Serebrovasküler hastalıklar, erişkinde ve yaşlıda nöbetlerin en sık nedenlerinden birisidir. İnme geçiren hastalarda epileptik nöbet insidansı, farklı çalışmalarda %2,3-43 gibi geniş bir aralıkta yer almaktadır. Hangi faktörlerin nöbet oluşturacağını önceden bilmek bazı önlemlerin alınması açısından önemlidir. Farklı çalışmalarda inme sonrası görülen nöbet insidansları farklı olup yakın zamanda yaygınlaşan dekompresif kraniyotomi, trombolitik tedavi ve endovaskuler girişimlerin inme sonrası nöbet ilişkisi hakkında yeterli bilgi yoktur. Bu çalışmada amacımız akut iskemik inme ile kliniğimizde takip edilen hastaların nöbet geçirme oranı, etyolojik ve prognostik faktörleri araştırmak ve yüksek riskli grupları saptamaktır. Bu çalışmaya retrospektif olarak 01.01.2012 ile 01.01.2015 tarihleri arasında Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Nöroloji Anabilim Dalı’nda akut iskemik inme tanısı ile takip edilen ve 1 yıl boyunca takibimizde kalan, inme eytolojisinin aydınlatılmış olduğu 172’si kadın 127’si erkek 299 hasta dahil edilmiştir. Hastaların ilk bir yıl içinde nöbet geçirme oranı %16,7’dir.Hastaların % 8’i erken inme sonrası nöbet, %12,7’si geç inme sonrası nöbet geçirmiştir. Yüksek riskli bulunan gruplar ise aterosklerotik damar hastalığı olanlar, kortikal tutulumu olanlar, dekompresif kraniyotomi geçirenler, mRs’ si yüksek olanlar idi. Bizim çalışmamızda trombolitik tedavi alan hastalar da değerlendirildi. Sonuçta, trombolitik tedavi almayan hastalar alanlara göre 2,6 kat daha fazla nöbet geçirmektedir. Hemorojik transformasyon, damar sulama alanı, cinsiyet, endovasküler girişim yapılması ve nöbet geçirme arasında ise anlamlı istatiksel sonuç saptanmamıştır.
Cerebrovascular diseases are one
of the most common reasons of seizures in adult and elderly people. In different studies
the incidence of epileptic seizures in patients with stroke is displayed a wide range
between 2.3% and 43%. To know which of the factors causing seizures is important in
order to take some precautions. Different studies showed different results for the
incidence of post-stroke seizures and, besides, there is not enough information about
the relationship between post-stroke seizures and recently wide spreading
decompressive craniotomy, thrombolytic therapy and endovascular interventions. Our
aim in this study is to investigate seizure rates, etiologic and prognostic factors of
patients followed in our clinic with acute ischemic stroke and to identify high-risk
groups. This retrospective study contains 299 patients, of whom 127 are men and 172
women with the diagnosis of acute ischemic stroke between January of 2012 and
January of 2015 in Eskişehir Osmangazi University Faculty of Medicine Department
of Neurology. Those patients were under our observation for totally one year and the
etiology of their stroke was clarified. The seizure rate of patients for the first year was
16,7%. 8% of patients had early seizures after stroke, and 12,7% of patients had late
seizures after stroke. The high risk groups were patients with atherosclerotic vascular
disease, cortical involvement, decompressive craniotomy, and with high score of mRs.
In our study, patients receiving thrombolytic therapy were also evaluated. Overall,
patients without receiving thrombolytic therapy have 2,6 times more seizures than
patients receiving thrombolytic therapy. No significant statistical results were detected
between seizures and hemorrhagic transformation, vessels irrigation area, gender, and
endovascular interventions.
