Bu çalışmada bir üniversite hastanesinin bazı polikliniklerine başvuran kişilerin aile hekimliği
sisteminden memnuniyeti ve sevk sistemi ile ilgili düşüncelerinin belirlenmesi amaçlanmıştır.
Kesitsel olan bu araştırma Gazi Üniversitesi Sağlık Araştırma ve Uygulama Merkezi Gazi
Hastanesi’nin bazı polikliniklerine başvuran 18 yaş üstü kişiler ve 18 yaş altındaki çocukların
ebeveynlerinin katılımıyla yürütülmüştür. Anket katılımcılara yüzyüze görüşme şeklinde
uygulanmıştır. İstatistiksel anlamlılık değeri p<0.05 olarak kabul edilmiştir. Araştırmaya 1302
kişi katılmıştır. Katılımcıların yaş ortalaması 36,7±13,1’dir. Katılımcıların %72,7’si kadın,
%79,3’ü evli, %33,6’sı lise mezunu, %39,5’i yüksekokul/üniversite mezunudur. Katılımcıların
%35,9’u çalışan, %32,1’i ev hanımıdır. Katılımcıların %41,3’ü aile hekimine başvurmadan
doğrudan diğer sağlık kuruluşlarına başvurduğunda katkı payı ödediğini bilmemektedir. Katkı
payı ödediğini bilmeyenlerden %60,0’ı bu durumun bundan sonra kendisini ilk olarak aile
hekimine başvurmaya yönelteceğini belirtmiştir. Mevcut aile hekimliği sisteminden memnun
olanlar tüm katılımcıların %54,1’idir. Memnun olmayanların en çok belirttikleri sebep %78,4
ile ASM’lerin şartlarını yetersiz bulmasıdır. Mevcut aile hekimliği sisteminden memnun
olanların en çok belirttikleri sebepler ise sırasıyla; %58,7 ile sıra beklenmemesi, %45,8 ile
ASM’lere ulaşımın kolay olmasıdır. Katılımcıların %60,1’i sevk sisteminin uygulanmasını
istememektedir. Sevk sisteminin uygulanmasını istemeyenlerin en çok belirttikleri sebepler
sırasıyla; %54,7 ile hekim seçme özgürlüğünün devam etmesi için, %40,4 ile sağlık sorunlarına
çözüm bulunmasından yetersizlik olacağı için, %39,3 ile hastaneler daha kaliteli hizmet verdiği
için şeklinde olmuştur. Aile hekimlerine duyulan güvensizlik ve Aile Sağlığı Merkezlerinin
katılımcılar tarafından yetersiz görülmesi memnuniyeti etkilemektedir. Aile hekimlerine
duyulan güveni arttıracak çalışmalar ve Aile Sağlığı Merkezlerine verilen teknik desteğin
arttırılması yerinde olacaktır.
In this study, it was aimed to determine the satisfaction of the family medicine system people
who applied to some polyclinics of a university hospital and their thoughts about the referral
system. This cross-sectional study was conducted with the attendance of the parents of children
under 18 and children under 18 years of age. This cross-sectional study was conducted with the
participation of over the age of 18 and parents of children under 18 years of age. This study
was conducted in a university hospital in Ankara. The questionnaire was applied to the
participants in the form of face-to-face interviews. Statistical significance was accepted as p
<0.05. 1302 people participated in the research. The mean age of the participants was 36.7 ±
13.1. 72.7% of the participants were women, 79.3% were married, 33.6% were high school
graduates and 39.5% were university graduates.. 35.9% of the participants were working and
32.1% were housewives. 41.3% of the participants do not know that they pay the contribution
directly to other health institutions without applying to the family physician. 54.1% of all
participants were satisfied with the current family medicine system. The most stated reason of
unsatisfied is that they find the conditions of Family Medicine Center’s insufficient with 78.4%.
The reasons stated by those who are satisfied with the current family medicine system are;
58.7% of the queue, 45.8% is easy to access to FMC’s. 60.1% of participants do not want the
referral system to be applied. The reasons most indicated by those who do not want to apply
the referral system, respectively; 54.7% to continue the freedom of physician choice, 40.4%
because of the inability to find solutions to health problems, with 39.3%, hospitals are in the
form of providing better quality services. The insecurity of family physicians and the
inadequacy of the Family Health Centers by the participants affect the satisfaction. Increasing
confidence in family physicians and increasing the technical support to Family Health Centers
will be appropriate.
