Bu çalışmada diyabetik ayak
enfeksiyonlu 40 hastanın sürüntü kültürü (SK) ile derin doku kültürü (DDK)
arasındaki ilişkiye bakılarak, patojenlerin antimikrobik duyarlılıkları incelendi. 2015-
2016 arasında, hastalardan lezyona uygun yöntemle sürüntü ve derin doku kültürü
alındı. Üreme olan örneklerde mikrobiyolojik tanımlama yapıldı. Biyopsiden sonra
başlanan ampirik antibiyoterapi mikrobiyolojik sonuçlara göre değerlendirildi ve
gerekli olgularda tedavi değişikliği yapıldı. Çalışmanın istatistiksel analizi IBM SPSS
Statistics (V22.0) programı kullanılarak yapıldı. Çalışmadaki hastaların yaş ortalaması
58,9 yıl, ortalama diyabet süresi 16,7 yıl, ortalama kan şekeri 278,5 mg/dl olarak
bulundu. Lezyonları topuk ve parmaklarda yoğunlaşan hastaların %35’ine ampütasyon
yapıldı. Wagner evre 2 ve üzeri çalışmaya alınan olguların %22,5’i evre 2, %57,5’i
evre 3, %15’i evre 4, %5’i evre 5’ti. 25 (%62,5) hastada SK ile DDK’ünde aynı
sonuçlar raporlandı. 26 (%65) hastanın SK, DDK’ündeki patojenleri içeriyordu.
Monomikrobiyal üreme oranları aynı idi. Hem SK hem DDK’lerinde polimikrobiyal
üreme oranı %12,5, patojen bakteri üremesi olan hasta başına düşen mikroorganizma
sayısı 1,2 ve en çok izole edilen patojen S. aureus idi. DDK’ündeki stafilokoklar
metisiline, E. faecalis suşları ampisiline ve tüm Gram (+) koklar vankomisine
duyarlıydı. Enterobacteriaceae suşlarının hepsi meropeneme duyarlıydı. A. baumannii
suşları amikasin ve imipeneme dirençli, kolistine duyarlıydı. P. aeruginosa suşlarının
ise %40’ı meropeneme duyarlıydı. Sonuç olarak; uygun debritman yapıldıktan sonra
alınan SK sonuçlarıyla tedavi başlanabileceği, tedaviye yanıtsız hastalarda DDK
alınmasının ve ampirik antimikrobiyal tedavinin hastaların klinik ve tıbbi öyküsünün
dikkate alınarak başlanılmasının uygun olduğu düşünüldü.
In this study;
the relation between deep tissue culture (DTC) and superficial swab culture (SSC) in
40 patients with diabetic foot infections were analysed and the antimicrobial
susceptibility status of the pathogens were evaluated. The cultures of deep tissue
biopsy specimens and swab samples were collected simultaneously from all patients
between 2016 and 2017. Positive cultures were identified microbiologically. Empirical
antibiotic that is started after procedure was reevaluated according to microbiological
results and treatmeant change was considered when necessary. Statistical analysis was
performed with IBM SPSS Statistics V22.0. In this study, mean age was 58.9 years.
Mean duration of diabetes was 16.7 years and mean blood glucose was 278.5 mg/dl.
Patients’ lesions were mostly on the heel and toes. 35% of the patients were undergone
amputation. Wagner grade ≥2 wounds were included to the study. 22.5% of these
wounds were grade 2 while 57.5% was grade 3 and 15% was grade 4. The percentage
of grade 5 wounds were 5%. The same microbiological results were reported in 25
(62.5%) patients’ SSC and DTC. Superficial swab culture of 26 (65%) patients was
including the pathogens in DTC. Monomicrobial infection rates were the same.
Polymicrobial infection rate in both SSC and DTC was 12.5% and S. aureus was the
most commonly isolated pathogen. The average number of isolates per culture positive
sample was 1.2. Staphylococci were sensitive to methicillin, E. faecalis species were
sensitive to ampicillin, all Gram-positive cocci were sensitive to vancomycin in the
DTC. Meropenem was efficient against Enterobacteriaceae family. A. baumannii
species were sensitive to colistin, but resistant to amikacin and imipenem. 40% of P.
aeruginosa species were sensitive to meropenem. In conclusion, our data shows that
treatment could be started according to the result of superficial swab culture that is
taken after debridement surgery. If the patient is unresponsive to treatment, DTC
should be taken. Empirical antimicrobial therapy should be started considering
patients’ medical history and clinical examination.
