Amaç: Havacılıkta kaza riski oluşturan akut hipobarik hipoksinin bilişsel
ve psikomotor fonksiyonlar üzerine olumsuz etkileri bulunmaktadır. Bu
nedenle akut hipoksi durumlarında pilotun zihinsel fonksiyonlarını
koruyabilmesi için hipoksi dayanıklılığını geliştirmesi önemlidir. Bu çalışmanın
amacı aerobik ve anaerobik kapasitelerin hipoksik şartlarda bilişsel ve
psikomotor işlevlere etkilerini incelemek ve bu sayede hangi tür egzersizin
hipoksi dayanıklılığına olumlu etki edebileceğini anlamaktır.
Yöntem: Çalışmaya uçuş fizyolojisi eğitimi almak amacıyla başvuran 25
pilot adayı alınmıştır. Katılımcılara rutin idari sağlık muayenelerinin ardından
fiziksel kondisyon testleri olan aerobik kapasite ölçümü (VO2maks) ve
anaerobik kapasite ölçüm testi olan Wingate testi uygulanmıştır. Hipoksi
uygulaması 5486 metre (18.000 ft) yüksekliğe eşdeğer basınçta yapılmıştır.
Nöropsikolojik testler olarak belirlenen Go/NoGo ve Digit Span testleri, eğitim
öncesi, hipokside ve eğitim sonrası olmak üzere üç aşamada uygulanmıştır.
Katılımcılar aerobik ve anaerobik kapasite ortalamalarına göre yüksek ve
düşük olarak ikişer gruba (dört grup) ayrılıp değerlendirilmişlerdir.
Bulgular: Katılımcıların Go/NoGo R-Go tepki süresi ve ihmal hatası
ortalamaları eğitim öncesi döneme göre hipokside ve eğitim sonrasında anlamlı
artış göstermiştir. Digit Span testinde ise hipoksideki bellek genişliği ve doğru
yanıt sayısında eğitim öncesi ve eğitim sonrasına göre anlamlı düşme tespit
edilmiştir. Bu bulgular aerobik kapasite açısından incelendiğinde hem Go/NoGo
hem Digit Span testlerindeki olumsuz etkilenmeler aerobik kapasitesi düşük
gruptakilerde aerobik kapasitesi yüksek gruptakilere göre daha belirgin
bulunmuştur. Anaerobik kapasite açısından yapılan incelemede ise anaerobik
kapasitesi yüksek grubun Go/NoGo verileri eğitim sonrasında anaerobik
kapasitesi düşük gruba göre daha başarılı fakat hipokside görülen bozulma
bakımından anaerobik yüksek ve anaerobik düşük gruptakilerde belirgin bir
fark saptanmamıştır. Digit Span testinde de yine hipoksideki başarısızlıkta
anaerobik yüksek ile anaerobik düşük gruplar arasında belirgin fark
gözlenmemiştir. Fakat eğitim sonrası dönemde anaerobik yüksek gruptakilerin
bellek skorları ve doğru yanıt sayıları anaerobik düşük gruptakilere göre daha
iyi çıkmıştır.
Sonuç: Akut hipoksinin bilişsel işlevleri olumsuz yönde etkilediğini
gösteren önceki çalışmalarla uyumlu olarak hipokside ve eğitim sonrasında
hem bilişsel hem de psikomotor işlevlerin olumsuz etkilenebileceği sonucuna
varılmıştır. Özellikle aerobik kapasitenin yüksek olmasının hem bilişsel hem de
psikomotor işlevlerin hipokside ve eğitim sonrasındaki bozulmaya karşı olumlu
yönde koruyucu etkisi vardır. Anaerobik kapasitenin derecesinin akut
hipoksideki bilişsel ve psikomotor bozulmayla doğrudan bir ilişkisi gözlenmemiştir. Fakat satürasyon düşüşlerinin ardıl etkisinden kaynaklanmış
olabileceği değerlendirilen eğitim sonrası bilişsel ve psikomotor bozulmaya
karşı olumlu yönde etkisinin olduğu tespit edilmiştir. Uçuculara aerobik ağırlıklı
ve anaerobik egzersizler de içeren düzenli spor alışkanlığı kazandırmanın
onların akut hipoksiye karşı dayanıklılıklarını arttırabilecek bir etken olarak
önerilmesi uygun görülmüştür.
Objective: Acute hypobaric hypoxia, which possesses an accident risk in
aviation, has negative effects on cognitive and psychomotor functions.
Therefore, it is important to develop hypoxia tolerance to preserve pilot’s
mental functions during possible hypoxic conditions. We intended to
investigate the roles of aerobic and anaerobic capacities on cognitive and
psychomotor functions during hypoxic conditions.
Methods: 25 pilot candidates, applied for training in flight physiology
were included in this study. After routine health examinations, physical fitness
tests of aerobic and anaerobic capacity measurements were applied to the
participants. Hypoxia was applied at equivalent pressure of 5486 meters
(18.000 ft) in height. Go/NoGo and Digit Span neuropsychological tests were
applied during pre-, post-training and hypoxia. Depending on means of aerobic
and anaerobic capacities, the participants were grouped as high and low for
each capacity.
Results: When compared to the pre-training phase, the means of
response times and negligence errors of GNG tests increased significantly
during hypoxia and post-training. For Digit Span test, the means of memory
span and the number of correct answers decreased significantly in hypoxia
when compared to pre- and post-training phases. When these findings were
examined in terms of aerobic capacity, negative effects in both
neuropsychological tests were more prominent in the low aerobic group than
the high aerobic group. In terms of anaerobic capacity, Go/NoGo scores of
high anaerobic group were better than low anaerobic group in the post-training
phase, but there was little hypoxic deterioration between high and low
anaerobic groups. According to Digit Span scores, there were also no hypoxic
differences between high and low anaerobic groups, but the means of memory
span and number of correct answers of high anaerobic group were better than
low anaerobic group in the post-training phase.
Conclusion: We found that both cognitive and psychomotor functions
could be adversely influenced during hypoxia and post-training phases. We
showed that if aerobic capacity is high, it might be a positive factor against
deterioration of both cognitive and psychomotor functions during hypoxia and
post-training phases. There was no direct relationship between the amount of
anaerobic capacity and neuropsychological impairments during hypoxia phase.
However, it has positive effects on cognitive and psychomotor impairments
during post-training phase considered due to post effects of consecutive desaturations. We suggest regular sport habits mostly aerobic, but including
anaerobic exercises for aircrew as a factor that may increase their acute
hypoxia tolerance.
