Frontotemporal lobar dejenerasyon (FTLD); klinik, patolojik ve genetik
olarak heterojen progresif beyin hastalıkları grubunu tanımlamaktadır. Familyal
ALS (Amyotrofik lateral skleroz) ve FTD (Frontotemporal Demans)’nin major
sebebi olarak C9orf72 geninin kodlamayan bölgesindeki patojenik >30
heksanükleotid (GGGGCC) tekrar artışları tanımlanmıştır.
Çalışmamızda; FTLD spektrumunda C9orf72 GGGGCC tekrar artışlarının
Türk olgularda frekansının belirlenmesi ve bu tekrar artışlarının fenotip üzerindeki
etkilerinin saptanması amaçlanmıştır. FTLD spektrumundan tanı almış ve MAPT,
PGRN, CHMP2B, VCP, TARDBP, FUS genlerinde herhangi patojenik bir mutasyon
saptanmamış 100 olgu ile nörolojik/psikiyatrik olarak patolojik bir bulgusu
olmayan yaşla uyumlu 100 kontrol bireyinde bu heksanükleotid tekrar sayıları RPPCR
(repeat primed PCR) ve sizing-PCR yöntemleri kullanılarak belirlenmiştir.
Olgularımızın %67’si sporadik, %33’ü ailesel olarak sınıflanmış ve sporadik
olguların hiçbirinde patojenik tekrar artışı saptanmamıştır. Ancak ailesel
olgulardan bir olguda (1/33) patojenik aralıkta (>30) tekrar saptanmıştır. Olgu ve
kontroller arasında tekrar sayıları istatistiksel olarak anlamlı bulunmuştur.
Olgularımızın %4’ünde orta derece (20-30) tekrar saptanmıştır. Psikotik belirtilere
sahip hastalarda orta/patojenik tekrarların baskınlığı görülmüştür.
Çalışmamız Türkiye FTLD spektrumu için C9orf72 GGGGCC tekrar
artışlarının frekansının ve klinik üzerindeki etkisinin değerlendirilmesine yönelik
bilgimiz dahilindeki ilk çalışmadır. Çalışmamız sonucunda patojenik tekrar
artışlarının Türkiye FTLD olgularında çok yaygın görülmediği ancak orta derece
tekrarların FTLD için artmış bir risk faktörü olabileceği ya da bir modifiye edici gen
olarak görev yapabileceği düşünülmektedir. Bunun yanı sıra psikotik
semptomlarla bu orta/patojenik tekrar artışlarının korelasyonunun prognostik
açıdan önemli olabileceği düşüncesindeyiz. FTLD tanılı bir olguda sizing-PCR
yöntemiyle belirlemeyediğimiz patojenik tekrar sayısı, RP-PCR yöntemi
kullanılarak tespit edilmiştir. Bu nedenle iki yöntemin birlikte kullanılması tekrar
sayısının belirlenmesinde daha etkindir.
Sonuç olarak, Türk FTLD hastalarında C9orf72 GGGGCC ekspansiyon
frekansı için yeni veriler elde edilmiştir. Ancak, C9orf72 ekspansiyon frekansının
kesin olarak saptanması için verilerimiz Türk popülasyonundaki farklı etnik grup
FTLD hastalarında daha ileri çalışmalarla desteklenmesi gerekmektedir. Orta
derece tekrar içeren bireylerde yapılacak metilasyon, ekspresyon ve postmortem
çalışmalar C9orf72 geninde bu tekrarların patojenik etkisi hakkında bilgi verici
olacaktır. Bunun yanı sıra bu artışlar etrafındaki indellerin belirlenmesi ve ortak
PCR yöntemlerinin kullanılması C9orf72 popülasyon frekansının gerçek oranını
belirlemede daha etkili olacaktır.
Frontotemporal lobar degeneration (FTLD); describes a group of
progressive brain disorders that are clinically, pathologically, and genetically
heterogeneous. Pathogenic >30 repeats of a noncoding GGGGCC hexanucleotide
repeats in the C9orf72 gene have been defined as a major cause of both familial
FTLD and amyotrophic lateral sclerosis (ALS).
In our study; it was aimed to determine the prevalence of C9orf72 GGGGCC
repeat expansion in the Turkish cases with FTLD spectrum and to determine the
effects on the phenotype. The repeats in C9orf72 gene were analysed by the
repeat- primed PCR (RP-PCR) and sizing-PCR techniques in the FTLD-diagnosed
cases who had no mutations for FTLD-related genes (MAPT, PGRN, CHMP2B, VCP,
TARDBP, FUS) that had been previously analysed by the NGS technique. For
control group same age and healthy individuals without pathologic
neurological/psychiatric findings were analysed by the same techniques.
Of our cases, 67% were sporadic, 33% were classified as familial. No
pathogenic expansion was detected in any of the sporadic cases. However, the
pathogenic range expansion (>30) was found in one of the familial cases (1/33).
The allele length difference between the cases and controls was statistically
significant. An intermediate (20-30) repeat was detected in 4% of our cases. The
dominancy of intermediate/pathogenic repeats was seen in the cases with
psychotic symptoms.
This study is the first examination of C9orf72 GGGGCC repeat expansions
in the Turkish cases with FTLD spectrum. As a result of our study, it is thought
that C9orf72 pathogenic repeat expansion is not common in Turkish FTLD cases,
but intermediate repeat may be an increased risk factor for FTLD or may act as a
modifying gene. In addition, we believe that the correlation of these
intermediate/pathogenic repeats with psychotic symptoms may prognostically
important. In one case with FTLD, pathogenic repeats were not identified by
sizing-PCR whereas the pathogenic expansion was revealed by the RP-PCR.
Therefore, the combination of the both methods is more effective in determining
hexanucleotide expansions.
In conclusion, we presented novel data of the frequency of C9orf72
GGGGCC expansions in Turkish FTLD patients. However, our study data should be
supported by further studies in different ethnic groups for detection of C9orf72
expansion frequency in FTLD patients. Methylation, expression, and postmortem
studies should be conducted in cases who have intermediate repeat to gain insight
into the possible pathogenic effect for C9orf72 gene. Identification of the indels
around GGGGCC repeats and revealing the repeat size by same techniques would
be more effective in determining the true ratio of C9orf72 population frequency.
