Çalışmamızın amacı, preoperatif dönemde ve neoadjuvan tedavi öncesi meme CA tanısı almış ve klinik tarafından uygun görülerek bölümümüzde F-18 FDG PET/BT görüntülemesi yapılan hastalarda histopatolojik ve immunohistokimyasal prognostik faktörler ile metabolik parametreler arasındaki ilişkiyi araştırmaktır. Serimiz, 94 kadın hastadan oluşmakta idi. Tümörün SUVmax, SUVmean, MTV, TLG ve SUVmax/kc SUVmean değerlerinin tümü ile T evresi, M evresi ve grade’i arasında anlamlı ilişki saptanırken histopatolojik tipler ve menapoz durumu ile arasında anlamlı ilişki saptanmadı. Lenf nodu metastaz durumu ile tümörün MTV, TLG ve SUVmax/kc SUVmean değerleri arasında anlamlı fark bulunurken diğer metabolik parametreler ile ilişki anlamsız düzeyde idi. Tümörün ER, HER-2 reseptör durumları, Kİ-67 düzeyleri ve moleküler alt tipleri ile MTV dışındaki metabolik parametreler arasında anlamlı ilişki saptandı. PR durumu ile MTV ve TLG değerleri dışındaki parametreler ile anlamlı ilişki saptandı. Tümör proliferasyonun göstergesi olan Ki-67 değeri ile en korele metabolik parametre tümörün SUVmax değeri bulundu. Meme kanseri moleküler sınıflama ile 5 alt gruba ayrıldı. MTV dışındaki metabolik parametrelerin anlamlı olarak Luminal A tipte en düşük, üçlü negatif grupta en yüksek değerlerde olduğu görüldü. Sonuç olarak çalışmamızda, tümörün SUVmax değerinin prognostik faktörler ile ilişkisinin diğer metabolik parametrelerden daha iyi düzeyde olduğu bulundu. Ancak tümörün hem hacimsel hem de glikolitik aktivitesini birlikte yansıtan TLG değerinin, aksiller lenf nodu tutulumunu ve uzak metastaz durumunu ön görmesi, tümör biyolojisi ve davranışını yansıtabileceğini düşündürmektedir.
The aim of our
study was to investigate the relationship between histopathological and
immunohistochemical prognostic factors and metabolic parameters in patients
undergoing F-18 FDG PET / CT imaging for preoperative and neoadjuvant pretreatment
breast cancer diagnosis and appropriate for clinical use. Our series
consisted of 94 female patients. There was a significant relationship between T-stage,
M-stage and grade of tumor with SUVmax, SUVmean, MTV, TLG and SUVmax /
kc SUVmean values in the tumor, but no significant correlation was found between
histopatholagic types and menopausal status. There was a significant difference
between lymph node metastasis status and tumor’s MTV, TLG and SUVmax / kc
SUVmean values, but the relation with other metabolic parameters was insignificant.
Significant correlations were found between tumor ER, HER-2 receptor status, Ki-67
levels, and molecular subtypes and metabolic parameters other than MTV.
Significant correlations were found between PR status and metabolic parameters
other than MTV and TLG values. The Ki-67 value, which is the indicator of tumor
proliferation, was the SUVmax value of the most correlated metabolic parameter’s of
tumor. In conclusion, our study showed that SUVmax value in relation to prognostic
factors of tumor was better than other metabolic parameters. However, the
association of TLG, which reflects both volumetric and glycolytic activity of the
tumor, is significant in relation to axillary lymph node involvement, meaning that it
is meaningless with other metabolic parameters and that the significance value in the
case of distant metastasis is better than other metabolic parameters suggests that it
may reflect tumor biology and behavior.
