Bu çalışmada
kataraktın yaşlı hastalarda kognisyona olan etkilerinin değerlendirilmesi
amaçlanmıştır. Çalışmaya katarakt tanısı ile operasyon planlanan görme keskinliği
kötü 60-80 yaş aralığında 30 hasta alınmıştır. Katılımcıların ameliyat öncesi
kognitif testleri yapılmış, operasyondan 3 ay sonra aynı testler yeniden tekrar
edilmiştir. Tüm hastalar lokal anestezi ile birer hafta arayla binokuler katarakt
cerrahisi geçirmişlerdir. Hastaların operasyondan gördükleri fayda görme keskinliği
ile karşılaştırılmıştır. Demans taraması Montreal Bilişsel Değerlendirme Ölçeği
(MOCA) testi ile yapılmıştır. Katılımcıların %46 sında demans , % 36 sında hafif
kognitif yetmezlik saptanmıştır. Demansı olan katılımcıların en çok bellek, lisan,
görsel- uzaysal bellekte; hafif kognitif yetmezlikli hastaların dikkat, yürütücü işlev
testlerinde kötüleşme izlenmiştir. Katarakt cerrahisi sonrası görme keskinliği ile
korele şekilde kognitif yetmezlikli olguların MOCA skorlarında en az 2 puan artış
izlenmiştir. Ancak görme keskinliğinde artış görsel bellek, konstruksiyonel praksi,
saat çizme, şekil kopyalama testlerindeki skorlarla korele bulunmamıştır. Hastaların
günlük yaşam aktivitesi, geriyatrik depresyon ölçeği entrümental günlük yaşam
aktivite skorları post operatif dönemde görme keskinliği ile korele şekilde
düzelmiştir. Sonuç olarak kognitif yetmezlik ve katarakt yaşlı populasyonda sık
birliktelik gösteren hastalıklardır. Görme yetisinin yeniden kazandırılması
kognisyonu olumlu şekilde etkileyebilir. Demans polikliniklerinde hastaların duysal
kayıpları ile birlikte değerlendirilmesi hastalığın takibi açısından önemlidir.
The assesment of the effects of
cataract on cognition on old patients was aimed. It was worked with 30 patients
between 60 and 80 year old whose visual acuity was not well and who was planned
to have operation because of cataract diagnosis in this study. Preoperative cognitive
tests of attenders was performed, the same tests was performed again after the
operation. Preoperative cognitive tests of attenders was performed, the same tests
was performed again after 3 months of the operation. All the patients had binocular
cataract operation under local anesthesia every other week. Benefits the patients had
were compared with their visual acuities. Dementia scanning was performed by
MOCA (Montreal Cognitive Assesment Scale). Dementia was diagnosed on 46 % oft
he attenders and mild cognitive impairment was diagnosed on 36 % of the attenders.
Deterioration on the tests of memory, speech, visual- spatial memory was observed
on the attenders who had dementia; deterioration on the tests of attention, executive
function was observed on the attenders had mild cognitive imairment. At least 2
points increase occurred MOCA scores of the cases had cognitive impairment
correlative with postoperative visual acuity. However, the increase in visual acuity
did not collerate with the scores fort he tests of visual memory, constructive praxia,
clock boot, figure copying. Daily life activities, geriatric depression scale,
instrumental daily life activtiy scores of the patients recupareted correlatively with
visual acuity in postoperation period. Consequently, cognitive impairment and
cataract are frequent diseases associated with old age population. Refunction of
visual acuity can affect cognition positively. Assesment of the patients with their
sensual losses is substantial for disease follow-up.
