Alerji deri testi alerjik duyarlılığı saptamak için kullanılan standart
bir yöntemdir. Solunum alerjenleri coğrafi bölge, iklim, yerleşim özelliklerine göre
değişkenlik göstermektedir. Bu alerjenlerden mantarlar, evlerde özellikle nemli-ıslak
yerlerde, banyo ve mutfaklarda, iyi havalanmayan karanlık köşelerde, havalandırma
sistemlerinde, saksı diplerinde, bozulmuş sebze ve meyvelerde görülmektedir. Çalışmamızda
01/01/2010 – 31/12/2014 tarihlerinde Çocuk Alerji ve İmmünoloji Polikliniği’ne
başvuran hastalardan 616 tanesine prick deri testi uygulanmıştır. Bu hastalardan
inhaler mantar alerjisi pozitif çıkan 40 hasta ve prick testi tamamen negatif
olan 60 hasta kontrol grubu olacak şekilde toplamda 100 hasta çalışmamıza dahil
edilmiştir. Test yapılan hastalardan 40 tanesinde (%6) inhaler küf mantarı alerjisi pozitif
olarak bulunmuştur. Küf mantarları içinde en yaygın olanı %62.5 ile A. Alternaria’dır.
2. Sırada %32.5’lik oranda Cladosporium yer almaktadır. Küf mantarı pozitifliği
olan hastaların, %85’inde astım, %87.5’inde alerjik rinit semptom varlığı,
%25’inde atopik dermatit, %20’sinde gıda allerjen duyarlılığı, %45’inde soğuk algınlığı
olmadan wheezing görüldüğü tespit edildi. Test pozitifliği olan hastalardan
%78.3’ü apartman dairesinde, %22.7’si müstakil evde yaşamakta ve %70.2’si kaloriferle,
%27’si soba, %2.8’i de diğer ısınma yöntemleriyle ısınmakta idi. Küf mantarı
pozitif hasta grubunun %29.7’sinin evinde gözle görülür şekilde küf olduğunu,
%21.6’sının evde evcil hayvan beslediğini, %54’ünün de evinde sigara içildiğini tespit
ettik. Sonuç olarak inhaler küf mantarı alerjisinin birçok alerjik şikayetle ilişkili
olduğu görülmüştür.
The skin
prick test is the standard method used to detect allergic sensitivity. Respiratory allergens
vary by geographical region, climate and characteristics of settlement. Mold,
one of these allergens, grows indoors particularly inmoist and wet surfaces, bathrooms
and kitchens, dark areas not ventilated appropriately, under flower pots, and in
rotten fruits and vegetables. For the purpose of this study, the skin prick test was administered
to 616 patients that presented to Pediatric Diseases, Allergy and Immunology
Outpatient Clinic of Osmangazi University Hospital between January 1, 2010
and December 31, 2014. The study was conducted with a total of 100 patients, i.e. 40
patients that developed positive reaction to mold and – as a control group – 60 patients
that developed totally negative reaction to mold. In the group, 40 patients (6%)
were allergic to inhaled mold. The study shows that the most common type of mold
was A. Alternariawith the rate of 62.5%, which is followed by
Cladosporium(32.5%). Symptoms seen in patients with positive reaction to inhaled
mold include asthma (85%), allergic rhinitis (87.5%), atopic dermatitis (25%), sensitivity
to food allergens (20%), and wheezing when common cold symptoms are not
present (45%). In the group, 78.3% of patients were living in apartments and 22.7%
were living in detached houses. The heating methods used in patients’ dwelling werecentral
heating (70.2%), heating stoves (27%), and other methods (2.8%). The results
show that there was mold visible to the naked eye in the home of 29.7% of patients,
there was a pet in the home of 21.6% of patients, and smoking was allowed in
the home of 54% of patients. As a consequence, inhaled mold allergy is associated
with several allergic complaints.
