Bu çalışmanın amacı, idiyopatik karpal tünel sendromunda (KTS) gece splint tedavisine ilave US ve steroid fonoforezinin klinik ve elektrofizyolojik parametrelere etkisini plasebo kontrollü değerlendirmekti. 27 hasta (46 el) çalışmaya alındı. 1. gruptaki hastalara (9 hasta, 15 el) 1 MHz frekansta, 1 W/cm² sürekli US, 2. gruptaki hastalara (9 hasta, 15 el) 1 MHz frekansta, 1 W/cm² sürekli % 0,1 deksametazon fonoforez, 3. gruptaki hastalara (9 hasta, 16 el) 0 MHz, 0 W/cm² plasebo US uygulandı. Tüm hastalara 2 hafta boyunca gece istirahat splinti uygulandı. Hastalar tedavi öncesi, tedavi sonrası ve tedavi sonrası 3. ayda vizüel analog skala (VAS), Boston semptom şiddet skalası, fonksiyonel kapasite skalası, kavrama gücü ve elektronörofizyolojik parametreler (median sinir motor sinir iletim hızı, median sinir motor distal latans, 2. parmak- avuç içi arası median sinir duysal sinir iletim hızı, 2. parmak- avuç içi arası median sinir duyusal distal latans, avuç içi- el bilek arası arası median sinir duysal sinir iletim hızı, avuç içi- el bilek arası median sinir duyusal distal latans) ile değerlendirildi. Tüm gruplarda tedavi sonrası VAS, Boston semptom şiddet skalası, fonksiyonel kapasite skalası ve kavrama gücünde iyileşme saptandı, bu iyileşmenin US grubunda kavrama gücü hariç diğer klinik parametrelerde 3. ayda da devam ettiği saptandı. Elektrofizyolojik olarak ise US grubunda avuç içi el bilek arası duyusal distal latansta tedavi sonrası 3. ayda, fonoforez ve plasebo US gruplarında, 2. parmak- avuç içi median sinir duyusal distal latansta tedavi sonrası ve 3. ayda bir iyileşme tespit edildi. Sonuçlarımız her 3 tedavinin de klinik ve elektrofizyolojik parametrelerde iyileşme sağladığını, hiçbir tedavinin birbirine üstün olmadığını gösterdi.
The aim of this placebo controlled study was to evaluate the efficiency of ultrasound and steroid phonophoresis treatment combined with the night splint on clinical and electrophysiological parameters in idiopathic carpal tunnel syndrome. 27 patients (46 hands) were enrolled in the study. Patients in the group 1 (9 patients, 15 hands) 1 MHz frequency, 1 W/cm² continuous US, patients in group 2 (9 patients, 15 hands) 1 MHz frequency, 1 W/cm² continuous % 0,1 dexamethasone phonophoresis, patients in group 3 (9 patients, 16 hands) 0 MHz, 0 W/cm² placebo ultrasound was applied. All patients were applied splint at night for 2 weeks. All patients were evaluated before the treatment, after the treatment and 3 month after the treatment by visual analog scale (VAS), Boston symptom severity scale, functional capacity scale, grip strength and electroneurophysiological parameters (median nerve motor conduction velocity, median nerve motor distal latency, median nerve sensory conduction velocity between 2nd finger and palm, median nerve sensory distal latency between 2nd finger and palm, median nerve sensory conduction velocity between palm and wrist, median nerve sensory distal latency between palm and wrist). VAS, Boston symptom severity scale, functional capacity scale and grip strength improved in all groups after treatment. This improvement continued at 3 month after treatment in other clinical parameters except grip strength in US group. Electrophysiological developments were detected sensory nerve conduction velocity between palm and wrist on the 3rd month in US group; sensory nerve distal latency between 2nd finger and palm after treatment and on the third month in phonophoresis and placebo groups. Our results show that improvement is achieved in electrophysiological parameters after each one of the tree treatments and none of the treatments is superior to the others.
