Karbohidrat sayım yöntemi uygulayan ve multipl doz intensif
insulin tedavisi ile birlikte standart diyabetik diyet uygulayan hastaların çalışma
başlangıcı ve 6 ay sonraki dönemde vücut ağırlığı, Vücut Kitle İndeksi, vücut
kompozisyonu Body Compositıon Analyzer (TBF-300M) cihazı kullanılarak, HbA1c
ve fruktozamin düzeyleri Indicates Roche / 911 Hitachi cihazı ve lipid parametreleri
ımmunometrik kemiluminesans yöntemi ile ölçülmüştür. Hastalara [ MEDİCAL
OUTCOMES STUDY 36-ITEM SHORT FORM (MOS SF- 36 ) ] 36 soruluk kısa bir
sağlık araştırması ve yaşam kalitesi ölçeği uygulanarak yaşam kaliteleri
değerlendirilmiştir. İstatiksel yöntem olarak; t- testi, ki-kare testi ve Mann-Whitney
U testi kullanılmıştır. Diyabetli bireylerde yeme fleksibilitesi sağlayan ve yaşam
kalitesini arttıran bu yöntemin başarısı diyabetli bireyin bu yöntemi ne kadar
algılayabildiğine bağlıdır. Çalışmamızda Kh sayan grupta, standart diyabetik diyet ve
multipl doz intensif insülin tedavisi uygulayan gruba göre glisemik kontrol
göstergelerinde anlamlı bir değişiklik saptanmamıştır. Karbohidrat sayan grubun
vücut ağırlığı ve vücut kompozisyonunda olumsuz bir bulgu saptanmamıştır.
Bununla birlikte Kh sayan grupta hipoglisemi sıklığı azalırken, yaşam kalitesi
belirgin artış göstermiştir. Sonuç olarak diyabetik bireylere esnek bir yeme planı
sunan Kh sayım yöntemi, işlerliği olan, hastayı hastalığı ve tedavisi konusunda daha
bilinçli ve aktif kılan bir yöntemdir.
Son, O. Investigation of Carbohydrate Counting Technique in Type1 Diabetes
Mellitus Patients. Eskişehir Osmangazi University Faculty of Medicine,
Medical Speciality Thesis in Department of Endocrinology and Metabolism,
Eskişehir, 2006. Results of DCCT study confirms the importance of medical
nutrition therapy in the treatment of DM. One of the nutrition therapy plans proposed
by DCCT is carbohydrate counting technique; which is receiving attention in recent
years. In the carbohydrate counting technique; which provides flexibility in the
feeding and improves the patients’ quality of life and tight blood glucose control;
daily feeding can be planned according to the ingredients of foods by the patients.
The aim of this study is investigating the effect of carbohydrate counting technique
on the efficacy of the treatment and on the quality of life in type1 DM patients. Of
37 type1 DM patients admitted to the Eskişehir Osmangazi University Faculty of
Medicine, Department of Endocrinology, Diabetes Clinic, 22 patients were followed
up for 6 month with carbohydrate counting technique and 15 patients were followed
as a control group with multiple dose intensive insülin therapy and standard diabetic
diet. At the beginning of the study and after six months weight, body mass index,
body composition was analyzed by using Body Composition Analyzer (TBF-300 M).
HbA1c and fruktozamin levels were counted by Indicates Roche/911 Hitachi and
lipid parameters by immunometric cemiluminescans technique in both carbohydrate
counting technique and standart diabetic diet- multiple dose intensive insülin therapy
groups. Quality of life of patients was evaluated by a medical research and life
quality measurement method containing 36 short questions [ Medical Outcomes
Study 36 Item Short Form (MOS –SF 36)]. Statistical analyses was made by using ttest,
ki-square test and Mann –Whitney U test. The success of the technique; which
provides eating flexibility and increases the quality of life, depends on the diabetic
patient’s skill in understanding the tecnique. In our study there was not a statistically
meaningful difference between the measures of glycemic control of carbohydrate
counting technique group and standart diabetic diet – multiple dose intensive insulin
therapy group; also there was no negative effects of this technique in the body weight
and in body composition of patients in carbohydrate counting technique group. On
the other hand, the frequency of hypoglycemia was decreased and there was an
improvement in the quality of life of the patients. As a result, carbohydrate counting
technique providing a flexible eating plan for the diabetic patients, is a current
therapy that not only makes the patient active, but also improves his awareness of the
disease and of the therapy.
