Çalışmanın amacı, Ailevi Akdeniz Ateşi (AAA) hastalarında genotip-fenotip ilişkisini değerlendirmek, hastalık şiddeti üzerinde mutasyonların rolünü ortaya koymak, eşlik eden inflamatuvar hastalık olup olmadığını ortaya koymaktır. Mutasyon saptanan 239 hastanın 79ʼunda (%33,1) homozigot mutasyon, 160ʼında (% 66,9) heterozigot veya birleşik heterozigot mutasyon saptandı. Homozigot mutasyona sahip hastalarda hastalık daha erken yaşta ortaya çıkmış (8.4 yıla karşı 13.6 yıl; p<0.001) ve tanı daha erken konmuştu (23.3 yıla karşı 28.6 yıl). Hafif ve orta hastalık şiddetine sahip hasta sayısı heterozigot ve birleşik heterozigot mutasyona sahip olanlarda daha yüksek iken (p<0.05, p<0.001), homozigot mutasyona sahip olanlarda şiddetli hastalığa sahip hasta sayısı daha fazlaydı (p<0.001). Homozigot mutasyonlularda, heterozigot+ birleşik heterozigot olanlara göre EBE görülme oranı daha yüksek iken (p<0.001) diğer klinik bulgular açısından fark bulunamadı. M694V mutasyonu taşıyanlar, diğer mutasyonu olan hastalara göre EBE ve artrit geliştirmeye daha eğilimlilerdi (p<0.001, p<0.05). Heterozigot ve birleşik heterozigot hastaların verilerini ikiye ayırarak homozigot grupla ayrı ayrı karşılaştırdık. Homozigot grupta hastalık diğer iki gruba göre daha erken başlamıştı ve tanı diğer iki gruba göre daha erken konmuştu. Şiddetli hastalığa sahip hasta sayısı homozigot grupta diğer iki gruba göre daha fazlaydı. Homozigot grupta hem amiloidoz hem de EBE daha sık görülmüştü. AS hasta sayısı M694V mutasyonu olanlarda diğer mutasyonlulara göre daha fazlaydı (21‘e karşı 2; p<0.01). Diğer hastalıklar açısından farklılık bulunamadı. Sonuç olarak, Homozigot mutasyona sahip olanlarda hastalık daha erken yaşta ortaya çıkmakta, şiddetli hastalığa sahip hasta sayısı daha çok olmakta ve amiloidoz daha sık görülmektedir.
The aim of this study is to evaluate the genotype-phenotype correlation, to reveal the role of mutations on the severity of the disease, and to determine the whether there is a concomitant inflammatory disease, in patients with (FMF). 79 of the 239 mutation detected patients(33.1%) were homozygous mutations, and 160 of the patients (66.9%) were heterozygous or compound heterozygous mutations. In patients with homozygous mutation the disease arose at an earlier age (8.4 years vs. 13.6 years; p<0.001), and were diagnosed earlier (23.3 years vs. 28.6 years). The number of patients with mild to moderate disease severity was higher in patients with heterozygous and compound heterozygous mutations (p<0.05, p<0.001), whereas the number of severe disease among suffering from FMF were higher in patients with homozygous mutations (p<0.001) . The ELE incidence was higher in patients with homozygous mutation when compared with heterozygous and compound heterozygous patients (p<0.001); there was no difference in terms of other clinical findings though. M694V mutation carriers were more prone to develop ELE and arthritis than the patients with other mutations (p <0.001, p <0.05). We seperated the data of heterozygous and compound heterozygous patients; and compared with the homozygous group individually. The disease in homozygous group had started and diagnosed earlier than the other two groups. The severe disease incidence was higher in homozygous group than the other two groups. Both amiloidosis and ELE were more likely to arise in homozygous group. There were more AS patients among M694V mutation carrying patients (21 vs. 2; p<0.01). There were no differences detected in terms of other diseases. In conclusion, among patients with homozygous mutation, the disease occurs earlier in age, the number of patients with severe disease gets higher, and amyloidosis is being seen more frequently.
