Bu çalışmada, Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Psikiyatri Polikliniğinde takip edilen ötimik dönemdeki İkiuçlu Duygudurum Bozukluğu (İUDB) tanılı hastalar ile daha önce mani/ hipomani öyküsü olmayan ve antidepresan kullanımına bağlı manik/hipomanik kayma (h/MK) yaşayan hastaların klinik ve sosyodemografik özellikler açısından karşılaştırılması, h/MK grubu hastalarının özelliklerinin belirlenmesi ile sınıflandırma sistemlerine katkıda bulunulması ve tedavi özellikleri konusunda farkındalığın arttırılması amaçlanmıştır. Çalışmaya 01.03.2015 ile 30.08.2016 tarihleri arasında başvuran ötimik dönemde IUDB tanılı hastalar ile h/MK yaşayan 120 hasta alındı. Hastalar İUDB grubunu oluşturan 80 ve h/MK grubunu oluşturan 40 hasta olarak iki gruba ayrıldı. Çalışmaya alınanlar ile DSM-IV için yapılandırılmış klinik görüşme (SCID-I) ile görüşmeler yapıldı. Sosyodemografik verilerin kaydedilmesi, klinik durumun saptanması ve işlevselliğin değerlendirilmesi amacıyla Sosyodemografik veri formu, Young Mani Derecelendirme Ölçeği (YMDÖ), Hamilton Depresyon Derecelendirme Ölçeği (HDDÖ), İşlevselliğin Genel Değerlendirilmesi Ölçeği (İGDÖ), Klinik Global İzlenim Ölçeği (CGİÖ) ve Bipolar Bozukluk İşlevsellik Ölçeği (BB-İÖ) uygulandı. Çalışmamızda İUDB grubu ile h/MK grubu arasında sosyodemografik veriler açısından fark olmadığı, h/MK grubunun daha sık depresif atak yaşadığı, manik/hipomanik atak sıklığının her iki grup arasında eşit olduğu ve gruplar arasında işlevsellik açısından fark olmadığı saptandı. Depresif şikâyetlerle gelen ve özellikle ilk depresif dönemi olan hastaların öyküsü özellikle hipomanik dönemler açısından ayrıntılı değerlendirilmeli, Hastaların İUDB açısından risk grubunda olabileceğini gösteren klinik ve sosyodemografik özelliklerine dikkat edilmeli, tedavi planı bu bilgiler ışığında yapılmalıdır.
The present study aimed to compare the clinical and sociodemographic characteristics of the patients in the euthymic period with bipolar disorder (BD) who were follow up patients at Eskişehir Osmangazi University Faculty of Medicine Psychiatry Clinic, and the patients in the euthymic period with manic/hypomanic switch associated with antidepressant (h/MS) who had no previous medical history of mania/hypomania, to make contribution to classification systems by specifying the characteristics of h/MS group of patients, and to increase the awareness about treatment features. 120 patients in the euthymic period with BD diagnosis and experience h/MS that consulted between 01.03.2015 and 30.08.2016 were included. The patients were divided into two groups as 80 patients constituting BD group and the 40 patients constituting h/MS group. Participants were interviewed with a structured clinical interview (SCID-I) for DSM-IV. In order to record sociodemographic data, to determine the clinical condition and to evaluate the functionality; Sociodemographic Data Form, Young Mania Rating Scale (YMRS), the Hamilton Depression Rating Scale (HDRS), Global Assessment of Functioning Scale (GAFS), and Bipolar Disorder Functioning Scale (BDFS) were applied. In the present study it was found that there was no difference between the BD group and h/MS group in terms of sociodemographic data, h/MS group experienced depressive episodes more often, the frequency of manic/hypomanic episodes was the same for both groups, and there was no difference in terms of functionality between the groups. Clinical history of patients with depressive complaints, especially those with the first depressive episode, should be evaluated in detail in terms of hypomanic episodes. Attention should be paid to clinical and sociodemographic features that indicate that patients may be at risk for BD. Treatment plan should be arranged in accordance with that information.
