Obstrüktif uyku apne
sendromu (OSAS) uyku sırasında tekrarlayan üst solunum yolu obstrüksiyonu
epizodları ve sıklıkla kan oksijen satürasyonunda azalma ile karakterize bir
sendromdur. OSAS’da tekrarlayan apne ve hipopne atakları hipoksi, hiperkapni ve
asidoza neden olur. Ortaya çıkan değişiklikler ise hemodinamik sistemde önemli
değişikliklere yol açarak kardiyovasküler, nörovasküler, metabolik bozukluklara yol
açabilir. Hipoksi sonucu oluşan endotelyal disfonksiyon, OSAS’ın vasküler
komplikasyonlarında önemli rol oynar. Bu nedenle OSAS hastalarında diğer risk
faktörlerinden bağımsız olarak serebrovasküler hastalık riski artmaktadır. Bu
çalışmada, OSAS tanılı hastalarda TCD nefes tutma testi kullanarak azalmış serebral
vasomotor reaktiviteyi göstermek ve stroke ile arasındaki ilişkiyi bulmak
amaçlanmıştır. Çalışmaya 150 olgu alınmış olup, 110’una TCD ile nefes tutma testi
yapıldı. 19 kontrol, 91 orta veya ağır OSAS hastası ile çalışma sonuçları incelendi.
Serebrovasküler reaktivite, OSAS hastalarında kontrol grubuna göre düşük bulundu.
Normale dönme zamanı, OSAS hastalarında kontrollere göre düşük bulundu.
Çalışma grubunun 3.,6. ve 9. ay takiplerinde inme geçiren olgu saptanmadı. Bu
veriler OSAS’lı hastalarda nefes tutma esnasındaki uyarana karşı vazodilatör yanıtın
azaldığını göstermektedir. TCD tabanlı serebrovasküler reaktivite testiyle
değerlendirilebilen bozuk endotelyal yanıt, OSAS hastalarında inme riskini gösteren
bir prediktör olarak kullanılabilir.
Obstructive sleep apnea syndrome (OSAS) is a syndrome
characterized by a reduction frequently in blood oxygen saturation and recurrent
upper airway obstruction episodes during sleep. The recurrent episodes of apnea and
hypopnea attacks in OSAS cause hypoxia, hypercapnia, and acidosis. The resulting
changes may lead to significant changes in hemodynamic cardiovascular system,
neurovascular, and metabolic disorders. Endothelial dysfunction caused by hypoxia
plays an important role in the vascular complications of OSAS. Therefore,
cerebrovascular disease risk in OSAS patients is increasing independent of other risk
factors. In this study, using TCD breath-holding test it is aimed to find the
relationship between stroke and the reduced cerebral vasomotor reactivity in patients
diagnosed with OSAS. 150 patients participated in the study, 110 of whom were
applied breath-holding test with TCD. The results of the study were based on 19
patients of control, and 91 patients with moderate or severe OSAS. Cerebrovascular
reactivity in patients with OSAS was significantly lower than the control group. It
was found that the duration of returning to normal was significantly lower in OSAS
patients compared to controls. There were no cases of stroke detected in the study
group in the monthly observations of third, sixth and ninth. The results show that
vasodilator response to stimuli decreases during breath-holding test in the patients
with OSAS. Damaged endothelial response, which can be assessed by TCD-based
cerebrovascular reactivity test, can be used as a predictor indicating the risk of stroke
in OSAS patients.
