Amacımız, ev egzersiz programına ek, devamlı ve kesikli piroksikam fonoforezi uygulamasının diz osteoartritli hastalardaki klinik parametrelere ve yaşam kalitesine etkisini değerlendirmekti. 1. Gruptaki 20 hastaya 1 MHz frekans ve 1.5 W/cm² yoğunlukta 1:4 görev siklusunda, 2. Gruptaki 20 hastaya, 1 MHz frekans ve 1.5 W/cm² yoğunlukta % 0,5 piroksikam fonoforezi uygulandı. Her üç gruba da ev egzersiz programı verildi. Hastalar tedavi öncesi (TÖ), tedavi sonrası (TS) ve TS 1. ayda The Western Ontario and Mcmaster Universities Osteoarthritis Index (WOMAC), Vizuel analog skala (VAS), Kısa Form- 36 (SF-36) ve 20 metre yürüme süresi ile değerlendirildi. Devamlı fonoforez grubunda, ek olarak WOMAC fiziksel fonksiyon, 20 metre yürüme süresi ve SF-36 fiziksel fonksiyon, fiziksel rol kısıtlılığı, sosyal fonksiyon parametrelerinde TS ve TS 1. Ayda iyileşme tespit edildi. TS ve TS 1. ay VAS fleksiyon ve hareket, SF-36 fiziksel rol kısıtlılığı parametrelerinde devamlı fonoforez grubunda egzersiz grubuna göre ve tedavi sonrası VAS hareket parametresinde, devamlı fonoforez grubunda, kesikli fonoforez grubuna göre iyileşme bulundu. Sonuçta, tüm uygulanan tedavilerin semptomatik primer diz osteoartriti tedavisinde etkili olduğunu saptadık. Ancak devamlı fonoforez tedavisinin ağrı parametrelerinde kısmı bir üstünlüğü olduğu gösterildi.
Our aim was to evaluate the effectiveness of continuous and pulsed piroxicam phonophoresis with the home exercise program on clinical parameters and quality of life in patients with knee osteoarthritis. Twenty patients in group 1 were applied to 0.5 % piroxicam phonophoresis at 1 MHz frequency, 1: 4 duty cycle and 1.5 W / cm² intensity. Twenty patients in group 2 were applied to 0.5 % piroxicam phonophoresis at 1 MHz frequency and 1.5 W / cm² intensity. All of the three groups were received the home exercise program. All patients were evaluated before the treatment (BT), after the treatment (AT) and 1 month AT by The Western Ontario and Mcmaster Universities Osteoarthritis Index (WOMAC), Visual Analog Scale (VAS), Short Form- 36 (SF- 36) and 20-meter walking time. WOMAC pain, WOMAC total scores, all VAS scores and SF-36 pain parameters were improved in all groups AT and this improvement continued at 1 month AT. In addition to above parameters, the pulsed phonophoresis group was also found to have improvements in all SF-36 parameters except for emotional role limitation, WOMAC stiffness and physical function and the continuous phonophoresis group was also detected to have improvements in WOMAC physical function, 20-m walking time and SF-36 physical function, physical role limitation, social functioning parameters at AT and 1 month AT. The continuous phonophoresis group was found to have improvements in SF-36 physical role limitation, VAS flexion and movement compared to exercise group at AT and 1 month AT and VAS movement compared to pulsed phonophoresis group at AT. As a result, we found that all treatments were effective in the treatment of symptomatic primary knee osteoarthritis. However, continuous phonophoresis treatment was shown to have a partial superiority in pain parameters.
