Periferik sinir hasarları motor, duyusal ve otonom fonksiyonlara zarar veren; aynı zamanda da yaşam kalitesini azaltarak önemli mali kayıplara yol açan yaralanmalardır. Özellikle motorlu araç kazalarına bağlı travmalarda ve daha az sıklıkla da derin travmalarda, düşmelerde ve endüstriyel kazalarda görülmektedirler. Büyük sinir boşlukları, proksimal lezyonlar ya da kronik sinir yaralanmalarında iyileşme oldukça zordur. Merkezi sinir siteminin aksine periferik sinir sistemi önemli bir yenilenme kapasitesine sahiptir ancak bu rejenerasyon tam olarak gerçekleşmemekte ve yaralanma öncesi duruma kıyasla fonksiyonel iyileşme çok nadir görülmektedir.
Çalışmamızda hyaluronik asitin biyobozunur bir hidrojeli yapılarak bu hidrojele lityum yüklenmesi gerçekleştirilmiştir. Rat siyatik siniri üzerinde periferik sinir hasarı modeli oluşturularak; oluşan sinir boşluğuna silikon konduit yerleştirilmiş ve ve konduit lümenine de bu hidrojeller enjekte edilmiştir. 12 haftalık bekleme süreci sonunda denekler disekte edilerek rejenerasyonun ultrayapısal boyuttaki değerlendirilmeleri yapılmıştır. Sonuçta hem lityum hem lityum yüklenmiş hyaluronik asit hidrojellerinin siyatik sinir rejenerasyonu üzerine olumlu etkileri olduğu gösterilmiştir.
Peripheral nerve injury damages motor, sensorial and autonomous functions, at the same time as reducing quality of life and causing significant financial losses. It is observed especially in trauma linked to motor vehicle accidents and less frequently with deep trauma, falls and industrial accidents. With large nerve gaps, proximal lesions or chronic nerve injuries, healing is very difficult. Contrary to the central nervous system, the peripheral nervous system has a significant capacity for renewal, however this regeneration is not fully complete and improvement is rarely observed compared to the situation before injury. For example, while a wide and externally directed neuronal development is required for proximal lesions, a weak regenerative environment at the distal tips is formed in chronically injured nerves. The regeneration mechanism of damaged nerves is based on many interactions between both cellular elements and extracellular matrix.
In the present study a biodegradable hydrogel were constructed from hyaluronic acid and loaded with lithium. A defective peripheral nerve injury were made on rat sciatic nerve and bridged with a silicone conduit filled with the prepared hydrogels. In conclusion both lithium and lithium loaded groups were demonstrated positive effects on peripheral nerve regeneration.
