Chlamydia trachomatis cinsel yolla bulaşan
hastalıkların en sık bakteriyel nedenidir. Enfeksiyon sıklıkla asemptomatik geçer.
Ancak pelvik inflamatuar hastalık, infertilite, kronik pelvik ağrı ve ektopik gebelik
risklerini artırarak uzun dönemde kalıcı sekeller bırakır. Bu yüzden enfeksiyonun
erken tespit ve tedavisi için taramanın önemi büyüktür. Literatürde Chlamydia
infeksiyonunun preterm doğum, erken membran rüptürü ve spontan abortus gibi kötü
gebelik sonuçlarına neden olup olmadığı tartışmalıdır. Bizim çalışmamızda preterm
eylem ve/veya EMR tanısı olan 55 gebe, spontan abortusu olan 51 gebe ve miada
ulaşmış komplikasyonsuz gebeliği olan 51 hasta dahil edildi. Servikal sürüntüde PCR
ve DFA tekniği ile Chlamydia trachomatis taraması yapıldı. Gebelerde Chlamydia
trachomatis prevalansı PCR ile % 1.2, DFA tekniği ile de % 8.2 olarak bulundu.
Spontan abortusu olan hastalarda her iki teknikle de antijen saptanamadı. Preterm
eylemi olan gebelerde PCR ile %3.6, DFA ile %14.5 oranında Chlamydia
trachomatis saptandı. Ancak kontrol grubu olan miad gebeler ile karşılaştırıldığında
istatistiki olarak anlamlı bir farklılık oluşmadı. Pelvik muayenede servisit tespit
edilen 19 gebe hastanın PCR ile ikisinde, DFA ile dokuzunda Chlamydia trachomatis
antijeni saptandı. Servisiti olmayan hastalar ile karşılaştırıldığında sonuçlar istatistiki
olarak anlamlı idi.(p<0.005) Sonuç olarak Chlamydia varlığı ile preterm eylem, EMR
ve abortus gibi gebelik komplikasyonlarını ilişkilendirebilmek için daha fazla
çalışmaya ihtiyaç vardır.
Chlamydia trachomatis is the most
common bacterial cause of sexually transmitted diseases. The infection is often
asymptomatic. However, the risk of permanent sequelae such as pelvic inflammatory
disease, infertility, chronic pelvic pain and ectopic pregnancy increases in the long
term. For early detection and treatment of infection, women screening is very
important. In the literature, whether there is a relation between Chlamydia infection
and preterm birth, premature rupture of membranes and spontaneous abortion that
result in poor pregnancy outcomes is controversial. In our study, 55 pregnant women
with a preterm labor and / or premature ruptur of membrane, 51 pregnant women
diagnosed with spontaneous abortion and 51 patients with uncomplicated
pregnancies were included. Cervical smears were tested for Chlamydia trachomatis
using PCR and DFA technique. The prevalence of Chlamydia trachomatis in
pregnant women by using PCR was 1.2% and by using DFA was 8.2%. Besides in
patients with spontaneous abortion Chlamydia trachomatis antigen was detected in
both techniques carried out. Furthermore, the percent of Chlamydia trachomatis in
pregnant women with preterm labor was 3.6% using PCR and 14.5% using DFA .
However, when the results were compared with the control group of pregnant
women with full term pregancy there is a statistically significant difference
occurred. On the other hand, pelvic examination of 19 pregnant patients with
cervicitis revealed two pregnant women with Chlamydia trachomatis antigens which
are detected by PCR and nine chlamydia trachomatis antigens detected by DFA;
however, the results were statistically different when compared with patients without
cervicitis (P <0.005) .As a result, more studies are needed to find the association
between the presence of Chlamydia and pregnancy complications such as preterm
labor,premature rupture of membranes (PROM) and abortion.
